Cancer carve outs, specialty networks, and disease management: a review of their evolution, effectiveness, and prognosis.

Specialty care programs for patients with cancer were among the first to be developed, yet they have been some of the slowest to grow or to demonstrate success. This paper reviews the evolution of cancer carve outs, disease management, and specialty networks by distinguishing purchasers from sellers on key attributes. It also describes financing and operational impediments to their growth and summarizes what little published data there is documenting the success of these programs. The paper analyzes the critical factors impeding the development of these cancer programs, and discusses the public policy changes and health services research that will need to be conducted before the performance and market influence of cancer carve outs will reach their full potential.

Cancer carve outs and outcomes measurement programs: an emerging paradigm.

Determining how to provide optimal, cost-efficient cancer care may well be one of the most critical issues facing health plans in the 1990s. Although challenging today, the provision of cancer care promises to become even more complex in the near future. To meet the challenge presented by the modern health care environment, some health plans are using the services of a new breed of cancer care delivery system--the cancer carve out.

The genes of the Paracoccus denitrificans bc1 complex. Nucleotide sequence and homologies between bacterial and mitochondrial subunits.

The genes for the three subunits of the cytochrome bc1 complex from the bacterium Paracoccus denitrificans were identified by screening a gene library constructed in pBR 322 for expression using a cytochrome c1-specific antibody. These three genes coding for the FeS subunit, cytochrome b, and cytochrome c1 were located on contiguous sites on the genome in a presumed operon arrangement. The DNA-deduced amino acid sequence shows that all three subunits are homologous to corresponding polypeptides of the mitochondrial cytochrome bc1 complex. Cytochrome c1 of Paracoccus is much larger than its mitochondrial counterpart due to an extra 150 amino acids of unique, highly acidic composition; in addition, it is most likely synthesized as a precursor polypeptide.
