Patient education needs assessment: constructing a generic guide.

A National Veterans Administration Multidisciplinary Task Force was developed to facilitate the comprehensive assessment of individual patient education needs. A review of literature was done for existing assessment instruments. Although the literature clearly stated the importance of patient assessment, no comprehensive tool could be found. This article describes the process by which the Task Force developed and tested a comprehensive guide to assist staff members in individual patient education assessment. Four target conditions were identified and used throughout the development process to assure that it was applicable to a variety of patient populations. The Task Force identified possible barriers to action for each of the conditions, and then developed questions to assess each barrier. Questions were refined through role plays and extensive review nationally. The guide was then pilot tested in 3 Veterans Administration (VA) Medical Centers. Changes were made based upon the results. The guide has been distributed within the VA for 2 primary purposes: As a basis for staff training regarding actual assessment, and as a guide for staff who assess patient education needs. An evaluation is planned for Spring 1987.

Educational coordinator--staff development--what is their impact on patient education?

The increasing complexities in the health care system have produced a need for well-planned patient education. Integral to the development of these programs is: the assessment of patient populations' educational needs; the design of a program to meet these defined needs; the implementation of this pre-designed program; and, the evaluation of that total learning experience. The literature suggests that a patient education coordinator facilitates this program planning process. It is also suggested that in agencies where a coordinator position is non-existent, that staff development related to patient education can enhance programming efforts. This investigation examined the impact of staff development and the coordinator on the program-planning process. It researched the effect of these factors singularly and collectively. The results suggest that individually and in tandem both factors have a positive impact on patient education. Best results were observed in institutions having a coordinator and a staff development program.

Evaluation of platelets prepared by apheresis and stored for 5 days. In vitro and in vivo studies.

To evaluate the effect of storage on apheresis platelets collected with a closed-system blood cell separator, an in vitro investigation was performed, with measurements of pH, lactate, ATP, the ratio of ATP to the total adenine nucleotide content, and adenylate kinase. Unmodified apheresis platelets and apheresis platelets with plasma added were compared with conventional platelets stored in PL-1240 or PL-732 plastic containers. During 6 days of storage, there were similar changes in all variables with one exception: the extracellular activity of adenylate kinase was lower in apheresis platelets with plasma than in the other three groups (p less than 0.01). In vivo studies were carried out with 111Indium-labeled autologous platelets in eight volunteers. Apheresis platelets with 100 mL of plasma added were stored in two 1000-mL containers (PL-732) at 22 degrees C during agitation. Platelets from one of the containers were labeled with 111Indium and transfused into the volunteer within 24 hours. Platelets from the other container were labeled after 5 days of storage and transfused into the same donor. There were no significant differences between apheresis platelets stored for 1 day and those stored for 5 days: the mean percentage of recovery was 58.4 and 57.6 percent, t1/2 was 69 and 67 hours, and the survival time was 5.5 and 5.6 days, respectively.
