IRS eases rules for physician representation on governing boards.

In September 1996, the IRS eased its policy prohibiting extensive physician representation on governing boards of tax-exempt integrated delivery systems (IDSs). The IRS now allows increased physician participation on governing boards if the organizations institute conflict-of-interest safeguards that prohibit physicians from obtaining private inurements. A section of the Internal Revenue Code, enacted in 1996 by the Taxpayer Bill of Rights 2, strengthens the IRS's enforcement policy by providing targeted sanctions against individual violators of the private inurement rule rather than penalizing entire IDSs and the communities they serve by revoking their tax-exempt status.

The Hamilton (McMaster) psychiatric network: the evolution of an integrated network of psychiatric services.

For 25 years, the Hamilton-Wentworth region has had a well integrated network of psychiatric services. The initial impetus for its establishment came from the founders of the Department of Psychiatry at McMaster University in 1967. They envisaged a regional network of services that integrated the resources of a community-focused university department with those of local community agencies and other mental health care professionals. Over the years, the evolution of the network has been shaped by changes in the size and composition of the faculty, the emergence of additional clinical services and community programs, new directions in the field of psychiatry and changing economic forces and social values.

Psychiatric networks: they make sense, but do they work?

With a trend toward coordinated networks of mental health services, it is necessary to be able to assess their impact. This paper outlines an approach to network analysis, using a variety of methodologies to come up with a composite picture. Areas to examine include the network processes, such as its goals, functions, structures, outcomes, and the satisfaction of all involved.

The prevalence of hand-mirror cells in acute myeloblastic and lymphoblastic leukemia.

To evaluate the prevalence and significance of hand-mirror cells (HMC) in acute leukemia, 214 cases were studied retrospectively. They were classified by FAB and included 102 pediatric and 112 adult cases. Greater than 10% HMC were found in 6.5%. No overall difference was noted with age group, though HMC were more common in lymphoblastic cases (10% versus 3%). However, the greatest number of HMC were found in four adult cases, three of which were myeloblastic. Of the lymphoblastic cases with greater than 10% HMC, T and B studies were done in three that were typed as one T cell and two non-T cell (L1, CALL+; L3, sIg+). Forty-eight other cases with less than 10% HMC included 13 T cell and 35 non-T cell. The authors conclude that HMC may not correlate with morphologic or immune cell type and they do not appear to be a specific marker for a type of lymphoblastic and/or T-cell leukemia.

Responses of marine bacteria under starvation conditions at a solid-water interface.

Size changes during starvation of 17 marine bacterial isolates at a solid-water interface and in the liquid phase were examined. Twelve rod-shaped, hydrophilic bacteria decreased in size more rapidly at the solid surface than in the liquid phase, a result parallel to that observed previously for one of the strains at an air-water interface. On the other hand, three rod-shaped, hydrophobic bacteria diminished in size more rapidly in the liquid phase than at the solid-water interface. The rapid size decrease (defined here as the dwarfing phase) in either situation appeared to be an active process which occurred more rapidly when the cells were in an early stage of logarithmic growth at the onset of starvation. Dwarfing was reversibly inhibited by low temperature and low pH but was not inhibited by chloramphenicol. Three coccoidal bacteria showed little tendency to become smaller upon starvation in the liquid phase or at a surface.

Prevalence and characteristics of frequent attenders in a prepaid Canadian family practice.

Frequent attenders in family practice represent a small proportion of the total population, yet they consume a large amount of services. A description of their characteristics and problems is needed in order to design a therapeutic intervention tailored to meet these needs. A comparative analytic survey of 9,313 patients was done in order to isolate three cohorts: 200 zero users in the previous year, 200 modal (one to two times per year) users, and 200 frequent users (nine or more times per year). It was found that frequent attenders (who represented 4.5 percent of the practice, yet generated 21 percent of the annual visits) differ from infrequent attenders in that they have twice the probability of being single; are more physically, socially, emotionally distressed; suffer from problems of self-esteem; show a slightly greater degree of family dysfunction; tend to be unemployed, retired, or mothers of infants; are externally controlled or tend to rely on others for help; receive low incomes, retirement pensions, or family benefits; tend to be high users of other physician, social work, nursing, and laboratory services; consume a significantly greater number of pills than their infrequent user counterparts; and tend to present more emotional and gastrointestinal complaints than their modal user counterparts.

Exopolysaccharide depolymerases induced by Rhizobium bacteriophages.

Enzymes induced by two Rhizobium trifolii bacteriophages caused depolymerization of exopolysaccharides from most R. trifolii and R. leguminosarum strains tested, but did not, in general, attack the exopolysaccharides of R. meliloti, the slow-growing rhizobia, or Agrobacterium. Ca2+ and (or) Mg2+ were required for enzyme activity. In all strains tested, depolymerization of exopolysaccharide occurred when there was successful phage infection, but depolymerization also occurred with exopolysaccharides from nonsusceptible strains.

Evaluation of daunorubicin (NSC-82151) and methotrexate (NSC-740) in combination as a remission maintenance regimen in the treatment of acute leukemia.

The addition of a single monthy dose of daunorubicin (25 mg/m given intravenously) to the maximum tolerated dose of methotrexate (25 mg/m/dose given orally twice weekly) was evaluated as a maintenance regimen in 0 children with acute lymphoblastic leukemia. Seven patients who were nonevaluable were excluded from the study; 31 patients received methotrexate alone and 32 patients received a combination of methotrezate and daunorubicin. There was no significant difference between the distribution of remission times for the two maintenance regimens (medians of 147 and 162 days, respectively). For patients with only one previous remission, the median length of remission in this study was significantly shorter than that of the original remission time. Despite the known myelosuppressive effects of both agents, daunorubicin did not cause additional toxicity nor did it interfere with the scheduled methotrexate therapy. Although the addition of daunorubicin to methotrexate does not appear to be beneficial in the treatment of acute leukemia, this combination could be evaluated in other cancers in which both agents are known to be effective.
