[Classic versus laparoscopic appendectomy].

The authors analyze a cohort of 1,831 appendectomy interventions, performed in the years 1996 to 2001 at the Surgery Ward in Prostĕjov. The laparoscopic technique of the intervention was introduced into the ward practice in 1995. The evaluation of patients operated on advanced appendicitis by laparoscopy in 1996 and those operated on in 2001 with a differential approach it became obvious that septic complications were markedly reduced when a differential indication for laparoscopic intervention was applied.

[Feeding jejunostomy and protective ileostomy in acute necrotizing pancreatitis].

The authors emphasize the complexity of the problem of treating acute necrotizing pancreatitis. On their own group of 52 operated patients during 1995-2000 they present the gradual, in 2000 dramatic, improvement of results in which participate in addition to modern therapeutic procedures also the concentrated interest in the problem and the personal experience of the surgeons.

[Electrocoagulation--a curative method in the treatment of rectal carcinoma].

The authors analyze a group of 27 patients with carcinoma of the rectum who were treated in the authors' department in 1975-1990 by electrocoagulation. The authors emphasize the view of local treatment of rectal carcinoma in conjunction with modern findings on biological properties of this tumour and with regard to clinical results published in the world literature. The prerequisite of success of the method is high standard technical equipment of the department. The achieved results in the group of treated patients (66% five-year survival, 50% ten-year survival) despite the possible error of small numbers justify the authors' conviction that under conditions of strict selection and perfect technical equipment this method holds it place in the treatment of carcinoma of the rectum.

[Reoperation in colorectal surgery].

Because of unsatisfactory results of surgical therapy in rectal carcinoma the authors have re-evaluated their attitudes and elaborated resection methods of the rectum with retaining the system of sphincters trough atypical approaches: abdominotransvaginal in female and anterior abdominoperineal in male patients. They have also worked out the methods of coloanal anastomosis with passing through by trans-sphincter approach from posterior colpotomy or transperineally. These new access ways have been used in reconstructive operations after rectal resections performed previously according to Hartmann with terminal sigmoideostomy. Physiological passage through the rectum was restored till 3 weeks. 55 patients underwent the reoperation, 47 of them with excellent and good results (i.e. 85.5%) and the patients have returned to the normal social and working process.
