Control of colonic variceal haemorrhage with a somatostatin analogue.

A 64 year old caucasian male with known alcoholic liver disease presented with rectal bleeding. The patient bled repeatedly despite resuscitation. A sigmoid colectomy was performed following an angiogram. Large colonic, rectal and mesenteric variceal vessels were noted at surgery. Further bleeding postoperatively was rapidly controlled with octreotide infusion. Octreotide may be as useful in the control of colonic variceal bleeding as it is in the control of oesophageal variceal bleeding.

Predictors and the rate of medical treatment failure in ulcerative colitis.

Eighty-nine admissions into Auckland Hospital with exacerbation of ulcerative colitis (between May 1985 and May 1991) were analyzed to determine the rate and any predictors of medical treatment failure. Twenty-five patients (28%) failed to respond to medical treatment and required surgery. Clinical information and laboratory indices available within 24 h of the admissions were compared between the patients requiring surgery and those who did not. There were no significant differences between the groups in sex distribution, age of onset of disease, duration of disease, hemoglobin concentration, or white cell count. However, "severe" diarrhea (chi 2 = 24.83, p = 0.0001), and lower albumin level (F1, 83 = 45.61, p = 0.0001) were noted in the surgical group. There was a tendency toward higher ESR (F1, 82 = 3.06, p = 0.08) and "extensive" colitis (chi 2 = 3.29, p = 0.07) in the patients requiring surgery. Univariate analysis confirmed albumin level and severity of diarrhea as significant discriminators. A discriminant function analysis showed that albumin level and severity of diarrhea would distinguish between surgical and nonsurgical outcome in 82% of cases. Distal colitis and mild to moderate diarrhea had negative predictive values of 80% and 91%, respectively, for nonsurgical outcome of acute ulcerative colitis. It is concluded that the above significant variables are good predictors of outcome of medical treatment for exacerbations of ulcerative colitis and that the proportion of patients needing surgery has not changed in the last 35 yr despite various management strategies.

Hepatic metastasis from a meningeal haemangiopericytoma.

Intracranial tumours rarely metastasize outside the central nervous system (CNS). Meningeal haemangiopericytoma is an exception. We report the case of a 41-year-old female who initially presented in 1984 with what was thought to be a posterior fossa meningioma which was excised, as were two subsequent recurrences in 1987 and 1988. In October 1990 she represented, having discovered a mass in her abdomen. This proved to be a metastatic haemangiopericytoma in the liver which had similar histology to the original intracranial tumour. This metastasis, and another found at operation, have been excised, and at the time of this report, she is alive and well seven months after surgery without any evidence of other metastasis or recurrence. Hemangiopericytomas are unusual in their propensity to metastasize outside the CNS and successful resection of hepatic metastases from this tumour has not been reported before.

Mechanism of action of natriuretic fraction of urine.

The nature of calcium dependence of natriuretic fraction (NF)-induced contractions in the rat smooth muscle anococcygeus and a possible correlation between the effect of NF on sodium transport and its natriuretic potency was investigated. NF and noradrenaline-induced contractions were partially dependent on external calcium concentration. Ouabain and potassium (K) free solution (KoPSS)-induced contractions were totally dependent on external calcium and were more effectively inhibited by calcium antagonists than those of NF and noradrenaline suggesting pharmacomechanical coupling as the mode of calcium dependence of NF. Like other agonists which contract by pharmacomechanical coupling, NF stimulated sodium transport (86rubidium uptake) in dog saphenous vein. Such stimulation correlated positively (r = 0.495, n = 17, P less than 0.05) with the natriuretic potency of NF. NF, like noradrenaline, contracts smooth muscle by pharmacomechanical coupling and NF-induced natriuresis is associated with stimulation of the sodium pump.
