[Sonography and computed tomography in postoperative abdominal abscesses].

After abdominal surgery there is a 1.3% incidence of early relaparatomy. Peritonitis is the most frequent underlying disease, with a mortality rate of 58%, if treatment is attempted by explorative surgery. On the basis of 24 postoperative abdominal abscesses out of 1023 laparatomies during an 18-month period sonography and CT scanning proved to be most accurate in detecting postoperative inflammatory lesions despite the lack of specific criteria. Thus, both methods reduce the risks of relaparatomy by facilitating the surgical approach on the one hand, and on the other hand, they alternatively provide the possibility of percutaneous puncture or drainage.

Primary and secondary structure specificity of the cleavage of 'single-stranded' DNA by endonuclease Hinf I.

The interaction of endonuclease Hinf I with single-stranded fd DNA was examined. The sizes of the cleavage products indicate that the enzyme cuts this substrate at the same sequences as double-stranded DNA (GANTC). To determine whether or not the recognition sites in single-stranded DNA have to be present in double-stranded form in order to be cleaved, DNA fragments containing complementary or non-complementary Hinf I sequences were prepared and treated as substrates. The results suggest that completely base-paired recognition sites are necessary for cleavage. Sequences surrounding the Hinf I pentanucleotides significantly modulate the reaction rates.

[Hemangio-endothelioma of the liver (case report) (author's transl)].

The infantile hemangio-endothelioma (IHE) of the liver, a rare neoplasm, may lead the clinician and the surgeon to suspect the presence of a malignant tumor, whereas histologic examination often reveals a benign condition; our case report illustrates this discrepancy in a 3 1/2-month-old girl suffering from multiple nodules in the liver. Conservative steroid treatment resulted in a marked reduction in tumor size; the girl is in good health more than two years after diagnosis. More than 80% of hepatic IHE are diagnosed during the first six months of life. Hepatomegaly, congestive heart failure and hemangiomas of the skin combine to the classic symptomatic triad. Since the mortality is high in untreated cases, early diagnosis is of importance. IHE may regress spontaneously. Therefore, conservative therapy, such as steroid medication and treatment of the secondary, tumor-induced complications appear indicated. Alternatively, embolization and/or resection of the tumor may be considered. Signs of cardiac or hepatic insufficiency at diagnosis and a low degree of histologic differentiation are of unfavorable prognostic significance.

Enzymatic synthesis, ligation, and restriction of DNA containing deoxy-4-thiothymidine.

Phage fd RF I DNA1 about 90% substituted by deoxy-4-thiothymidine (s4Td) in the codogenic strand was synthesized by the simultaneous actions of DNA polymerase I and DNA ligase. While the rate of DNA synthesis was considerably reduced, the yield the rate of DNA synthesis was considerably reduced, the yield was not affected in the presence of s4TdTP. The conversion of RF II to RF I DNA by DNA ligase was even improved. This effect seems to be related with an altered ratio of affinity of polymerase and ligase for the s4Td-containing substrate. The presence of the base analogue in the DNA was verified independently by chromatographic and spectroscopic methods. The modified genome could be cleaved by restriction endonucleases Hpa II (C/CGG)d and Taq I (T/CGA)d. A number of the fragments produced showed altered mobilities under the conditions of polyacrylamide gel electrophoresis.

On the influence of thymidine analogues on the activity of phage fd promoters in vitro.

RF I DNA of phage fd containing 5-bromo-deoxyuridine (br5Ud) or deoxyuridine (Ud) instead of deoxythymidine (Td) inthe codogenic strand was synthesized in vitro. The modified genomes could be cleaved by restriction endonuclease Hpa II. Although the recognition site of Hpa II is CCGG, the cleavage rate was significantly reduced with Ud-containing DNA. Both base substitutions altered the mobilities of several DNA fragments under the conditions of polyacrylamide gel electrophoresis. The fragments containing binding sites for RNA polymerase were assayed for the rates of stable complex formation. The substitution of Td for both, Ud and br5Ud, strongly influenced this parameter. Thus the methyl group of Td has to be regarded as one of the sites in DNA which determine the rate of stable RNA polymerase binding and thereby possibly mediate promoter activity in vitro (24,25,26). In most cases the rate of complex formation was decreased by Ud, but increased by br5Ud.

[X-ray diagnosis of pancreatic carcinoma].

On the basis of 27 periampullary carcinomas and 3 carcinomas of the body and tail of the pancreas observed in the last 18 months, the authors summarize their experience with various invasive and noninvasive radiologic techniques and express their views on the diagnostic approach to these tumors.

[The value and limitations of sonography and angiography in the investigation of renal masses (author's transl)].

The diagnostic value of ultrasound and angiography is compared in 80 renal masses. Whereas the overall accuracy is approximately the same (95%), there are inherent limitations to both methods. As to sonography, the limiting factors consist of size, complex echopattern and lack of tissue distinction. Angiography, on the other hand, has its limitations in hypovascular tumors as well as in benign lesions simulating malignancy (abscess, hamartoma).

[The importance of sonography in bile duct surgery].

Ultrasonographic examination was carried out on 38 patients suffering from obstructive jaundice. Diagnoses after these examinations were compared with the intraoperative findings: 37 of the 38 obstructions of the bile ducts had been diagnosed and located correctly. No such success was obtained in the identification of the aetiology. Of 15 cases of gall stones 9 were identified and of 20 tumors 15 were determined. A skilled examiner performs the examination in less than half an hour. The method is not invasive and may be applied also on patients with severe malfunction of the liver. Even high risk patients can undergo this examination without undue sufferings.

[Sonography and phlebography in the investigation of the suprarenals (author's transl)].

Twelve patients were examined by sonography and phlebography; of these, ten had suprarenal tumours and two suprarenal hyperplasia. The value of sonography and its place compared with radiological examinations is discussed. Sonography is an informative method of screening and should be employed whenever there is clinical suspicion of enlargement of the suprarenals before other radiological methods are used.
