[Surgical treatment of hypernasality in Denmark].

OBJECTIVE: A palatopharyngoplasty is a widely used operation performed to diminish hypernasality. The purpose of this study was to assess the results of the palatopharyngoplasties and to estimate if the type of cleft or other structural defect influenced these results. MATERIAL AND METHOD: This is a retrospective study of the patient records from 1/1-1991 to 1/11-1996 comprising 297 operations on which 284 records were available. The results of the operations were evaluated by speech pathologists who have rated the nasality before and after the palatopharyngoplasties. RESULTS: Following operation 90.1% of the patients obtained normal resonance or reduced hypernasality. The type of cleft or other structural defect did not influence the results. DISCUSSION: Palatopharyngoplasty is an effective operative procedure in the treatment of hypernasality, regardless of the type of cleft or structural defect. To further optimize the treatment a higher level of standardization is recommended.

Zinc supplementation during pregnancy: a double blind randomised controlled trial.

OBJECTIVE: To investigate whether zinc supplementation during pregnancy improves maternal and fetal outcome. DESIGN: Controlled clinical trial started at registration until discharge of mother and child from hospital. Two thousand volunteer mothers were randomly assigned to receive zinc supplementation or placebo in a double blind trial. PATIENTS: Women less than 20 weeks pregnant at the first visit who agreed to take part in the study. One thousand two hundred and six mothers were available for study. INTERVENTIONS: Volunteers were randomly selected to receive two tablets of zinc (44 mg zinc in total) or two placebo tablets containing inert substances, indistinguishable in appearance and taste from the zinc tablets. OUTCOME MEASURE: Large for gestational age (LGA), small for gestational age (SGA), premature rupture of the membrane (PROM), preterm labor (PL), preeclampsia and bleeding in the second or third trimester. RESULTS: There were no differences between mothers given zinc supplementation concerning the outcome parameters and those given placebo. CONCLUSION: Zinc supplementation in pregnancy in a normal healthy middle class population in Denmark does not seem to offer any benefits to the mother or her fetus.

Burn injuries caused by fireworks: effect of prophylaxis.

During the New Year 1991-92, a total of 17 patients with burns caused by fireworks (an unprecedentedly high number) were admitted to the two Danish burns units. They were all males and all had carried the causative firework in their clothes; 88 per cent were minors, and 87 per cent of these had bought their fireworks themselves in ordinary shops. Fireworks described as 'whistles' were responsible in 88 per cent of the cases. Appeals to the authorities resulted in a change of the legislation in November 1992, allowing only whistles equipped with safety fuses. Altogether 25 tons of unauthorized fireworks were confiscated and destroyed before January 1993. Campaigns were conducted at all schools and in the media in November and December 1992 and 1993, giving information about the dangers of carrying fireworks close to the body. During the New Year 1992-93, only four children were admitted with firework burns; the number of patients was significantly lower than in the preceding New Year, as was also the extent of their burn injuries. Furthermore, the patients were all younger than the age group targeted by the school campaign. The following New Year 1993-94, only three children were admitted with minor burns caused by fireworks, confirming the effect of the prophylactic actions. We conclude that the prophylactic actions were effective enough to reduce the number and severity of burn injuries caused by fireworks.

[Burns due to fireworks. A 4-year study of the incidence and causes with suggestions for prevention].

During the period around New Year 1991-1992, significantly more patients were admitted to the burns units in Denmark with burns caused by fireworks, compared to the previous three years. All patients were males and 88% under 18 years of age. The areas burned were localized primarily to the thighs and the front of the body. The main causative fireworks were whistles. Analyses revealed the fuses to be highly combustible. New directives have been issued, requiring whistles to be equipped with safety fuses. Although 70% of the patients were aware that fireworks should not be carried close to the body, the message has not been sufficiently stressed. A campaign for the oncoming New Year, informing about burns, eye and hand injuries caused by fireworks is under preparation. Although sale of fireworks to persons under 18 years of age is prohibited, more than 80% of the children had purchased the fireworks themselves. We recommend reinforcement of the law.

[False aneurysms after vascular surgery].

The time of development of false aneurysms following reconstructive vascular surgery is established in an attempt to rationalise the postoperative vascular control. During a period of 12 years, 51 false aneurysms were found in 40 patients after a total of 803 reconstructive vascular operations. Routine ultrasonic scanning revealed 34 cases of false aneurysms. During follow-up control, particular attention should be paid to patients who have genuine aneurysms already and patients with postoperative wound infection and prolonged leakage of serum from the wound. Follow-up control should probably be continued throughout life.
