[Aneurismal bone cyst of clavicle: A case report].

Aneurismal bone cyst is a rare tumour. The authors report an exceptional case in its clavicle localization. They review the diagnosis circumstances and treatment modalities of this pathology. The occurrence of the cyst is linked to vascular homodynamic disturbance and its discovery in a post-traumatic context has been assessed by the histology exam. Although numerous therapeutic trials, radical removal versus conservatory methods avoid relapse. Finally, resection of the clavicle leading to few functional consequences should be recommended for the cure of evoluted aneurismal bone cyst.

[Abruptio placentae: epidemiological, clinical and prognostic aspects with respect to a 177 case series].

OBJECTIVE: A retrospective study based on 177 cases of abruptio placentae, and describing the epidemiological, clinical and prognosis aspects. PATIENTS AND METHODS: A hundred and seventy-seven cases, corresponding to 185 children, were registered over a five-year period in the department of gynecology and obstetrics of the CHNYO of Ouagadougou. RESULTS: Abruptio placentae occurrence rate was about 9.6 per 1000 deliveries. In our study, this type of accident was most frequent with 30 to 34-year-old women (31.1%), with multiparous ones (56.5%), and with those suffering from arterial hypertension linked to pregnancy (31.1%). The clinical picture was most often complete, 83.1% of patients having reached grade 3 of abruptio placentae with complete symptomatology and foetal death. Vaginal delivery was preferred to cesarean section in 64.4% of the cases. Maternal death rate was about 3.9% and mainly caused by severe anemia (61.6%) and puerperal infections (7.9%). Foetal prognosis was dominated by the high rate of mortinatality (85.9%). DISCUSSION AND CONCLUSION: To reduce maternal mortality as well as morbidity due to abruptio placentae, correct prenatal follow-up, early diagnosis and prompt evacuation of the womb are required.

[Determinants of risk factors associated with severe maternal morbidity: application during antenatal consultations].

In order to assess the incidence of severe maternal mortality (SMM) and search for associated determinants of risk factors, a population-based survey of 20,326 pregnant women was conducted in six counties in West Africa (MOMA study). Complete files were available for 19,545 (96.2%) women. The rate of SMM, assessed with an overall indicator, was 6.7% (5.7-6.4). The relationship between SMM and various socio-economic features, gyneco-obstetrical antecedents and characteristics of the current pregnancy was studied. Multivariate analysis identified 10 factors of risk of SMM. Prevalence, adjusted odds-ratio, positive predictive value and adjusted attributable risk were determined for these ten factors. By decreasing order of positive predictive value, these factors were: hemorrhage during pregnancy (51.5%), antecedent cesarean (27.1%), high blood pressure (diastolic pressure 10 (18.9%), systolic pressure 14 (14.6%)), antecedent multiple pregnancy (15.8%), height 150 cm (12.9%), lack of fetal movements (12.6%), history of 3 stillborns (9.3%), age over 35 years (8%), nulliparity (7.3%), presence of disease during the pregnancy (7.1%). These results show the importance of antenatal consultation during which these factors are easily identifiable.

[Features of hysterosalpingography performed in Burkina Faso].

PURPOSE: The goal of this study was to describe the distinctive hysterosalpingographic features of women with infertility in Burkina Faso. MATERIALS AND METHODS: We performed a retrospective study of 1019 women who were referred for infertility between January 1996 to June 1997 at the National Hospital Centre of Ouagadougou. Clinical and radiological features were reviewed and statistical data analysed on the software EPI Info 5.0. RESULTS: The mean age of the patients was 28+/-0,3 years. Primary infertility was found in 34.4% and secondary infertility in 65.6%. Abnormalities on plain film were present in 6.9% of the cases. Hysterosalpingography was abnormal in 62.1% of the cases. Tubal abnormalities were found in 62% and uterine lesions in 38.3%. The main hysterosalpingographic abnormalities were tubal obstructions, hydrosalpinx, uterine fibromas, and trauma sequelae in 25,9%, 25,3%, 16,5% and 10% of cases respectively. Seventy one percent of tubal and uterine lesions represent outcomes of chronic infections and underground abortions. CONCLUSION: In Burkina Faso, hysterosalpingographic abnormalities are characterized by the predominance of infectious and traumatic sequelae. Such a frequency could be reduced by active politics against underground abortion and information about genital infections.

[Ultrasound foetal biometry in the town of Ouagadougou, Burkina Faso. Report of a cohort of 126 pregnant women].

PURPOSE: This inquiry on the ultrasound fetal biometry in the town of Ouagadougou aimed to study the fetal growth through the ultrasound biometry in order to compare it with other authors's works and to lay down a local chart. PATIENTS AND METHODS: This prospective study led from March 1997 to April 1998 The follow-up of 126 pregnant women allowed to lay down the graphs of following elements: biparietal diameter, cephalic circumference, transverse abdominal diameter, abdominal circumference and femur. RESULTS: The average age of the subjects was 26 years old+/-0.8 and 52.4% of them belonged to an average socio-economic level. All the studied parameters were closely connected with the age of pregnancy. The DAT and the abdominal circumference presented the greatest correlation (0.998). The comparison of our growth graphs with those of others authors led to notice significant differences as well as with African, European and American graphs. CONCLUSION: There is a need to lay down local charts in order to have a best estimation of the pregnancy age and the fetal growth.

Potential source of human exposure to Mycobacterium bovis in Burkina Faso, in the context of the HIV epidemic.

OBJECTIVE: To identify potential sources of human Mycobacterium bovis infection in Bobo-Dioulasso, Burkina Faso. METHODS: A tuberculin survey among 174 cattle was performed. Mycobacteriologic identification in 64 samples of pooled milk, and in 199 tissue samples collected from the slaughterhouse of Bobo-Dioulasso, Burkina Faso, was also done. We retrospectively analyzed the distribution of tuberculosis (TB) cases on 1140 clinical records according to professional occupation and to ethnic group. The frequency of pulmonary and extrapulmonary TB was related to potential exposure and route of transmission of M. bovis from animals. RESULTS: Out of six herds (total 170 bovines), only one was free of any positive tuberculin test. Among 199 bovines which had been slaughtered over four consecutive nights, 38 (19%) had morphologic lesions suggestive of TB; 17 (45%) of those were positive for acid-fast bacilli by microscopic examination on one of their lesions, and 20 samples (53%) presented a positive culture for a pathogenic mycobacterium, including M. bovis and M. tuberculosis. In the retrospective analysis, Peuls more frequently had a pulmonary form of disease. This may be related to the route of transmission. CONCLUSIONS: Attention has to be paid to human TB of bovine origin in Burkina Faso. The identification of M. tuberculosis in milk and in tissue samples raises the question of the transmission of TB from humans to cattle.

[Contraception with levonorgestrel (Norplant) subcutaneous implants. African experience in Burkina Faso].

The subcutaneous implants from the levonorgestrel (Norplant) has been introduced in Burkina Faso in September 1992 within the context of a project assisted by a non-governmental organization (the Population Council). The aim was to reinforce the family planning programme in Burkina Faso by increasing the number of family planning methods available for the clients. 4 years after its introduction, we propose to make a report of our experience in terms of the contraception through subcutaneous implants in order to better set up an IEC campaign on this method. Within 4 years, 1,660 users benefitted from that contraceptive method. They were housewives in 65% of the cases. The age-group of 30-34 years old most used this method with 28.6%. Multipares most benefitted from this method with 64.3%. In 72% of the cases, the insertion was made during the premenstrual period. On the whole, 247 withdrawals have been made before the fourth years for various reasons. Among those reasons are cycle disorders (60 withdrawals), medical reasons (53 withdrawals), personal conveniences (47 withdrawals), weight gaining (14 withdrawals), failures (2 withdrawals). Regarding the side effects, they were mainly represented by the menstrual cycle disorders such as amenorrhoea, spotting, hypermenorrhoea in 51% of the cases. A good information and sensitization campaign should reduce the number of implants withdrawals before the fourth year of use. Moreover, a perfect knowledge of contraindications is indispensable before any prescription.

[Screening of post-partum depressive states in the Yalgado Ouedraogo National Hospital Center maternity ward in Ouagadougou Burkina Faso].

OBJECTIVES: Depression is a frequent early complication of the post-partum period. It affects the mother's own psychological function and has an important impact on the mother-child relation. PATIENTS AND METHODS: We recruited a successive series of 34 women (mean age 27.4 +/- 4.5 years) who delivered at the Yalgado Ouedraogo University Hospital maternity ward. Patients were evaluated with the Beck Depression Inventory on day 3 and 8 post-partum and a questionnaire was used to record social, demographic and sanitary data. RESULTS: Average Beck Depression Inventory scores on day 3 and 8 were 9.3 +/- 4.5 and 13.5 +/- 6.2. In our series, the state of depression was the same on day 3 and 8 in 78.6% of the mothers. Mood disorders were significantly correlated with parity and the sex of the child. Other variables studies (age, occupation, matrimonial status, mode of nursing) did not influence significantly the development of a mood disorder. CONCLUSION: The psychosocial environment plays a determining role in the development of depressive states. Adequate management requires cooperative care by the obstetrician, psychiatrist and pediatrician.

[Female sterilization in Burkina Faso. Report of 142 cases].

Through a transversal study over a twelve month period, the authors report 142 cases of female sterilization (FS) in Burkina Faso. The aims of this study were to determine the profile of women undergoing FS and to precise the indications and the immediate outcome. The study took place in the two main gynecologic and obstetric units of the country. Various data about the socio-demographic characteristics of the women, their reproductive history, the indications for the FS and the short term follow up were recorded. The woman undergoing tubal ligation (TL) is 36 years old, gravida 7 para 7 with 5 children (3 boys for 2 girls). Her occupation is housewifery, she is married and is illiterate in 84.4% of the cases. This woman has no prior history of contraceptive use in 71% of the cases. FS was performed for medical reasons in 50% of the cases whereas economic reason was invoked by only 5% of the couples. Prior to the TL oral informed consent was obtained from both the woman and her husband. The TL was performed during caesarean section in 55.6%, in the postpartum period in 34.5% of the cases. Two wound abscesses occurred. With a follow up ranging from 2 to 14 months, no pregnancy has been reported. To give a chance to FS to be popular in Burkina Faso, the authors made some suggestions.

[Eighty cases of uterine rupture at the maternity service of the National Hospital Center of Ouagadougou , Burkina Faso].

In this retrospective study, we recall the frequency of uterine ruptures within a national health center of West Africa and propose solutions in order to reduce the incidence. This very high frequency was 23/1000, i.e. one uterine rupture for 44 deliveries. The feto-maternal prognosis was poor with a high stillbirth rate and maternal mortality. Maternal mortality which was 35% of cases represented 22.7% of the causes of maternal mortality observed within the unit during the survey. The stillbirth rate was 95% representing 145/1000 of the hospital stillbirth rate recorded during the survey. Maternal morbidity was also high because 14% of those who survived presented a parietal suppuration with the release of suture during the immediate postsurgical period. The seriousness of the feto-maternal prognosis is linked to a lack of adequate patient management. The importance of uterine ruptures in the obstetrical activity of the unit needs a joint and urgent action of all intervening parties within the sanitary system in order to contend with this scourge, which is the sign of poor quality health care in Obstetrics.

[Effects of malaria outbreak in a quarter of the city of Bobo-Dioulasso (Burkina Faso)].

Weekly malaria attack incidence rate has been studied from June to September 1993 in a cluster of children and adults living in a quarter of the city of Bobo-Dioulasso (Burkina Faso). The city is located in the savanna area with an average of 1000 mm rainfall in five months. In this recently built quarter at the outskirts, yearly average malaria transmission was of four infected bites mainly due to Anopheles gambiae. The medical team visited each week each included family (28 families, 290 persons). Malaria attack has been defined as a fever attack or the certitude of fever during the hours before, with clinical malaria-related signs, a patent parasitaemia and the lack of obvious other etiology. Parasitic density threshold of 2000 PRBC/mm3 for the children and adolescents, till 16 years old and of 1,000 PRBC/mm3 for the adults, have been utilized for the definition of unquestionable malaria attack. 4128 medical observations have been done. 17 unquestionable malaria attacks have been observed in infants, children and adults with parasitic densities from 40,000 to 720,000 PRBC/mm3. 18 likely attacks were also observed with clinical signs but low density of Plasmodium. All were induced by P. falciparum. From June to the end of July the weekly incidence rate was five for one thousand and from August twelve for one thousand. In a place with a low transmission, the success of an infection is higher than in places with an intense transmission. This observation is of some importance for the control of the disease by reduction of the transmission.

Neonatal mid-upper-arm circumference is a valid proxy for birth-weight.

A survey was carried out on a consecutive sample of 973 newborn babies representative for the city of Ouagadougou to assess the validity of neonatal mid-upper-arm-circumference (MUAC) as an indicator of birth-weight. The optimal cut-off point of neonatal MUAC to differentiate between low (less than 2500 g) and normal (greater than or equal to 2500 g) birth-weight babies was 9.5 cm. High values for sensitivity (91%) and specificity (83%) indicate an excellent validity. A newborn with a MUAC of less than 9.5 cm runs a 23 fold risk of being of low birth-weight (relative risk). The advantages of this simple, locally available and inexpensive indicator, which can be used by trained traditional birth attendants (TBA) and by the mothers themselves as a substitute for birth-weight, are discussed.

[Gynecology-obstetrics at the Yalgado-Ouédraogo (Ouagadougou) National Hospital Center. Cancer of the cervix uteri: epidemio-clinical and anatomopathologic aspects].

We report the findings of a 3-year retrospective study aimed at describing the epidemiological, clinical, anatomical and pathological profile of cervical cancers in an African country. We studied 46 cases of invasive cervical cancer. This type of cancer accounts for 31.7% of female genital cancers. The mean waiting time for a consultation was 7 +/- 2.4 months. The average age of the patients was 48 +/- 3.7 years. They were mostly women who had had several pregnancies without paid work. The main reasons for seeking medical help were metrorrhagia (95.6%), pelvic pain (58.7%) and purulent discharge (45.6%). In 89.1% of the cases, the cancer was inoperable. Pathology results were available in 37 cases. We found 36 cases (97.3%) of epidermoid carcinoma and a single case (2.7%) of adenocarcinoma. There were associated condylomatous lesions in 21.6% of cases. These results demonstrate the importance of making cervical smear tests a routine part of medical examination and of making the public aware of the importance of screening for cervical cancer.

[Gynecology-obstetrics at the Yalgado-Ouédraogo National Hospital Center. Eclampsia: epidemiologic, clinical and prognostic aspects].

We present the results of a retrospective study carried out between 1992 and 1995 aimed at describing the epidemiological, clinical and developmental profile of eclampsia in an African maternity unit towards the end of the 20th Century. The incidence of eclampsia was 108 cases in 12,175 births (0.89%), mostly in young patients during their first pregnancy. 40.7% of the patients were less than 20 years old and 59.3% were expecting their first child. Eclampsia occurred between the 28th and 37th weeks of amenorrhea in 37% of cases. Thirty four patients (31.5%) had had at least three episodes of eclampsia prior to admission. Diastolic arterial blood pressure was higher than 120 mmHg in 25.9% of cases. Eclampsia occurred before labor in 30.6% of cases, during labor in 38% of cases and after giving birth in 31.5% of cases. Postpartum episodes occurred an average of 67 +/- 18.7 hours after the birth. There were complication with infection in 7 cases, renal insufficiency in 14 cases and one case of retro placental hematoma. Seventeen patients died, giving a death rate of 15.7%. During the same period, 3.4% of maternal deaths were due to eclampsia. The perinatal mortality rate was 23.1%. A quantitative and qualitative improvement in prenatal consultations should make it possible to reduce the incidence of eclampsia. Measuring arterial blood pressure daily for at least 14 days after the birth appears to be necessary for diagnosis and treatment of all cases of hypertension.
