Nurses' contributions to the US space program.

Nurses have been engaged in aspects of aviation and space endeavors from their beginnings. Although no nurse has been chosen as a potential astronaut, nurses have contributed to the space program in many ways. Nurses and other health care providers use technology adapted from the space program. The future of extended space flight remains unclear, although work continues on completing a space station for extended stays on the moon and trips to Mars. Nurses may be one of the health care team members on these excursions.

Perioperative care of the elderly surgical patient.

Elderly surgical patients undergo approximately 20% or more of all surgical procedures in acute care hospitals across the country. Elderly people also are more at risk for surgical intervention because of age-related system changes and comorbid conditions. In recent years, greater knowledge about aging and advanced surgical, anesthetic, and monitoring technology has made it possible for many elderly people to undergo surgery without significant mortality. This article is intended to increase the ability of perioperative nurses to anticipate, plan, and provide care for their elderly surgical patients.

Hypothyroidism in elderly patients.

Hypothyroidism is an endocrine condition found in elderly patients that is often confused with normal changes associated with aging. Consequently, the disorder may be quite advanced before it is diagnosed and treated. Depressed T4 and elevated TSH levels confirm primary hypothyroidism. Patients are required to take levothyroxine for the rest of their lives. Myxedema coma, a life-threatening complication of hypothyroidism, occurs primarily in elderly women and requires immediate treatment to prevent death. Recovery and stabilization may take six to eight months.

Hyperthyroidism in elderly patients.

Hyperthyroid disorders occur in older people and often are present with atypical findings or as apathetic hyperthyroidism. Hyperthyroidism exacerbates underlying chronic disease, especially cardiovascular conditions. Elderly people can be treated successfully and stabilized with careful screening, astute diagnostics, and appropriate therapy.

Disseminated intravascular coagulation. Principles, treatment, nursing management.

Disseminated intravascular coagulation is an abnormal syndrome that occurs secondary to clinical conditions that result in widespread microthrombosis, macrothrombosis, and hemorrhage. Depending on the primary clinical condition and the intensity and duration of the bleeding episode, mortality can be high. Treatment is first directed at the primary clinical condition followed by measures to stabilize coagulation and fibrinolysis and to replenish body fluids, blood, and blood products. Continual assessment of clinical manifestations and evaluation of laboratory values are used to monitor therapy effectiveness.

Perioperative care of patients undergoing spinal stabilization with internal fixation (continuing education credit).

1. Low back pain affects approximately 80% of the adult population. There are between 200,000 and 500,000 spinal surgeries performed every year. 2. New spinal implant systems offer hope for persons with failed spinal surgery--as well as for those with spinal fractures, metastatic disease, and severe degenerative disorders. 3. Although complications can develop, the benefits of new spinal fixation systems far outweigh the problems.
