Maxillofacial injury.

This chapter has emphasized the relative infrequency of severe skeletal maxillofacial injuries in children. When they do occur, it is extremely important that they be approached by a team of physicians, each of whom is competent to manage the injury and complications related to his specialty. Particular attention must be given to the issues of general facial bone growth as well as future dental development. All operative procedures must be designed to avoid further injury to unerupted teeth, and the traditional methods of immobilization must be modified in order to avoid the avulsion of deciduous teeth.

Olfactory neuroblastoma. Management and prognosis.

Olfactory neuroblastoma is an uncommon malignant tumor originating in the olfactory epithelium in the roof of the nose. It usually produces nasal obstruction and secondary sinus disease and is often mistaken by the physician for a nasal polyp. Olfactory neuroblastoma has been observed to cause death by distant metastasis or by invasion through the cribriform plate and secondary meningitis in most instances, and is generally associated with a low cure rate. During the past eight years, we have treated three patients with olfactory neuroblastoma by a combination of extensive local surgery using the lateral rhinotomy approach with ethmoidectomy and medial partial maxillectomy, combined with either preoperative or postoperative irradiation therapy. From a review of the literature we have tried to draw meaningful conclusions regarding treatment.

On estimating the frequency of a recessive gene in a random mating population.

The maximum likelihood estimator of the frequency of a recessive gene in a random mating population is simply the square root of the proportion of recessive individuals in a random sample. However, its asymptotic variance, i.e. the inverse of Fisher's information, has an unexpected functional form, and its use may lead to incorrect inferences. An explanation for this is sought by deriving an expansion for the exact variance of the estimator. Further, a number of calculations reveal conditions under which the asymptotic variance provides a reasonable approximation to the exact variance. Lastly, the problem of setting confidence limits to the gene frequency is discussed, with a number of approximations being considered.

Glottic reconstruction after hemilaryngectomy: bipedicle muscle flap laryngoplasty.

This paper presents the techniques for glottic reconstruction after partial laryngectomy using a bipedicle muscle/perichondrial flap. The use of this technique extends the scope of lesions amenable to conservation surgery and maximizes the postoperative laryngeal function in terms of satisfactory phonation, respiration, and deglutition.

Acupuncture Therapy

An implanted electronic laryngeal prosthesis.

The authors present a preliminary progress report on their investigations dealing with the development of an implanted electronic laryngeal prosthesis system. Their initial observations indicate success in the design and fabrication of the implantable, retropharyngeal transducer and secondary induction coil elements. The surgical procedure for implantation is described, and the complications encountered during the first thirteen procedures are reported. These complications included five seromas, one postoperative abscess, one partial wound breakdown secondary to animal-inflicited traumn, and two anesthetic deaths at the time of follow-up radiographic procedures. Additional observations are presented regarding the acoustical output of the transducer in air and after implantation. Projections are made concerning the use of this laryngeal prosthesis system in human patients for vocal rehabilitation after total laryngectomy.
