The aim of our study was to determine the role of cytokine interleukin-1-beta (IL-1-beta) in patients with chronic hepatitis C virus (HCV) infection. Twenty-eight patients with chronic HCV infection were studied and compared with 18 healthy subjects. IL-1-beta levels were measured with an enzyme-linked immunosorbent assay in plasma and in supernatants of peripheral blood mononuclear cells (PBMC) incubated alone or in the presence of lipopolysaccharide (LPS). No IL-1-beta was found in plasma or unstimulated PBMC supernatants. The mean LPS-induced IL-1-beta production was 20.0 +/- 4.0 ng/ml in patients with chronic HCV infection and 29.4 +/- 3.7 ng/ml in the control group. The patients had significantly lower levels of LPS-induced IL-1-beta than the control group (p < 0.00001). No difference in LPS-induced IL-1-beta production was found in patients in relation to the histologic diagnosis (p > 0.05). There was no correlation between LPS-induced IL-1-beta production and serum alanine aminotransferase levels in patients with chronic HCV infection (r = 0.37, p > 0.05). Although limited to a small number of cases, our results suggest that LPS-induced IL-1-beta production by PBMC is impaired in patients with chronic HCV infection.

This paper presents the preliminary results of a study designed to evaluate the effects of alpha interferon in chronic hepatitis B. After six months' treatment with interferon alfa-2b (5 million units (MU), three times weekly) 15 of 25 (60%) patients achieved seroconversion of hepatitis B e antigen, 17 (68%) normalised alanine aminotransferase (ALT) activity, and 15 (60%) showed a decrease in the inflammatory reaction on liver histology. No seroconversions occurred in the control group (n = 10), and none of the control patients achieved a normal ALT or showed a reduction in the inflammatory reaction. Adverse effects were experienced by most patients who received interferon but none warranted stopping the treatment.
The authors report their experience with 30 adult patients with Budd-Chiari syndrome (BCS), which is a rare and serious disorder, characterized by hepatic outflow obstruction caused by many different conditions. The diagnosis was based on the clinical data, ultrasonography (US), vena cavography and hepatic venography, computed tomography (CT), and liver biopsy. Behçet's disease (BD) was found in 10 patients with BCS as an underlying disease. Two patients used oral contraceptive drugs, 2 had liver tumor hepatocellular carcinoma and liver lymphoma, and 1 patient had chronic lymphocytic leukemia. Despite full investigation, the authors could not find any obvious underlying cause in the other 15 patients. The results suggest that (1) BCS must be considered as a possible complication in patients with Behçet's disease when they have hepatomegaly even if there were no cardinal manifestations of the disease at the time of admission, and BD is the most common etiologic factor in BCS (33%) in Turkey, where the incidence of Behçet's disease is relatively high. (2) Anti-aggregant treatment seems to be effective in many instances. (3) There were space-occupying lesion-like appearances in the liver of 7 cases by CT and US examination in the acute stage, and these disappeared on the follow-up CT and US in 5 patients but continued in 2. BCS should thus be differentiated from other liver lesions. (4) There were other great-vessel involvements in 43% of the cases, mostly venous, but only 1 pulmonary arterial occlusion.
In the present study, we investigated Hepatitis B virus (HBV), Hepatitis C virus (HCV), and Hepatitis D virus (HDV) status in patients with chronic liver diseases seen in the period of 1990-1992 using enzyme-linked immunosorbent assay (ELISA) kits. There were 126 male and 64 female patients with a mean age of 42.3 years (range 17-70). Of the 198 patients, 68 (35.7%) had evidence of HCV infection. One hundred and twenty-three patients (64.7%) were positive for hepatitis B surface antigen (HBsAg). Of the 123 HBsA positive patients, 35 (28.4%) were positive for Anti-HDV. Fourteen patients were negative for markers of both HCV and HBV. Liver histology showed persistent hepatitis in 31 patients (16.3%), chronic active hepatitis in 77 (40.6%). Cirrhosis was diagnosed in 82 patients (43.1%). Our results indicate that HCV infection plays a role in chronic liver disease especially where the etiology appears obscure.

A total of 832 patients with portal hypertension resulting from different etiology was studied by ultrasonograph as a screening test. In 17 of the 832 patients, cavernous transformation of the portal vein was detected by means of ultrasonography. We have prospectively studied these 17 patients, and the diagnosis of cavernous transformation was confirmed by portography in all patients. To evaluate how much biliary tract has been affected from cavernous transformation of the portal vein, and to explain the cause of mildly increased alkaline phosphatase and serum bilirubin levels, endoscopic retrograde cholangiopancreatography (ERCP) was performed in 16 of the 17 patients. There were narrowing, irregularity, undulation and nodular extrinsic defects resulting from compression of thrombosis of the portal vein and the collateral vessels, mimicking cholangiocarcinoma spreading along the common bile duct on the extrahepatic biliary tract in all 16 patients who underwent ERCP. Similar ERCP findings were not found in six patients with portal hypertension due to liver cirrhosis. The ultrasonographic, portographic, and ERCP findings corresponded to surgical findings in six patients who had undergone splenectomy for either hypersplenism or bleeding from esophageal varices. The results indicate that cavernous transformation of the portal vein cause the above-mentioned radiographic findings that we propose to call "pseudo-cholangiocarcinoma sign."

The incidence of neural tube defects was monitored among all live- and still-births delivered between June 1986 and July 1988. In all there were 19,115 total births. The incidence of neural tube defects (NTD) was 1.9 per 1000 among those whose conception time was prior to May 1986. However, in conceptions occurring in May, June and July 1986, the NTD rate increased to a level of 8.9 per 1000, subsequently gradually declining to previous levels within 9 months. This dramatic change in incidence, which parallels a previous finding from Turkey, suggests that this finding may be associated with the Chernobyl disaster of May 1986.
Fifty patients with bronchial asthma and 50 normal controls were studied in Hacettepe University Medical Center. Forty per cent of the patients with bronchial asthma and 14% of the controls had ascarides in the stool examinations. The significant difference in these two groups indicates that intestinal parasites, should be looked for in the patients with bronchial asthma.
Thirty patients were studied at the Hacettepe University Medical Center. Ten patients had hepatic cirrhosis, 20 patients had viral hepatitis. HB-Ag in the serum was positive in all patients by the counterimmunoelectrophoresis method. One and a half milliliters of sweat was collected from each patient, dried and dissolved in 0.2 ml. of buffer solution. HB-Ag was positive in all patients by radioimmunoassay method. This study indicates that direct contact is one of the most important factors in the epidemiology of the disease and this may even be more important in asymptomatic carriers.
