[Effect of low and high molecular weight dextrans on selected serum lipid parameters. 1].

Three male patients with hyperlipidaemia were given daily infusions of dextran over a period of 10 days. Plasma levels of total cholesterol, LDL and HDL cholesterol, apolipoprotein B and AI and triglycerides were analysed after the infusion of dextrans different in dose and molecular size. Dextrans caused alterations of some plasma lipids and lipoproteins exceeding the haemodilution effect. The degree of lipid lowering depends on the volume and molecular size of the administered dextran.

[The effect of low and high molecular weight dextrans on selected serum lipid parameters. 2].

The effect of dextran infusions on plasma levels of total cholesterol, LDL and HDL cholesterol, apolipoprotein B and AI and triglycerides is investigated within a patient group. Some plasma lipids and lipoproteins still remain significantly lowered after the end of the 10 days treatment period. Comparing the normolipidaemic with the hyperlipidaemic patients of type IIa and IIb the degree of alterations is different between these 3 groups.

[Protection from uv-light-induced oxidative stress by nutritional radical scavengers].

Two series of examinations were carried out in two voluntary test groups for the purpose of elucidating the correlation between ultraviolet light load and oxidative stress as well as the way it is influenced by nutritive radical scavengers. After a 6 to 7-hour impact of sunshine on the whole body (sunbathing beach) at n = 8 a continuous progredient increase of thiobarbituric acid reactive substances could be identified in the serum (from 5.56 +/- 0.98 to 8.91 +/- 0.99 mumol/l, p < 0.001), which after new exposure to sunshine reached 11.3 +/- 2.4 mumol/l. Likewise, a 15-minute exposure to ultraviolet light at n = 24 induced increases of TBRS concentrations lasting from 1-2 days. After 14-day supplementation with beta-carotene (n = 6), D-alpha-tocopherol (n = 6), selenium (n = 6), and ginkgo biloba extract (n = 6) the extent of the oxidative stress could be inhibited during a second exposure to ultraviolet light up to the following efficiency: Se > Ginkgo > beta-carotene > vitamin E. The clastogenous effect of sunshine and ultraviolet light must be regarded as a factor for initiating and promoting cancerogenesis in the total organism. Concerning the aetiopathogenesis of malignant melanoma the paramagnetic properties of free radicals with their nonenergetic effects of the magnetic field have to be considered more carefully in scientific examinations.

[Superoxide dismutase activity of Ginkgo biloba extract].

The Ginkgo biloba extract is obtained from green leaves of the Ginkgo biloba tree. Preparations with this active substance are among others used for the treatment of disturbances of the cerebral function and arteriosclerotic diseases. In in-vitro and in-vivo studies antagonistic effects of radical scavenger and PAF (platelet activating factor) were described. In this study a concentration-depending superoxide dismutase activity of the Ginkgo biloba extract rökan liquid could be made evident.

Diterpenes

[Anti-oxidative therapy of pancreatitis--an 18-month interim evaluation].

99 patients affected with acute pancreatitis of different genesis were treated in hospital (necrotizing n = 38, mild form n = 61) from May 1990 to November 1991. Nearly 80% of these illnesses were ethanol-induced, 12% were of biliary origin. 90 patients were submitted to an adjuvant antioxidant therapy with selenium and D-alpha-tocopherol (necrotizing form n = 29, mild form n = 61). The average lethality rate of 34% (1982-1989) fell to 1.1% (1 female patient with biliarily induced pancreatitis). No lethal courses were observed in alcohol-induced, idiopathic, post-traumatic, and post-operative forms. Clinical courses proceeded more easily under adjuvant antioxidant therapy, surgical treatment was not necessary. A treatment at reasonable costs can be made in all general internal wards.

Acute Disease

[Acute pancreatitis--a free radical disease. Decrease in fatality with sodium selenite (Na2SeO3) therapy].

Malonic dialdehyde as an indirect marker of the lipid peroxidation was found increased in the acute pancreatitis compared with persons of the same age and sex. Its concentrations inversely correlated to those of the serum calcium during the course of the disease and additionally they proved to be indicator of the prognosis. Postulating that the acute pancreatitis must be a "free radical disease", in a randomized clinical study the adjuvant therapy of the acute necrotizing pancreatitis (n = 8) with sodium selenite was carried out in a daily dose of 500 micrograms. The lethality of the control group was 89% (8 out of altogether 9 patients), no patient died in the therapy group. By the selenium therapy within 24 hours a normalization of the serum calcium and a decrease of the increased MDA-values could be achieved. It was concluded that by selenium increased activities of the phospholipid-hydroperoxide-glutathione peroxidase were induced, by means of which a peroxidation protection of membrane fatty acids, an inhibition of the activity of phospholipase A2 and an interruption of the arachidonic acid cascade must have been reached.

Acute Disease

[Tangier disease--a "free radical"-associated disease. Results of HDL and anti-oxidant therapy with selenium and D-alpha tocopherol].

In a 26-year-old patient there have been benign enlargements of the lymphatic nodes and a splenomegaly since the end of the adolescence. In the 21st year of age the diagnosis of a Tangier disease was made. Allogenic HDL-rich serum fraction (COHN IV/1-fraction, prepared according to the modified method 6) infused under therapeutic aspect led to a prolonged increase of the serum total cholesterol and of the thrombocytes. The results pled for an activation of the reverse cholesterol transport. Excessively high malonic dialdehyde concentrations in the serum were relating to a "free radical"-associated metabolic defect, which was caused by the hypocholesterolaemia, the reduced transport capacity of vitamin E in the plasma and the nutrition poor in selenium and cholesterol, respectively. Under a nutritive antioxidant supplementation with sodium selenite and D-alpha-tocopherol a slight increase of the total cholesterol, of the thrombocytes as well as a normalization of the MDA values could be reached. The chronic oxidative stress appeared in the patient in a distinct lipofuscinosis of the skin and formations of naevus-cell naevi as an expression of massive denaturations of protein-lipids. In the Tangier disease we must reckon with an increased mutagenic effect of free radicals with an additional DNS repair capacity as well as an increased sensitivity to radical-generating cancerogenic xenobiotics.(ABSTRACT TRUNCATED AT 250 WORDS)

[Latent antioxidant deficiency in the East German population--causes and clinical significance. II].

The hitherto yielded results of the research groups for arteriosclerosis and trace elements of the Medical Clinic of Leipzig University were summarized and valuated. The most important results were than in 800 healthy persons of three counties of the GDR who were examined depending on sex and age a serum selenium deficiency with regional differences was stated and this deficiency could be increased by an antiatherogenic mode of nutrition (n = 94, p less than 0.001). A consecutive depletion of vitamin E out of normal values was to be explained as a reference of an insufficient status of antioxidants. Analyses on nearly 1,000 foodstuffs and sorts of coffee, tea, tobacco, alcohol, etc. showed as cause a high-degree depletion of vegetable foodstuffs in selenium. The established daily minimum uptake of selenium of 12 to 15 micrograms by cooked foods was covered by above all cholesterol-rich animal foodstuffs and was further lowered by their withdrawal. In a double blind study first results of a supplementation of sodium selenite referred to a favourable influence on the immune status. After explanation of the reasons which may lead to an additional insufficiency of antioxidants was postulated that the protection from activated oxygen species is indicated both for the primary prevention of autoimmunigenic, atherogenic and cancerogenic diseases and for the curative medicine by a supplementation of selenium.

Influence of high density lipoprotein (HDL), prepared from human blood, on prostanoid formation, serum and tissue lipids and development of arteriosclerosis in cholesterol rich fed rabbits.

The i.v. administration of high density lipoprotein (HDL) into cholesterol fed rabbits decreased statistically significantly the serum level of total cholesterol and of low density lipoprotein cholesterol after a feeding period of 8 weeks. These diminished levels of cholesterol were associated with a statistically significant reduction in the levels of cholesterol esters in kidneys and platelets but not in hepatic tissue or in aorta. Macroscopically detectable arteriosclerosis was not statistically significantly diminished. The formation of prostanoids by the aorta remained unchanged. The atherogenic role of immunologic factors acting against the heterologous HDL may have compensated for the antiatherogenic HDL action on plasma and tissue lipids.

[Computer-aided image processing of digital subtraction angiographs of the pelvis-leg region].

Within a clinical study on possible regression of arteriosclerotic processes in humans i.v. DSA-images were made and objectively evaluated. For the first time the digital system Densitron 4 was used for this. The image processing nodes of contrast enhancement, gray-scale and color conversion and stenosis evaluation by density sums are demonstrated for two selected cases from 52 intravenous digital subtraction angiographies of the pelvis-leg region. The results show, that digital image processing can enhance information-relevant detail. The preprocessing of densitometric values accounts for the third dimension of the vessel in its morphologic description. Compared with subjective visual evaluation the digital image interpretation facilitates a much higher degree of detectability of small detail located near the vessel walls and a more objective and reproducible evaluation of the degree of stenosis.

[In vivo effects of high density lipoprotein rich serum fractions (HDLF) in the human. 1. Modification of circulating thrombocyte aggregates, cardiovascular parameters and lipid metabolism by HDLF].

By means of the modified Cohn-method 6 Cohn-IV-1-fractions were prepared in the small pool technique as sterile, pyrogen-free and HDL-rich serum fractions (HDLF), the HDL-concentrations of which were 15-20 g/l. In in-vitro investigations they inhibited the spontaneous aggregation of erythrocytes as well as the ADP-induced aggregation of thrombocytes and activated the molar and fractional initial cholesterol estirification rates as well as the prostacyclin synthesis. From HDLF intravenously applied in people as immediate effects result significant decreases of the cardiac pre- and afterload, an increase of the cardiac ejection fraction as well as a complete dissolution of circulating aggregates of thrombocytes. In 35 patients with hyperlipidaemias after infusion of 100 ml HDLF up to the 5th day increasing and for 14 days remaining increases of concentration of biliary bile acids, total phospholipids and of cholesterol were observed in lithogenic indices remaining in the normal region. The plasmatic lipo- and apolipoproteins reacted with increases, decreases or constancy of concentration which might be based on the different ethryopathogeneses of the hyperlipoproteinaemias.

[In vivo effects of high density lipoprotein rich serum fractions (HDLF) in the human. 2. Effect of repeated HDLF infusions on atherogenic risk factors and arteriosclerotic vascular changes in patients with and without hyperlipoproteinemia].

10 patients with hyperlipoproteinaemia and 4 normolipaemic smokers with angiographically proved arteriosclerosis of different degrees of severity were infused in 2-week intervals 100 ml HDL-rich Cohn-IV-1 fractions each. After the infusion for 5 days essential phospholipids were injected and a permanent medication with cholestyramin and a choleretic drug were prescribed. In the patients with HLP increases of concentration, decreases of concentration, constancy of the plasma lipids and apoproteins independent of the phaenotype of the HLP as well as an improvement of the state of the antioxidant state were observed. In 6 of 10 patients in reproducible vascular areas regressions of athero- und arteriosclerotic findings could be made evident. Progressions were not observed also in extremely changed arteries in contrast to the normolipaemic smokers and the patients of the control group.

[The importance of the trace element selenium in internal diseases].

Issuing from the existence of selenium and its interactions with other elements its essentiality in biochemical processes of man was explained. In correspondence with other research teams also own results speak for a relative selenium deficiency in the Middle-European region. Alterations of the cell integrity caused by selenium deficiency may play a role in the etiopathogenesis and perpetuation of chronic cardiovascular diseases as well as of disturbances of the immunoregulation. Recent epidemiological studies and clinical experimental investigations show potential tendencies for prophylaxis and therapy of diseases which are remarkable in health policy.

Influence of a plasma fraction of human blood, rich in high density lipoprotein, on in vitro formation of prostaglandin I2 (PGI2) and thromboxane A2 (TXA2).

The influence of a plasma fraction of human blood, rich in high density lipoprotein (HDL), was investigated on the "in vitro" formation of PGI2 and TXA2. The addition of 1 mg HDL-cholesterol per ml incubation fluid stimulated significantly the biotransformation of prostaglandin H2 into PGI2 by the microsomal fraction of pig aorta. The TXB2 formation capacity of whole clotted blood was inhibited by administration of HDL in a dose dependent manner. These results suggest that added HDL is able to enhance the ratio PGI2:TXA2. This did not depend on the preparation of HDL either by ultracentrifugation or by precipitation.

[Initial experiences with the effect of chronic intermittent electromyostimulation in patients with hyperlipoproteinemias and arterial circulatory disorders].

Metabolic and clinical effects of an original electromyostimulation are explained which induces muscle contractions by changes of the field intensity in the region of microseconds in forms of delivered impulse groups. On account of the good local compatibility due to optimal irritation parameters the muscle stimulator was suitable for a long-term application. Examinations on 23 patients with hyperlipoproteinaemia and mixed HLP (n = 15) and primary hypercholesterolaemia (n = 8) after a 2 times 2 hours/day mutual stimulation of the femoral musculature in a frequency of 2 s-1 lasting four weeks yielded a significant decrease of the triglyceride and cholesterol concentrations as well as an increase of the HDL-cholesterol in mixed HLP-forms. Examinations not yet finished refer to an increase of the extrahepatic activity of the postheparin-lipoprotein lipase and to the concentration of the HDL2-cholesterol. An influence of the hypercholesterolaemia IIa could not be made evident. An electrostimulation performed for 3 months in 8 patients with obliterating arteriosclerosis resulted in a more sufficient collateral blood supply of the legs. The long-term use of the device was unobjectionable also under ambulatory conditions. On the basis of additionally gathered experiences further indications of an electrostimulation were cited. Possible variations of the electrostimulation irritation parameters might make accessible further fields for the preventive and curative medicine.

[Results of a long-term study of arteriosclerotic circulatory disorders with polyene fatty acid therapy].

The antiatherogenic oil diet as one factor of therapy causes the hope of favourable effects in the complex primary prophylaxis. By means of this alone, however, a regression of a clinically significant arteriosclerosis cannot be achieved within 3 years, through biochemically unequivocal "antiatherogenic" effects are to be proved. Linseed-oil, fish and fish oil seem to be particularly suited to cause these effects desired.
