[Left ventricular function of patients with chronic nonspecific lung diseases].

Echocardiographic studies of left ventricular function were carried out in 20 patients with chronic unspecific pulmonary diseases (CUPD). These patients showed no true left ventricular myocardial hypertrophy, while electrocardiographic signs of the latter might be related to the recording of potentials from the hypertrophic ventricular septum produced by positional cardiac changes due to pulmonary emphysema. In addition to septal hypertrophy, end diastolic left ventricular size was shown to be diminished. A hypothesis is proposed for the pathogenesis of left ventricular pumping disorders associated with CUPD.

[Current approaches to the early diagnosis of primary subacute infectious endocarditis].

A study of 22 patients with a diagnosis of primary infectious endocarditis based on clinical, laboratory and instrumental (electro-, phono- and echocardiography) investigation showed the fever syndrome and rigors to be the main clinical manifestations of the disease. Echocardiography can contribute to its early diagnosis, while increased erythrocyte sedimentation rate remains the most valuable diagnostic test.

[Dilatation (congestive) cardiomyopathy: clinical aspects, diagnosis and treatment].

Clinical and electrocardiographic manifestations of congestive cardiomyopathy are considered on the basis of observations of 25 patients with this pathology of the myocardium. No clinico-electrocardiographic signs specific for this disease were found. Most characteristic were cardiomegaly, congestive cardiac failure and various disturbances of rhythm and conductivity. Thromboembolic complications in the lung and kidney vessels are frequently found. Cardiac glycosides, saluretics and vasodilators were employed in the treatment of patients with congestive cardiomyopathy but the prognosis was usually poor.

[Liver-specific enzymatic activity and radiohepatographic indices in evaluating the functional state of the liver in myocardial infarct].

Data on changes in the blood activity of sorbit dehydrogenase (SDH) and fructose-1-phosphate aldolase (P-1-PA) in blood serum and the findings of radiohepatography in 224 patients with macrofocal and 120 patients with microfocal myocardial infarction are discussed. The studies were conducted in dynamics. Observations showed that SDH and P-1-PA activity grew in the acute period of mycrofocal myocardial infarction. The increase in enzyme activity was most marked in patients with cardiogenic shock. In patients with microfocal myocardial infarctions the increase in the activity of the enzymes studied was mainly encountered when there were signs of congestion in the liver. Radiohepatography revealed disorders of hepatic function in all patients with macrofocal myocardial infarction and in those patients with microfocal infarction who had congestion in the lijer. Changes in the radiogepatogram indices were most marked in patients with myocardial infarction attended by shock and congestion in the liver. The results of the study are evidence that simultaneous tests for SDH and P-1-PA activity in blood serum and radiohepatography give the fullest and most reliable information on the functional condition of the liver in patients with myocardial infarction.

Clinical Enzyme Tests↗

[Value of the enzyme tests in the evaluation of the severity of disease and prognosis in acute myocardial infarct].

The article discusses the results of simultaneous tests for the activity of total lactate dehydrogenase (LDH), its isoenzymes and creatine phosphokinase (CPK) in blood serum of 450 patients with macrofocal myocardial infarction; in 213 of them the disease followed a severe course and in 237 a mild course. The activity of these enzymes was used in this group of patients for judging the immediate and in 190 patients wht late prognosis. The authors' study showed that the maximum indices of the activity of LDH, LDH1 fraction and CPK in blood serum were higher in patients with severe myocardial infarction than in those with a mild course of the disease. The immediate and late prognosis in myocardial infarction was graver in higher activity of serum enzymes.

Clinical Enzyme Tests↗

[Gamma-glutamyl transpeptidase activity in ischemic heart disease].

The blood plasma gamma-glutamyltranspeptidase (GGTP) activity was studied in 133 patients with macrofocal myocardial infarction, in 40 patients with microfocal myocardial infarction, in 30 patients with angina pectoris, and in 75 patients with cardiosclerosis and congestive cardiac failure. The activity of the enzyme increased in most patients with macrofocal myocardial infarction and in less than half of those with microfocal myocardial infarction beginning with the 3rd or 4th day, reached maximum by the 6th to 8th day of the disease, and then returned to normal levels in various lengths of time. In all patients with angina pectoris and acute left-ventricular failure the activity of the enzyme remained normal. It may be assumed from the results of the study that determination of GGTP activity in dynamics may be mainly employed in the diagnosis of macrofocal myocardial infarction, particularly after the first days of the disease. The enzyme test is hardly suitable for differential diagnosis between microfocal myocardial infarction and angina pectoris.

[Treatment of congestive heart failure with beta-methyldigoxin (lanitop)].

Methylated Digoxin preparation--beta-Methyldigoxin (Lanitop) was used in 50 patients with congestive heart failure of different etiology (tablets of 0.3--0.6 mG per day). Its employment proved very effective in the treatment of congestive heart failure. It is not contraindicated to patients with chronic coronary insufficiency with underlying athrosclerotic cardiosclerosis with signs of congestive circulatory insufficiency. Side-effects of beta-Methyldigoxin are mild and noted rather seldom.

[Importance of determination of the activity of lactate dehydrogenase and its isoenzymes in blood serum for differential diagnosis of myocardial infarct, thromboembolism of the pulmonary artery and acute pancreatitis].

In 12 patients with macrofocal myocardial infarction and in 14 others with rheumatic mitral cardiac defects and congestion in the greater circulation complicated by thromboembolism of the pulmonary artery the activity of total lactate-dehydrogenase (LDG) and of its isoenzymes was determined in the blood serum. As shown by investigations the intercurrence of the lastly named complication was accompanied in these patients by an augmented activity of the total LDG and by readjustment of its isoenzymatic spectrum. The latter involved a progressive percentage growth of the activity displayed by LDG3 and LDG4 fractions. In patients with myocardial infarction without thromboembolism of the pulmonary artery the nature of the isoenzymatic spectrum changes is dissimilar, this enabling the method to be used in the differential diagnosis of the affections. The blood serum of patients with acute pancreatitis shows an increased activity of the total LDG, this being conditioned by a growing activity of the LDG3 and LDG4 fractions. The determination of the blood serum LDG isoenzymes activity as a test supplementing other methods of investigation may, with due account of the clinico-electrocardiographic picture, be used in the differential diagnosis of myocardial infarction and acute pancreatitis.
