Bladder transection revisited.

OBJECTIVE: To ascertain the safety and effectiveness of bladder transection for the treatment of enuresis and urinary frequency. PATIENTS AND METHODS: Twelve patients who had an open bladder transection performed at the Victoria Infirmary, Glasgow, were followed up for a mean of 37 months. RESULTS: Of the 12 patients, three were 'cured', seven were better, two were no better and none were worse than before the operation. All six patients with preoperative enuresis were cured of this by the procedure. CONCLUSIONS: Bladder transection remains a useful procedure in selected patients and avoids the problems associated with enterocystoplasty.

Fused vesical calculi.

A case is described in which a patient, who initially presented with five separate bladder calculi, was found to have had coalescence of three calculi to produce a large stone. This may be one mechanism in the development of a giant vesical calculus.

Locus of control and retirement preparation.

Locus of desired control and participation in a retirement preparation program were investigated in relation to retirement attitudes and adjustment. Fifty-nine older workers and retirees, approximately half of whom had participated in a retirement preparation program, completed measures assessing locus of control, attitudes toward work and retirement, life satisfaction and self-esteem. Retirement preparation was found to maintain both the desirability and expectancy of internal control and positive retirement attitudes among the worker participants relative to controls. Retirees did not differ on measures of adjustment; those with retirement preparation, however, evidenced higher externality than those without. Although the results indicated that internal expectancies were associated with positive psychological functioning, the role of retirement preparation in maintaining such expectancies into retirement remained equivocal.

Randomised trial of mesterolone versus vitamin C for male infertility. Scottish Infertility Group.

A prospective randomised trial was carried out to assess the effect of 9 months' treatment with either mesterolone or vitamin C in 368 men complaining of an infertile marriage. There was no significant difference in the wife's pregnancy rate between the two treatment groups. A separate analysis of the data was made to include only men with a low normal testosterone (less than 16 nmol/l) and low sperm density (less than 10 million/ml). This separate analysis was also restricted to men married to wives with normal test results for tubal patency and ovulation and again there was no difference in pregnancy rates between the two treatment groups. These results indicate that mesterolone therapy given in the absence of any defined endocrinopathy is of no benefit in promoting male fertility and furthermore we have been unable to find any subgroup who will respond. Improvements in the therapy of male infertility are only likely after a diagnosis has been defined.

Fournier's gangrene and urethral problems.

Fournier's classical description of infective genital gangrene has been frequently amplified. We present seven patients in whom the exact site of gangrene was related to the origin of the infection, either from the urethra or from the anorectum. A sixth case of diabetes presenting as Fournier's gangrene is described. The need for urological assessment in this condition is discussed.

Florid granulomatous reaction in a seminoma.

A case of seminoma of the testis complicated by an extensive granulomatous reaction is reported. This reaction was so florid that a benign condition was initially considered until further material for histology was examined and the correct diagnosis made.

The results of lower polar renal vessel transposition (the Chapman procedure) in the management of hydronephrosis.

Over a 15-year period, 19 patients with hydronephrosis due to pelviureteric obstruction associated with lower polar vessels were treated by ureterolysis and transposition of the vessels. Only those patients who, at operation, showed rapid pelvic emptying was not observed, a pyeloplasty was performed in addition. Complete relief of symptoms occurred in 90% of the patients. Post-operative reduction in the degree of hydronephrosis was demonstrated by intravenous urography in 74% and renal pelvic drainage was judged excellent by diuresis renography in 80%. Diuresis renography allowed the demonstration of free and rapid pelvic drainage in the majority of those patients with persisting hydronephrosis shown by intravenous urography.

Randomised trial of clomiphene citrate treatment and vitamin C for male infertility.

A prospective randomised trial was carried out to assess the effect of 6 months' treatment with either clomiphene citrate or vitamin C in 179 men complaining of infertile marriage. There was no significant difference in the wife's pregnancy rates between the 2 treatment groups. In order to detect a subgroup that might respond to clomiphene the results were analysed according to pre-treatment FSH and sperm density, with allowance being made for the result of the wife's tests, but still there was no significant difference in treatment response Vitamin C would seem a preferable and cheaper alternative treatment for male infertility.

Cecocystoplasty for bladder augmentation.

Augmentation of the bladder by cecocystoplasty has been studied in 37 patients during a 15-year period. The indications for an operation were a small contracted bladder secondary to tuberculosis in 31 cases, interstitial cystitis in 5 and carcinoma of the bladder in 1. Results were studied by the assessment of symptomatic improvement, the effect on renal function, urinary tract infection and the postoperative complications. Urodynamic studies also were done on 23 patients with tuberculosis of the renal tract and the results are discussed. Azotemia was not a contraindication to an operation but patients with enuresis or those who have evidence of psychiatric disturbance should be assessed carefully.

Idiopathic calcinosis cutis of the penis.

Calcification of the penis, whether in the skin, subcutaneous tissue or corpora is exceedingly rare and is usually secondary to trauma or Peyronie's disease. A case of idiopathic calcinosis cutis of the penis is presented.
