Vacuum-supported endoscopic access.

The rapid increase in the number of endoscopic operations performed has resulted in a corresponding rise in the incidence of severe complications. This issue has generated widespread concern at recent congresses. It is now indisputable that safer access to the abdominal cavity is required. We have developed a technique of vacuum-supported visual access which allows the surgeon to anticipate dangerous sites of access and avoid injuries to internal organs or retroperitoneal blood vessels. The procedure has two major advantages: Firstly, a pre-peritoneal approach allows the surgeon to visualise the structures behind the peritoneum, and secondly, puncture of the insufflation-needle is facilitated and the depth of insertion is minimised. The underlying principle is to use negative pressure to elevate the peritoneum. Using this approach, the principles of open surgery are adhered to during abdominal access, thereby avoiding visceral injury.

[Multiple work stations for an ultrasound device].

A possible technical innovation of ultrasound technique could take into consideration the multiple use of the ultrasound system and thereby facilitate the utilisation for the examining physician and the equipment. This innovation is described and discussed in detail in the following essay.

Future trends in endoscopic suturing.

This paper deals with future aspects and developments in endoscopic sutures, needles, needle drivers, and sewing devices. Shape memory alloys such as superelastic nickel-titanium can be used for surgical needles and hingeless needle drivers. A sewing device consists of a T-Needle which can be shuttled between the jaws of specially designed instruments. The jaws possess small elements that grip the needle tips. The "Needle Rotor" facilitates intracorporeal swivelling and positioning of the needle because one jaw can be moved longitudinally over the other. A new "cutting knot pusher" permits immediate cutting of a slip knot subsequent to tightening.

[Acoustically evoked brain stem potentials in acute alcoholic intoxication].

Under the acute influence of alcohol with blood alcohol concentrations (BAK) ranging between 0.62 to 2.04%, the IPL prolongations of our subjects were within standard deviations compared with normal data. Depending on the BAK, these minor IPL alterations hint at a special vulnerability in the pontomesodiencephal area. The increase in BAK significantly correlates with the reduction of body temperature. The BAEP proves to be suitable for defining undear states of coma different origin.

[Acoustically evoked brain stem potentials in chronic alcoholism and withdrawal].

BAEP studies hint at different neurophysiological findings between gamma and delta alcoholics. On the first day of withdrawal gamma alcoholics show slight vegetative withdrawal symptoms in the form of CNS hyperexitability trough the shortening of the early IPL. However delta alcoholics show prolongations of the late IPL which can be interpreted as a consequence of brain stem transmission disorders resulting from chronic intoxication. After four weeks the BAEP deviations found in both groups indicate a clear tendency of recovery which points to functional disorders.

[Immunoglobulins after experimental liver transplantation in pig].

During a period of 14 days after porcine liver transplantation immunoglobulins IgA, IgG, and IgM were investigated. IgA and IgG were slightly elevated at the end of investigation. The increase in IgM was higher and significant. This rise in immunoglobulin-level is thought to be a result of immunisation by parenchymatous insults after liver transplantation.

[Metabolite changes and ulcer formation in the cornea. Experiments with acid burns of varying severity (author's transl)].

Acid burns with 0,25 n H2SO4 constantly cause mild corneal opacities. Burns with 2 n acid are too severe to be investigated for more than one week. Lactate and pyruvate levels drop in the aqueous and, less pronounced, in the cornea. A very similar decrease of glucose levels in aqueous and cornea can be found depending in its degree on the acid concentration used. In milder burns the levels return to normal within 2--3 weeks. The results can be explained by changes of the blood-aqueous barrier and of the rate of aqueous flow. It seems very unlikely that the metabolite changes cause the formation of corneal ulcers, as severe ulcerations can be found along with almost normal metabolite levels.
