Difficulties in the diagnosis of intracystic tumors of the female breast.

The objective of the current study was to determine the accuracy of radiological and cytological diagnoses of intracystic and papillary lesions in distinguishing between true papillary benign and malignant lesions. Seventy cytological reports of intracystic proliferations were selected from our cytopathological database at the Breast Health Corporation, Budapest, Hungary, dating back to the last 7 years. Retrospective analysis of the diagnostic approaches--mammography, ultrasonography, clinical examination and cytology--was performed in selected cases. The results of imaging and cytological examination are routinely reported on a categorical scale ranging from 1 to 5. 44 patients underwent surgical excision: histology showed benign lesions in 21 and malignant lesions in 23 cases. Twelve patients, who did not undergo biopsy and presented a stable disease at follow-ups, were also included in the group of benign lesion. Fifteen patients were not available for follow-up. Concerning the total investigated cases the mean categorical values of mammography, ultrasonography and cytology were 2.24, 2.78 and 3.05 respectively. The malignant and benign groups significantly differ from each other concerning the mean age of the patients (p=0.0216), the distribution of the coded mammographical results (p= 0.0171) the cytological results (p=0.0001), and average tumor size measured on mammogram images (p=0.0199). The two group does not significantly differ over the distribution of mammographical density patterns (p=0.1075), radiomorphological appearance (p=0.1101), average tumor size measured on ultrasonography (p=0.2665), and patient complaints (p=0.2634). The evaluation of ultrasonography shows borderline significance (Pearson Chi-square test: p=0.0616, M-L Chi-square test: p=0.0404) between the malignant and benign groups. Differential diagnosis between malignant and benign intracystic and papillary lesions is feasible using common radiological diagnostics. However, more efficient teamwork is needed with the cooperation of a well-trained cytologist and radiologist, who are able to produce precise images of the lesions, and guides the aspiration of the adequate samples for cytology, which is the most valuable examination.

Leiomyosarcoma of the female breast.

Leiomyosarcomas of the breast are rare tumors. Less than 15 such cases have been reported in the literature so far. In this paper authors describe a case of leiomyosarcoma of a female breast presenting as a firm lobulated mass, mimicking a phylloid tumor radiographically. By fine needle aspiration biopsy, on the smears discohesive malignant looking cells were conclusive to a poorly differentiated invasive ductal carcinoma of the breast. The mastectomy specimen contained a lobulated mass, microscopically showing a partly epithelioid spindle cell tumor, immunoreactive for vimentin, desmin, smooth muscle actin antibodies, and negative for epithelial markers, hormone and growth factor receptors. Axillary lymph nodes were free of tumor. A primary leiomyosarcoma of the breast was diagnosed.

[Eosinophil cystitis].

Authors report on two rare cases of eosinophilic cystitis, giving a review of the etiologic assumptions and pathogenetic, pathologic aspects of the disease, based on the available literary data. The course of the disease can be either acute or subacute, and is most often chronic. Relapse and progression can interchange in irregular manner, attention is therefore called to the importance of follow-ups. Resection deep into the intact--also containing muscle fibre--as well as histologic examination are considered essential as the only method of differentiation, giving precise diagnosis in the present two cases, too.

[Renal oncocytoma].

Authors review the case history of three patients with kidney tumours which had been surgically removed. In two of the cases radical nephrectomy while in one, organ-preserving surgery was performed. In all three cases histology revealed benign kidney tumours-oncocytomas. In light of the available literature authors indicate that preoperative diagnosis is almost impossible. Furthermore, it is their opinion that this benign kidney tumour has a less frequent occurrence than earlier thought.

[Ultrasound-guided fine-needle aspiration biopsy of focal liver lesions].

The ultrasound guided percutaneous fine needle biopsy (US-FNAB) of focal lesions in the liver is indispensable in many clinical situations. During the last 12 years, 657 US-FNAB were performed on patients with suspected neoplastic involvement of the liver with 22-gauge Chiba needles at our department. US-FNAB was performed mostly with the "free hand" technique. Sufficient material for cytologic analysis was obtained in 84% of the cases. The biopsies confirmed malignancy in 39.3%, including 9% primary hepatocellular carcinoma, 8% of the cases were suspect for malignancy, and in 36.7% were diagnosed benign lesion. 233 cases were confirmed histologically and with other follow up methods. The sensitivity rate was 91%, and specificity was 100%. There was no false positive diagnosis and no noteworthy complications were observed. US-FNAB is a highly reliable, safe, inexpensive and easy diagnostic procedure. On the basis of our experience, we recommend US-FNAB as a routine, first level procedure for the diagnosis of focal liver diseases.

Search for prognostic factors in head and neck cancer.

Fifteen patients with head and neck carcinoma were treated with irradiation (14-70 Gy telecobalt). Apoptotic and mitotic index, DNA index, ratio of cells in S phase, p53 protein overexpression in untreated tumours as well as the changes of these parameters after the first 2 Gy irradiation except proliferative kinetics parameters were examined in order to determine the prognostic value of these factors. The data show that inducibility of apoptosis is very low in head and neck carcinomas which correlate with the unfavourable prognosis. The decrease in mitotic index after the first 2 Gy irradiation, which occurred in 7 cases--5 of them still alive--indicates a better chance for relatively longer survival. In immunohistochemically p53 positive tumours frequently occurring aneuploidy and elevated S-phase rate were found.

[Papillary cystic neoplasms in the ectopic pancreas].

The authors reported a case of a young woman. From her omentum a clinically and radiologically benign tumour was removed that had been observed for years and caused minor symptoms. Histologically the tumour proved to be a so called papillary and cystic tumour that originated from ectopic pancreatic tissue. The histological diagnosis was justified by immunohistochemical and electronmicroscopical examinations. Reports of this type of tumour evolving in an ectopic pancreas appeared only twice in the world literature to the knowledge of the authors.

[Glandular myxoma of the heart].

Light microscopic, immunohistochemical and ultrastructural studies were performed on a surgically removed left atrial myxoma containing glandular structures. Routine examination showed glandular lumina lined by cylindric epithelium in the characteristic myoxoid stroma, and the glands contained PAS positive mucin. The glandular structures demonstrated significant positivity for wide-spectrum-CK, EMA, and moderate positivity for CEA. Stromal cells were positive for vimentin, FVIII. Ag was present only in surface cells and blood vessels. Ultrastructurally the gland showed characteristics of mucin secreting epithelium. The authors briefly overview the currently existing theories about the histogenesis of myxomas.

[Cytodiagnosis of Ewing sarcoma].

Pelvic Ewing's sarcomas were studied by fine needle aspiration biopsy in two young male patients. Local pain, subfebrility, high ESR had occurred in both cases. X ray examination showed osteolytic lesions in both patients. The differential diagnostic question was, are the lesions osteomyelitis or malignant tumors? Fine needle aspiration biopsies and cytological examinations were performed in order to determine the nature of the lesions. The aspirates consisted of monotonous population of small undifferentiated tumor cells. The nuclei were monomorph with small nucleoli and finely granular chromatin. The tumor cells contained large amounts of PAS positive diastase-digestible granular cytoplasmic glycogen. The aspirated material was examined by immunocytochemistry and electronmicroscopy in one of the cases. Both of these methods had diagnostic value in our patients in the detection of Ewing's sarcoma.

Prognostic significance of P53 histochemistry and DNA histogram parameters in head and neck malignancies.

BACKGROUND: The aim of the study was to determine the role of quantitative pathological parameters in prognosis of head and neck malignancies. MATERIALS AND METHODS: 51 head and neck squamous cell carcinoma patients were examined for mutant p53 gene expression (45 out of 51 patients) by immunohistochemistry and for cellular DNA-content (44 out of 51 patients) using digital picture analyzer. Statistical analysis was performed using BMDP package. RESULTS: No correlation with prognosis was found for age, sex, localization, T-classification and therapy. There was significant relationship between N-status and overall survival (p = 0.0008). No correlation was found with overall and disease-free survival for either histologic type or grading. P53: No significant correlation was detected with overall survival. A relationship was found between mutant p53 and metastasis-free time (p = 0.06). Ploidy: There were no significant differences between aneuploid and euploid tumors for either disease-free or overall survival. Synthetic (S)-phase fraction: A correlation was found for both survival rates (p = 0.029) and metastasis-free time (p = 0.05). Polyploid fraction (PF): correlation was shown for both overall survival (p = 0.0128) and metastasis-free time (p = 0.0038). CONCLUSION: There is correlation between p53 overexpression and metastatic potential and there is a significant relationship between SPF and PF value and prognosis (metastasis-free and overall survival) of head and neck cancer.
