[Gastric stump cancer with osteoplastic metastases and microangiopathic hemolytic anemia].

In a case of adenocarcinoma in the anastomosis of a stomach resected by the Billroth II technique osteoplastic bone infiltration and microangiopatic haemolytic anaemia are described as paraneoplastic syndromes. Both phenomena open up a great deal of differential diagnoses for consideration. Even when the extended bone infiltration could already be observed, the primary tumor macroscopically in the stomach was not visible. Even an unremarkable B II anastomosis should be biopsied as a matter of routine.

Diamine oxidase activities in the large bowel mucosa of ulcerative colitis patients.

The term colitis suggests mucosal inflammation as the key event. However, it may be that the disease starts with mucosal hyperproliferation, and inflammation of the impaired mucosa is a succeeding event. Therefore we studied the activity of the intestinal diamine oxidase (DAO) in ulcerative colitis (UC). This enzyme was shown to have a mucosal antiproliferative function. Biopsy specimens of 30 patients having a normal rectosigmoidal mucosa showed a DAO activity of 22.8 nmol/min g. In 12 UC patients the DAO activity was 2.7 nmol/min g (p = 0.01). In 3 patients where UC was in remission the DAO activity was 103, 107 and 208 nmol/min g, indicating an antiproliferative rebound effect. Together with the strongly reduced monoamine oxidase (MAO) activity, the decrease in DAO activity indicates that the large bowel in UC is unable to produce a proliferation terminating substance (probably gamma-aminobutyrate) derived from polyamine metabolism by oxidative deamination (DAO) or by the interconversion pathway (MAO).

[Primary and metastatic liver tumors--evaluation of tumor regression and response with regional cytostatic drug therapy by sonography and fine needle puncture histology].

Primary and metastatic hepatic tumours--evaluation of tumour regression or response under regional cytostasis with sonography and fine needle puncture histology. The results obtained through the use of regional liver perfusion with a 5-FU-BCNU application on non-resectable hepatic metastases of colorectal tumours following primary curative removal of the primary tumour and primary hepatocellular carcinomas display comparable results to those specified in the literature after the use of 5-FUDR. The effectivity of the cytostasis regime is checked using histological criteria. The ultrasonically guided fine needle puncture can prove the success of the cytostasis regime on the basis of cytomorphologic criteria more reliably than has been the case up to now. Patients displaying histological signs of tumour regression have a significantly longer survival time than those displaying no signs of regression (12 +/- 9.2 vs 4.5 +/- 2.2 months; p less than 0.05).

[Prevention of stress-induced hemorrhage of the upper gastrointestinal tract in neurosurgical intensive care patients. A controlled, randomized double-blind study with ranitidine alone and in combination with pirenzepine].

During a controlled, randomized, double blind study, 30 patients of a neurosurgical intensive care unit were treated with ranitidin and placebo (group 1) or ranitidin and pirenzepin (group 2). No significant difference in the effectivity of both methods was established, presumably due to the small number of cases. The pH values of group 2 lay in the median above 3.5, whereby the difference only became statistically significant after a seven-day period of therapy. In how far the increase of the pH value represents the only factor of a stress ulcer prophylaxis requires further investigation, particularly in connection with pulmonary risks caused by this.

Steady-state intravenous pharmacokinetics of pirenzepine in patients with hepatic insufficiency and combined renal- and hepatic insufficiency.

The steady-state intravenous pharmacokinetics of pirenzepine has been investigated in patients with chronic liver disease and others with combined chronic liver disease and renal insufficiency. The plasma clearance (CL) of Pirenzepine, steady-state plasma concentration Cmin(ss) and dominant half life t1/2 gamma were not significantly altered in the chronic liver disease group. In patients with renal and hepatic insufficiency, CL was reduced, t1/2 gamma was prolonged from 11.1 to 19.4 h and Cmin(ss) was elevated from 36 ng/ml to 66 ng/ml compared to healthy controls. Plasma concentrations remained in the therapeutic range and the dosage regimen was well tolerated. Adjustment of the dose of pirenzepine need be considered only in cases of severe impairment of both renal and hepatic elimination.

Steady-state intravenous pharmacokinetics of pirenzepine in patients with differing degrees of renal dysfunction.

The steady-state intravenous pharmacokinetics of pirenzepine has been investigated in 57 subjects whose renal function ranged from normal to chronic failure requiring regular haemodialysis. Pirenzepine renal clearance, total clearance and terminal (dominant) half-life were found to be correlated with the creatinine clearance (CLCR), but this was not the case for the volume of distribution and the nonrenal clearance. The therapeutic regimen was well tolerated by all subjects. Haemodialysis did not significantly contribute to the elimination of pirenzepine. Dosage adjustment need only be considered in patients with CLCR less than 25 ml/min in order to reduce the frequency of minor side-effects.

[Angiotensin-converting enzyme and 1,25-dihydroxyvitamin D3 in hypercalcemia of unknown origin].

Sarcoidosis of the kidneys and liver without radiologically demonstrable lung involvement in a 44-year-old woman was shown to be the cause of a hypercalcaemia syndrome. The hypercalcaemia was presumably due to an increased production of 1,25-(OH)2-vitamin D3 which--like the increased angiotensin converting enzyme--is produced in the epithelioid and giant cells. Corticoid treatment normalized serum calcium and ACE levels and improved renal function. These observations demonstrate that measurement of ACE and 1,25-(OH)2-vitamin D3 levels is helpful in the differential diagnosis of hypercalcaemia, in view of the possibility of sarcoidosis, and should be among the diagnostic tests.

Large bowel tumor promotion by diamine oxidase inhibition: animal model and clinical aspects.

UNLABELLED: In the intestinal mucosa, diamine oxidase probably is involved in a feedback regulation mechanism for the termination of proliferation. This was proven by a considerable promotion of large bowel tumors induced by diamine oxidase inhibition in a rat model. Carcinogenesis was accompanied by characteristic alterations in mucosal diamine oxidase activity. In large bowel cancer patients similar changes in intestinal diamine oxidase activity had been observed as in the animal model. CONCLUSION: Events entailing a reduction of diamine oxidase activity are suspicious for large bowel cancer promotion under experimental and clinical conditions.

Large bowel tumors and diamine oxidase (DAO) activity in patients: a new approach for risk group identification.

In the intestinal mucosa, diamine oxidase (DAO) seems to be involved in a feed-back regulation mechanism for the termination of proliferation. Therefore, we studied the DAO activity in large bowel tumors and in the non-affected mucosa of these patients in comparison with patients having a normal large mucosa. The DAO activity in the tumor tissue itself was diminished by 85% as compared to the surrounding mucosa. Comparing the colonic mucosa of normal and tumor bearing individuals, the DAO activity in cancer patients was diminished by 22%, while it was elevated by 64% in patients with polyps (biphasic response of the DAO activity). Histologically proven hyperproliferative mucosal alterations were indicated by a reduced DAO activity with 75% sensitivity and 42% specificity. It remains open whether this limited specificity may indicate a more sensitive reaction of the DAO activity to proliferative mucosal alterations than the histological examinations.

Pharmacokinetics of isosorbide-5-nitrate in renal failure.

The pharmacokinetics of the antianginal drug isosorbide-5-nitrate (IS-5-N) was studied in 20 patients with varying degrees of chronic renal failure after repeated oral doses of standard 20 mg tablets t.d.s. Blood samples were taken in the steady state on the 2nd and 28th days, and the plasma level was assayed by HPLC. There was no statistically significant difference in Cssmax, t1/2 and AUCss0-8 between the 2nd and 28th days, nor was a difference found between patients with mild and severe renal failure.
