[The treatment of fibular ligament lesions using the Ortho-Rehab shoe].

In a prospective study we examined 46 patients (14-49 years of age) with fresh ruptures of the lateral ligaments of the ankle (talar tilt greater than or equal to 10 degrees). Treatment consisted in six weeks of taping combined with wearing of a special rehabilitation shoe (Ortho Rehab). 28 patients were treated conservatively and 18 had an operative repair of their ruptured ligaments. After 8 weeks every patient was clinically controlled. Until now 35 patients were reevaluated more than one year after injury. Treatment was judged as very comfortable and positive. Working capacity was achieved after an average of 14.8 days and all patients returned to their former sports activity 3 to 8 weeks after the event. Mechanical and functional stability after more than one year are excellent with the exception of 3 cases. The results are discussed and compared with other studies.

Preoperative arthroscopic assessment of fractures about the shoulder.

Soft tissue lesions in fractures about the shoulder are not completely defined by conventional imaging procedures. The purpose of the present study was to arthroscopically determine the soft-tissue pathology of the labrum, capsuloligamentous, and rotator cuff structures associated with fractures of the proximal humerus and/or scapula and to correlate them to the fracture type. In a series of 80 fractures (52 proximal humeral fractures, 20 fracture-dislocations, 8 glenoid or scapular fractures) arthroscopy was performed through a posterior approach under general anesthesia. Arthroscopy revealed significant numbers of labral, capsuloligamentous, and rotator cuff lesions, as well as cartilage damage not identified by clinical examination or preoperative imaging. Fracture dislocations (Neer type VI) and more benign fractures of the two-part variety are frequently associated with labral lesions (in 56% and 31%, respectively). This data suggest that it is important not to underestimate soft-tissue pathology in fractures that seem radiologically relatively harmless. Arthroscopic assessment in shoulder fractures is shown to be a useful tool in completely understanding the extent of the injury.

[Sub-Achilles bursitis--a biomechanical analysis and clinical study].

Chronic inflammations of the distal Achilles tendon correspond frequently to a mechanical irritation of the subcalcaneal bursa. On grounds of the biomechanical analysis an excessive inward bending of the hindfoot is postulated as the reason. So a conception for the clinical therapy is detected. The first measure will be a statics correction of the foot and a local application of antalgics. In case of unsuccessfulness after 3 months the surgical resection of the calcaneus exostosis is performed. After 3 months of conservative treatment 15 of 24 athletic patients are free of pain. The other 9 patients are free of pain 3 months after the surgical treatment. In either case all patients return to their former sports activity 6 months after the beginning of the treatment.

Skier's thumb--the significance of bony injuries.

In a retrospective study to determine the anatomic nature of injuries in thumbs that were treated surgically for either fracture or instability, we reviewed 63 consecutive patients with acute skier's thumb injury. Of the 63 thumbs, 25 (40%) had a fracture. Surgical exploration showed 2 fracture types: a fragment that was attached to the ulnar collateral ligament, and a fragment that was not attached to the ulnar collateral ligament. The 1st type, corresponding to true avulsion fracture of the ulnar collaternal ligament, was found in 8 cases; the same fracture type was seen in another 7 cases, with an isolated fragment that was not attached to the ligament. Such an isolated fragment was observed in 10 other cases in which the ulnar collateral ligament was completely disrupted. This type of bony fragmentation cannot be differentiated from a bony avulsion of the ulnar collateral ligament on routine films. Therefore, stress testing the injured thumb is mandatory even when bony avulsion fracture with minimal displacement is suspected from a radiograph, as indeed the fracture may not be a bony avulsion but may be a fragmentation of the ulnar volar aspect of the proximal phalanx associated with a complete disruption of the ulnar collateral ligament.

Arthroscopic findings after shoulder dislocation.

The purpose of this study was to evaluate prospectively the arthroscopic findings of the unstable shoulder, to provide insights into the causes and mechanisms of shoulder instability, and to establish a rationale for using special surgical procedures. Arthroscopic examination was performed on 212 patients who had at least 1 documented shoulder dislocation. Of these 212 patients, 184 (87%) patients had anterior glenoid labral tears, 168 (79%) patients had ventral capsule insufficiency, 144 (68%) patients had Hill-Sachs compression fractures, 116 (55%) patients had glenohumeral ligament insufficiency, 30 (14%) patients had complete rotator cuff tendon tears, 26 (12%) patients had posterior glenoid labral tears, 14 (7%) patients had superior labrum anterior and inferior lesions. As this prospective study shows, multiple morphologic changes are associated with instability of the glenohumeral joint; there is no single cause for an unstable shoulder. Arthroscopic examination of the shoulder before surgery revealed a significant amount of information that would have been undetected without the aid of expensive diagnostic tools. For instance, the labrum and rim of the anteroinferior glenoid showed typical abnormalities corresponding to different entities of anterior instability.
