Acardiac twinning where the pump twin dies in utero due to thrombosis in the umbilical arteries.

A case of acardiac twinning where the pump twin dies in utero due to massive thrombotic occlusion of umbilical arteries at a site of umbilical cord stricture of the intact twin. In acardiac twinning the morbidity and mortality of the pump twin can be reduced by some invasive or medical approaches. The authors believe that in this interesting case the death of the pump twin could not be prevented by antenatal treatment.

An acardiac acephalic monster following in-utero anti-epileptic drug exposure.

Acardia, the absence of the heart, is one of the rarest medical anomalies. The exact mechanism which causes this anomaly is still unknown. The authors report the acardiac acephalic fetus of an epileptic mother who was on primidone therapy. The mother who received no antenatal care stopped taking primidone (her sole medication) in the third month of pregnancy with the fear of delivering a malformed baby and had three convulsions until delivery. This is the first reported case of acardia associated with anti-epileptic medication. The cause of the anomaly in this patient may be an unknown genetic defect, the maternal epileptic disorder, the convulsions, the anti-epileptic medication, or a combination of these factors.

Cesarean section rates and perinatal outcomes in resident and midwife attended low risk deliveries.

Cesarean section rates and perinatal outcomes of low risk pregnancies were evaluated in two different Turkish state hospitals. In this retrospective study, we compared cesarean section rates of 1668 low risk and non-private pregnancies managed by residents (group 1) and midwives (group 2), 53.3% and 46.6%, respectively. All patients belonged to a low socioeconomic class and none of them paid hospital costs themselves. Patient characteristics, mode of delivery and perinatal outcome information were studied by blind reviewers. Although patient characteristics and perinatal outcomes among the two groups were not statistically significant (P > 0.05), the cesarean section rate was three times higher in the resident group (9.88% vs. 3.08%) and this difference was statistically significant (P < 0.05). These findings suggest that reducing the expanded use of cesarean section for low risk pregnancies in teaching hospitals would not have an adverse effect on perinatal outcome.

The effect of octreotide (SMS 201-995) on experimentally induced pancreatitis with 50% ethyl alcohol in rats.

Brattle-Boro type rats with average weight of 200 gms were used for the experiment. We established 5 groups with 10 rats in each. Group I was the control group, Group II pancreatic trauma group and Group III rats were the pancreatitis group induced by 50% alcohol. Groups IV and V were the groups in which Octreotide was injected in different time intervals after induction of pancreatitis by 50% alcohol. Amylase values were statistically significant between the control group in which Octreotide was injected in different time intervals after induction of pancreatitis by 50% alcohol. The amylase values were statistically significant between the control group and the experiments (t2 = 4.69 p < 0.001, t3 = 8.06 p < 0.00001, t4 = 4.23 p < 0.002, t5 = 4.3 p < 0.002), and it was also significant between Group III and Groups II, IV, V (t2 = 9.62 p < 0.0001, t4 = 10.26 p < 0.0001, t5 = 3.69 p < 0.005), but it was not found significant between Groups II and IV, V (t4 = 0.52 p < 0.6, t5 = 1.69 p < 0.1). Histopathologic examination of the trauma group showed congestion, minimal lymphomonocyte infiltration. Patchy necrosis and shrinkage of the acinar cells with ductal dilatation were seen in the SMS 201-995 injection groups which were more pronounced in Group V. As a conclusion SMS 201-995 is not effective to prevent the ongoing pathology of pancreatitis but the increasing values of amylase were limited on the level of simply induced traumatic pancreatitis. It may be useful in the suppression of the enzymatic production during the course of pancreatitis.
