Ultrasonographic observations of the uterine regression in the ewe under different obstetrical conditions.

Involution of the ovine uterus after parturition was studied using real-time B-mode ultrasonography. The diameter of the uterus, the uterine lumen and the caruncles were investigated during the first 30 days post-parturition (p.p.) by transrectal and transabdominal application of the ultrasound probe. Ewes that spontaneously delivered showed a physiological regression of the uterus with a transversal diameter of 4.9 (+/- 0.86) cm on day 1 p.p. which decreased to 1.84 (+/- 0.14) cm until day 30. The course of uterine regression was established on these data and was described by the exponential equation: y = E + A * e(-b*t). 'E' means the final value on day 30 p.p. and 'A' the difference between the initial transversal diameter on day 1 p.p. minus the final value on day 30 p.p. The exponents 'b' and 't' indicate the downward gradient of the regression curve and also the factor time in days. Furthermore, we found that the uterine regression in ewes finishes approximately on day 17 p.p. The uterine involution was delayed in ewes after manual obstetrics and caesarean section, in addition the incidence for retention of foetal membranes was increased. The present study demonstrates that the ultrasonographic technique is a suitable method for the control of the uterine involution and for the diagnosis of pathological aberrations in sheep.

Combining membrane extraction with mobile gas chromatography for the field analysis of volatile organic compounds in contaminated waters.

A mobile gas chromatographic device (Airmobtx HC 1000 monitor manufactured by Airmotec, Germany), originally designed for the analysis of benzene, toluene, ethylbenzene and xylenes (BTEX) in air, was connected to a flow cell for dynamic membrane extraction. Volatile organic compounds (VOCs) diffuse out of a water stream through a hollow fibre, are enriched onto sorption tubes integrated in the mobile device, and are then thermally desorbed and analysed by gas chromatography-flame ionisation detection. Battery operation of the device enables continuous on-site analysis of VOCs. Influences of the water flow-rate on system response and memory effects were investigated. The linear range of the method depends on the flow-rate of the water sample and did not exceed two orders of magnitude. The detection limits for trichloroethene, chlorobenzene and the BTEX compounds were found to be between 0.1 and 1.0 microg/l using a water flow-rate of 30 ml/min. Dynamic membrane extraction combined with the mobile gas chromatographic device was used for the on-site analysis of contaminated waters in the area of Leipzig.

Management of ingested foreign bodies in childhood and review of the literature.

UNLABELLED: The management of ingested foreign bodies in children is not standardised. During a 15-year period, we recorded 325 consecutive paediatric cases of accidental ingestion of foreign bodies or with symptoms suggesting oesophageal obstruction presented at the emergency department or the paediatric gastroenterology unit. The foreign bodies that had to be removed were, in decreasing order of frequency: coins, toy parts, jewels, batteries, sharp materials such as needles and pins, fish and chicken bones, and "large" amounts of food. Only 54% of the patients had transient symptoms at the moment of ingestion, such as retrosternal pain, cyanosis and dysphagia. A minority (28, 9%) of foreign bodies could be removed with a McGill forceps; 65 (20%) were removed with a magnet probe. Endoscopic removal was performed in 82 cases (25%). In the majority of cases (150, 46%) natural elimination occurred. The outcome of all patients was uneventful. CONCLUSION: Recommendations for management of children presenting with a history of suspected accidental ingestion of a foreign body for the community paediatrician are proposed.

Sebaceous adenitis in the Akita: clinical observations, histopathology and heredity.

Ninety-seven pure-bred Akitas were examined clinically and histologically for sebaceous adenitis. The diagnosis was established histologically in 23 Akitas by demonstrating an inflammatory reaction targeted against the sebaceous glands or a reduction in the number of glands. The clinical course of sebaceous adenitis in the Akita was similar to that seen in other breeds. The first skin lesions occurred mainly on the dorsal midline and ears. Compared with the Poodle, the age at first onset of the disease was more variable and the hair loss affected mainly the undercoat. The progression of sebaceous gland destruction varied between dogs and was not seen in all cases. Because bud-like sebaceous gland proliferation could be identified, it seems that regeneration of the sebaceous glands may occur. An autosomal recessive inheritance appears to be possible. Apart from a genetic background, immune-mediated factors possibly influence the onset and course of sebaceous adenitis.

Epidermal dysplasia and Malassezia infection in two West Highland White Terrier siblings: an inherited skin disorder or reaction to severe Malassezia infection?

Two 9-month-old West Highland White Terrier siblings were referred to our clinic with pruritus, alopecia and lichenification. Cytological examination of Scotch tape strippings revealed Malassezia organisms and cocci. Skin biopsy specimens showed epidermal dysplasia. Treatment included bathing with a 2% miconazole/chlorhexidine-containing shampoo, orally administered ketoconazole (5 mg kg-1, every 12 h) and cloxacillin (25 mg kg-1 every 8 h). Six weeks later, the dermal infection had resolved and there was hair regrowth. However, the dogs were still moderately pruritic. Intradermal allergy testing was positive for house dust mites, storage mites and Malassezia. Immunotherapy was initiated, and treatment with ketoconazole and cloxacillin was stopped. Skin biopsies, which were performed in both dogs 4 months after the first presentation, revealed mild superficial perivascular dermatitis. The remaining mild facial pruritus was easily controlled with topical treatment. These two cases indicate that epidermal dysplasia might be an inflammatory or hypersensitivity reaction to the Malassezia infection or a result of excessive self-trauma, rather than a congenital keratinization disorder.

PH monitoring in the distal and proximal esophagus in symptomatic infants.

BACKGROUND: Standard distal esophageal pH monitoring data are sometimes within normal ranges in children with clinically suspected gastroesophageal reflux disease. Therefore, the authors hypothesized that the amount of acid reflux reaching the proximal esophagus may be greater in some subgroups of patients than in healthy controls or in other subgroups of patients. METHODS: The parameters of 24-hour pH monitoring in the proximal part of the esophagus were analyzed in 120 symptomatic infants in who the reflux parameters in the lower esophagus were clearly within normal ranges (reflux index < 5.0%). The infants were classified into four patient groups: excessive regurgitation (n = 41): inconsolable crying (n = 31), apparent life-threatening event (ALTE) (n = 18), and chronic respiratory disorders (n = 30). The control group consisted also of 120 infants. The following parameters were calculated: reflux index, the number of reflux episodes, the number of reflux episodes lasting longer than 5 minutes, the duration of the longest reflux episode, and the acid clearance time (ACT, duration of reflux episodes divided by number of reflux episodes). RESULTS: The patients with chronic respiratory disorders were significantly older than the patients in the other groups and the controls. In the distal esophagus, there was no statistically significant difference between the reflux parameters. As could be expected, every parameter was statistically (paired t test, Wilcoxon signed-rank test) significantly smaller in the proximal than in the distal esophagus, except for the ACT in infants who presented with inconsolable crying. In the proximal esophagus, there was no statistically significant difference between the different patient subgroups or controls, except for the number of reflux episodes in the group with chronic respiratory disorders and the group with inconsolable crying, applying one-way analysis of variance. As determined by applying the Mann-Whitney test, the number of reflux episodes in the upper esophagus was significantly higher in the group with chronic respiratory disorders than in the other patient groups and controls. Therefore, the authors' data do not support the hypothesis that reflux reaching the proximal esophagus is a frequent cause of ALTE. However, the data may suggest that the number of reflux episodes reaching the proximal esophagus in children with chronic respiratory disorders and with distal pH monitoring data within normal ranges may be increased. Whether this finding reflects reality or a statistical coincidence, or is influenced by the older age of this patient group, needs further evaluation. CONCLUSIONS: In theory, dual simultaneous esophageal pH monitoring in the distal and proximal esophagus may increase the diagnostic accuracy of pH monitoring in infants. Our results do not support a substantial advantage of a systematic application of this new technique, especially not in infants presenting with ALTE, excessive regurgitation, or inconsolable crying. In the subgroup of patients with chronic respiratory disorders, more data are needed before conclusions can be determined and recommendations can be made.

Efficacy and tolerability of cisapride in children.

As gastro-oesophageal reflux disease (GORD) in infants and children is a motility disorder which differs in pathophysiology and clinical course from GORD in adults, prokinetics should be considered the drug of choice in certain circumstances. Indeed, cisapride may result in improvement of feeding tolerance in premature infants. Cisapride has a better tolerability profile than a 'wait-and-see-if-improvement-comes-spontaneously' policy or the other therapeutic options available. A careful and critical review of published data suggests that cisapride may have a QTc-prolonging effect. However, provided the precautions for cisapride administration are followed, the QTc-prolonging effect remains consistently without clinically relevant adverse effects. Correct dosage and avoidance of concurrent treatment with macrolides and/or azoles are the most relevant tolerability recommendations in children. Although there is a need for a prokinetic with better efficacy, cisapride is currently the prokinetic with the best benefit-to-risk ratio available. Thus, withdrawal of cisapride would result in a significantly increased risk for severe complications in infants and children with GORD or other gastrointestinal motility disorders such as chronic intestinal pseudo-obstruction, gastroparesis and feed intolerance in premature infants.

[The health professional's approach to chronic lumbago: clinical experience at the Spinal Cord Unit at the orthopedic hospital].

The treatment of back pain patients refers to the biopsychosocial model of care. This model includes illness in patient's personal and relational life. In this context, it is not only the physical symptom of the patient which is focused but also his psychological distress often hidden by algic complain. Clinical interviews conducted with back pain patients have highlighted psychosocial aspects able to influence the relationship between health care user and provider. Taking account of psychosocial aspects implies an interdisciplinary approach that identify and assesses patients' needs through adequate tools. As a result, the different health care providers implied with back pain patients have to collaborate in a structured network.

Chronic Disease↗

[Flesh-eating bacteria infection of an immunocompromised patient].

After years of steadily declining morbidity and mortality due to group A streptococcal infections, a resurgence of severe, invasive disease has been ongoing since 1980, leading to the recognition of streptococcal shock syndrome (STSS), necrotizing fasciitis, the most severe form of invasive infection. The patients suffer from rapid local deep soft tissue destruction, severe septic shock and multi organ failure. The increased incidence of these infections has been accompanied by remarkable vigor in virulence and severity of the disease. The reason for this impressive change in the epidemiology and clinical manifestation of group A streptococcal infections remains unknown. The possible etiological factor is changing in virulence factor or the lack of protective immunity of the population (immunocompromise) against the invasive strains. We describe a severe necrotizing fasciitis of a 41-year-old previously immunocompromised woman. The patient developed severe septic shock, multi organ failure and perineal and lower abdominal skin, fat and fascia necrosis due to mixed GAS (aerob, anaerob) infection of the perineum and the Bartholini glands. After an aggressive surgical debridement, antibiotic and supportive therapy the generalised and local infection was treated.

[Malignant catarrhal fever in Switzerland: 2. Evaluation of the diagnosis].

Malignant catarrhal fever (MCF) is a mostly fatal lymphoproliferative disease of cattle. In 1995 a PCR based method was introduced for the detection of the ovine herpesvirus 2 (OvHV-2), which is regarded as the causative agent of the sheep-associated form of the disease. This PCR can be regarded as a gold standard for the in vivo diagnosis of sheep-associated MCF in cattle (Müller-Doblies et al., 1998). This semi-nested PCR was now used as a reference test for the reassessment of diagnostic criteria in the clinical and post mortem diagnosis that could previously not be quantitated. Based on 83 suspected cases with a complete clinical record the clinical signs were weighted and grouped according to their sensitivity and specificity into lead signs indicative of MCF and frequently accompanying signs supportive for the diagnosis of MCF and general clinical signs that were less reliable for the diagnosis. Differential diagnoses are discussed, which are of particular significance due to their status as OIE list A diseases e.g. foot-and-mouth disease or rinderpest. 38 PCR confirmed cattle with MCF served for the quantitative analysis of organ lesions. For the post mortem diagnosis an essential set of organ samples is defined to permit a reliable histological diagnosis, as the gross pathology often did not give any indication for the diagnosis. These criteria should help to improve the diagnostic efficiency and to select the appropriate laboratory diagnostic procedures for MCF-suspected cattle.

A male to female sex-reversed dog with a reciprocal translocation.

A registered female Yorkshire terrier aged 3 years was diagnosed as intersex. Molecular and cytogenetic studies were carried out to determine whether chromosomal abnormalities were involved in the development of this condition. Analysis of the testicular tissue in the gonads and PCR amplification of three canine Y chromosome-specific DNA sequences from the genomic DNA of this animal indicated that the dog had a genetic complement with a Y chromosome. An analysis of > 100 banded metaphase chromosome spreads of this intersex dog showed uniformly a 78,XY chromosome complement and confirmed that the animal might be a male to female sex-reversed dog. However, two different types of X chromosome were observed. A whole chromosome painting probe was developed by microdissection of ten X chromosomes from the intersex dog and used for fluorescence in situ hybridization experiments on metaphase chromosome spreads from a normal male dog. Hybridization signals were detected on the X chromosome and on an autosome (not yet internationally standardized) indicating that a reciprocal translocation event had occurred. The karyotype of this mosaic sex-reversed dog was designated tentatively as 78,XY/78,XYrcp (X; autosome). This is believed to be the first report of a canine intersex dog showing male to female sex reversal. Further investigations should identify the autosome and the breaking points of the chromosomal rearrangement. These investigations potentially add to our knowledge of autosomal and X-linked loci involved in sexual differentiation of mammals.

Gastro-oesophageal reflux, sleep pattern, apparent life threatening event and sudden infant death. The point of view of a gastro-enterologist.

UNLABELLED: Sudden infant death, apparent life threatening events and regurgitation occur mostly during the first 6 months of life, making it very tempting to suspect a relation between these phenomena. According to epidemiological data, the role of gastro-oesophageal reflux in the aetiology of apparent life threatening events is unclear. It has been demonstrated that sudden infant death is decreased in the supine versus the prone sleeping position. On the contrary, gastro-oesophageal reflux is more pronounced in the supine than in the prone position, both in infants and in older children, both in infants with physiological and with pathological reflux. In the supine position, infants do sleep shorter, have more rapid-eye-movement sleep and have more arousals than in the prone position. CONCLUSION: It is thought that in the majority of infants, gastro-oesophageal reflux stimulates arousals and thus may well be considered as "protective" for rather than "provoking" sudden infant death. However, this hypothesis needs to be validated.

Glucagon-producing neuroendocrine tumour associated with hypoaminoacidaemia and skin lesions.

A nine-year-old neutered female crossbred Bernese mountain dog was diagnosed with superficial necrolytic dermatitis and a glucagon-producing islet cell tumour. Laboratory findings included hyperglucagonaemia and hypoaminoacidaemia. The dog was euthanased because of progression of the disease, and necropsy revealed liver metastases of a neuroendocrine carcinoma with immunohistochemical expression of glucagon and somatostatin. This report represents a case of canine glucagonoma syndrome; the previously reported cases in dogs are also briefly described.

[Radiation therapy in two cats with pituitary tumors].

Two cats with large pituitary neoplasms (adenoma and adenocarcinoma) were treated with fractionated radiation therapy. Total doses of 40 Gy, respectively 36 Gy, were applied in 10 fractions of 4 Gy, and 3.6 Gy respectively. Side effects were minimal and transient. Anesthesia was well tolerated. Improvement of clinical signs could be observed during radiation therapy in both cats. One cat had a complete, the other a partial tumor response. One cat (suspicion of adenoma) was euthanized 1 3/4 years after therapy due to unrelated disease. No tumor was found on histopathology, however a small focal necrosis of brain tissue in the irradiated field was observed. The second animal with a pituitary adenocarcinoma was euthanized because of tumor recurrence 1 1/2 years after therapy. Radiation therapy was effective, despite the low total doses of radiation applied.

Sry-negative XX true hermaphrodite in a Basset hound.

A true hermaphrodite was diagnosed in a 7-mo.-old Basset hound. The diagnosis was based on the clinical signs, the histology of the gonads and the karyogram. Additionally, the dog was tested for the Y-linked gene Sry, which was negative. The Basset hound presented here is compared to other XX sex reversed animals described in the literature. In man, XX sex reversal is a heterogenous condition. The pathogenesis in Sry-negative individuals is not understood. Thus Sry-negative animals could serve as an animal model of the human disease.

Nutritional secondary hyperparathyroidism in six cats.

Nutritional secondary hyperparathyroidism (NSH) was diagnosed in six cats during a three-year period, based on clinical, radiographic and laboratory findings. Clinical signs were attributable to severe osteopenia (n = 5) and hypocalcaemia (n = 4), which had resulted in spontaneous fractures of long bones, scapulae, pelvis, nasal bones, or spine, and in excitation, muscle twitching or seizures, respectively. Serum parathormone levels were markedly elevated, and 1,25(OH)2-vitamin D3 mildly elevated, whereas 25(OH)-vitamin D3 was mildly decreased compared to age-matched healthy cats. Treatment was limited to short-term parenteral calcium gluconate injections, as clinically indicated, a balanced diet and cage rest, which resulted in quick clinical recovery in four cases. The remaining two cats had to be euthanased because of progressive neurological deficits secondary to spinal fractures. At the time of writing, a multitude of commercial balanced diets is widely available and diseases secondary to dietary deficiencies have become rare. Nevertheless, NSH is still an important clinical entity, and should be considered in growing cats presenting with spontaneous fractures or seizures.

Potential tumour doubling time: determination of Tpot for various canine and feline tumours.

Spontaneous tumours in dogs and cats are an excellent model for clinical human research, such as in developing proton conformation radiotherapy for humans. The kinetics of tumour cells can be used effectively to predict prognosis and response to therapy in patients with tumours. Knowledge of the kinetic parameters in these tumours is therefore important. In the present study the kinetic parameters evaluated included the labelling index (LI), relative movement (RM), mitotic index (MI), and potential doubling time (Tpot). These parameters were determined using in vivo labelling with bromodeoxyuridine, flow cytometry and histological preparation. Samples were obtained and evaluated from 72 dogs and 20 cats, presenting as patients in our clinic. Within the groups of epithelial and mesenchymal tumours from dogs and cats, the kinetic parameters LI, RM and MI were compared with Tpot. Significant correlations were observed for the comparison Tpot and LI. No correlation was found between Tpot and RM.
