[Varices and pregnancy].

Pregnancy is an important factor in the onset and development of varices (30 to 40% of pregnant women), essentially related to a hormonal influences. These varices present multiple clinical manifestations and generally an important but incomplete reversibility after delivery. Effective treatment and prevention of complications requires routine training of hygienic measures, the use of an appropriate, effective elastic bandage and sometimes limited sclerosis procedures. Phlebologic exploration after delivery, and possibility of further pregnancies, allows treatment to be defined for existing varices.

[Sclerotherapy of varices and protein C deficiency].

The appearance of relapsing venous thrombosis after sclerosis of varices in a woman aged 30 led us to discover a quantitative protein C deficiency, found in her father and 4 of her brothers and sisters. The transmission mode is autosomal and dominant. Within this family, 6 subjects are affected, 4 showing symptoms, clearly showing the heterogeneity of the clinical manifestation of the deficiency. Dealing with the special problem of pregnancy in women with the biological disorder, one ought to consider the literature on the subject, showing the large occurrence of ante- and post-natal accidents. The treatments of thromboembolic complications is based on the use of heparin administered appropriately through antivitamins K for a period which is till to be defined. In asymptomatic subjects, a preventive anticoagulant treatment is recommended in all circumstances likely to encourage the relapse of thrombosis.

[Post-stripping telangiectasis].

These telangiectasia appear between one and six months after the operation, especially in cases of capillary fragility. The most common localizations are the antero-internal and external sides of the thighs and knees. Unnecessary strippings, of continent saphenous veins, are the most frequent cause of these. Their prevention consists of the least possible traumatising stripping, using a fine stripper, a very rigorous post-operative support, and the wearing of light varicose stockings or tights for between one and three months. The treatment using microsclerosis, often delicate, should not be undertaken before six months.

[Immunopathologic aspects of recurrent phlebitis and nodular vascularitis. Therapeutic applications].

Recurrence of vasculitis is conditioned by 2 factors : a) one is extrinsic (bacteria, mycobacteria, viruses, chemicals) ; b) the other comes from the host. From host-extrinsic factors inter-relationships, depend recurrences of the disease. The role of streptococcosis is of primordial importance : 1) by its exotoxins (Streptolysin O) ;2) by its total exoproteins (highly sensitizing for human beings) ; 3) by an antigenic community with some tissular antigens. Immunotherapy when successful may act by an inhibiting effect on allergy (probably exerted by means of a stimulation of the reticuloendothelial system).
