[Circulating platelet aggregates in patients with acute ischemic and so-called chronic cerebrovascular disorders].

A modified platelet function test was assessed as a routine method in 71 normal subjects and 96 patients. In patients with cerebral infarction (33) and those with transitory ischaemic attacks (30) definite changes in platelet function were demonstrated. In patients with cerebrovascular insufficiency (33) there was significantly increased platelet aggregation, but in only 46% of these patients were the values over 3 standard deviations above the mean of the control group (71).

[Calcium blockers in the therapy of vertebrobasilar insufficiency. Results of animal experiments and clinical studies].

Vertebrobasilar insufficiency (VBI) is a clinical syndrome which includes different transient brain stem symptoms. In addition to anamnesis and clinical study electronystagmography (ENG) is regarded as the most important method for testing the labyrinthine and brain stem system. Typical findings, not only in patients with VBI, but also in animal experiments (occlusion of one vertebral artery) are an increase in nystagmus frequency and decrease in nystagmus amplitude. The present study concerns two calcium antagonists: flunarizine and nimodipine. During tests on patients the caloric and the rotatory nystagmus and during animal experiments the rotatory nystagmus was investigated. Patient tests as well as animal experiments showed, that flunarizine and nimodipine have a depressant influence on the pathologically increased nystagmus frequency. With both substances there was also a trend to normalization of nystagmus amplitude as well as an improvement of clinical symptoms.

[The usefulness of dexamethasonephosphate in the conservative treatment of lumbar pain--a double-blind study (author's transl)].

Analgesic drugs, physiotherapy and corticosteroids are concurrently used in the conservative treatment of lumbar pain. In 100 patients a double-blind study with dexamethasonephosphate versus placebo was performed. Lumbar pain and no findings with myelography were the basic requirements for inclusion into the study. There was no significant difference between the treatment group and the placebo group concerning the clinical investigation before and after treatment. The positive effect of dexamethasonephosphate, which was seen in a pilot study, was not confirmed in the double-blind study. Because corticosteroids are also psychotropic drugs there is a mild antidepressive effect. Considering the fact that cortisone has no effect on the symptoms of lumbar pain, it should not be used furthermore in the conservative treatment of lumbar pain.

[Maintenance of a steady level of vigilance during the electronystagmographic investigation (author's transl)].

During electronystagmography (ENG) a magnetic tape is used and with it simple arithmetical problems including the solutions are presented in time intervals of 15 seconds, the solutions being correct or incorrect in a random succession. The test person holds a simple electric bell in this hand having been instructed to press the bell-button once or twice, depending whether the announced solution is correct or not. During the recording of the ENG these signals are made visible as rectangular impulses on a separate trace, thus allowing control over the degree of cooperation of the test person. To prove the value of the described procedure 20 normal persons were investigated after sleep deprivation of one night with and without presentation of arithmetical problems, both investigations changing alternately. The evaluation of the parameters of ENG clearly demonstrates a significant difference in both experimental procedure, particularly in respect of frequency and angular velocity at the slow nystagmus phase. The described method has not only proved valuable for increasing vigilance but also not for maintaining a steady level of vigilance during the electronystagmographic investigation.

[Treatment of vertebrobasilar insufficiency with flunarizine (author's transl)].

Vertigo, buzzing in the ear and other brain stem symptoms are often complained of in connection with disturbed blood supply in elderly patients. These can be objectified by means of an electronystagmography (ENG). In vertebrobasilar insufficiency (VBI), a large number of central pathological ENG findings are determinable in the sense of a general deviation of the parameters. In a pilot study using clinical and apparative otoneurological methods, we systematically examined the labyrinth-depressive effect of a new substance, flunarizine, a further development of cinnarizine. 20 patients (14 men and 6 women, average age 59.8 years) with the cardinal symptom "vertigo" and one or more further symptoms of VBI were examined over a period of 8 weeks. In all cases, the dose was 20 mg/d. An otoneurological examination and questioning were carried out before starting the therapy, after 4 and 8 weeks. Symptoms such as vertigo, otoneurological and vegetative symptoms, vision disorders, other cerebral nerve failures, etc., were recorded and scaled. The objective examination methods at the assessment intervals were ENG, EEG and Doppler sonograph. In 17 of the 20 patients examined, the symptoms of VBI improved considerably, which was objectified by simultaneous normalisation of the ENG findings. 6 patients described their condition after treatment as very good, 9 as good and 2 as satisfactory. Flunarizine proved to be an effective and applicable drug in vertigo of vascular origin in elderly patients.

[Central nystagmus writing during the pendular test. Findings on post-punctural symptoms after lumbar puncture (author's transl)].

The central nystagmus writing (ecriture centrale) during the pendular test is characteristically found in cerebral organic or functional lesions. Thirty patients with symptoms following lumbar puncture (L.P.) were investigated with the pendular test. In 46.6% central nystagmus writing was found 3 days after L.P. After subsiding of post L.P. troubles all the nystagmograms became normal. A control group of 30 patients without symptoms after L.P. did not show patholgic variations in the nystagmus writing during the pendular test.
