[The problem of urinary bladder cancer--an attempt at a therapeutic standard].

The particular uncertainties of the carcinoma of the urinary bladder results from diagnostic and therapeutic deficiencies the causes of which are to be found in the dignity and inclination to dissemination of tumours of the bladder. At present with the help of diagnostic methods which are at our disposal it is difficult to establish an exact clinical staging. Nevertheless it was tried to set certain therapeutic central points. For this purpose malignant tumours of the urinary bladder were subdivided into three groups, taking into consideration the tumour infiltration as well as the tumour grading and adequate therapeutic measures were discussed under a curative, adjuvant and palliative aim.

Combined Modality Therapy↗

[Therapy of advanced prostate cancer].

The carcinoma of the prostate gland is a malignant tumour like the other. The locoregional carcinoma of the prostate gland is locally curatively treated with prostatovesiculectomy, percutaneous high-voltage therapy or interstitial radiotherapy. The disseminated diseases of the prostate gland is systemically palliatively treated. A survey is given of the forms of therapy for the advanced carcinoma of the prostate gland. The estrogen therapy has its justification still nowadays. With the preparation Turisteron (ethinylestradiol propansulphonate) of the VEB Jenapharm an oral depot estrogen is at our disposal, which has distinctly smaller side effects than diethylstilbestrol (DES).

[Transplantation antigen specific rosette test and its inhibition with autologous serum--a new immunologic method for predicting rejection crisis following allogeneic organ transplantation].

A combination of transplantation-antigen-specific rosette test with inhibition of rosette formation, using autologous serum, was used in postoperative follow-up checks on patients who had undergone allogeneic kidney transplantation and proved successful in early diagnosis or prediction of rejection episodes even before clinical manifestation. The numbers of antigen-detecting and antigen-binding cells differed considerably from each other, depending on patients who had undergone kidney transplantation. Almost complete inhibition of antigen detection was generally achieved by addition of patient serum. If sensitization to alloantigen in excess of the limit value (30 RBZ/10(3) KHZ) is accompanied by rosette formation with autologous serum addition in excess of about 18 RBZ/10(3) KHZ, clinically relevant rejection can be expected to occur after another two or three days and will have to be treated by immunosuppressives. Such partial or even complete elimination of rosette inhibition is usually only of short length (one day) and has rarely been observed to last longer. No rejection calling for treatment will develop, on the other hand, as long as blocking serum factors provide for high-stability inhibition of any bond of transplantation antigen (rosette formation). Constantly low rosette formation with serum addition and without was exhibited by control persons (blood donors, patients with cerebrocranial trauma) in daily examinations. Limit values of 18 or 30 RBZ/10(3) KHZ were not surpassed.

Improvement in the preservation of ischemically impaired renal transplants of pigs by iloprost (ZK 36 374).

Kidneys of five pigs were preoperatively perfused with iloprost in situ (infusion of 0.5 microgram X kg-1 for 15 minutes) and exposed to a warm ischemic period of 45 minutes. The behaviour of these kidneys during initial perfusion by gravity and hypothermic mechanical perfusion for 24 hours was comparable to that of kidneys having had a warm ischemic period of less than 5 minutes. After retransplantation all kidneys showed a sufficient blood flow and immediate production of urine during the test period of 3 hours.

[Hemodynamic studies of iloprost-conditioned ischemically stressed kidneys in animal experiments].

By means of computer-assisted Doppler signal analysis on the exposed renal artery of the pig the change of the haemodynamics under the influence of Iloprost is investigated in comparison to the electromagnetic flow measurement. The Doppler sonographic establishment of the speed of the blood flow and the relative volume of the blood flow allows additional informations about characteristic changes of the form of the pulse. During the measurement in situ on the renal artery the result was during the intraarterial application of Iloprost a distinct increase of the renal blood flow with simultaneous decrease of the renal resistance, which, issuing from the Doppler signal analysis, is characterized as change of the renal resistance index. The kidneys conditioned under influence of Iloprost were retransplanted after a warm ischaemia-time of 45 minutes with following machine preservation of 24 hours and showed a stable flow characteristics for the whole investigation period. The Doppler signals directly conducted via the renal parenchyma showed a stable pulsation also in the smaller arterial vessels.

[Effect of iloprost on aggregation behavior in kidney conditioning and changes in iloprost concentration in kidney preservation].

The conditioning of the donor by 1.0 microgram/kg X min Iloprost over 15 minutes leads to a considerable inhibition of the aggregation of thrombocytes in the domestic pig. 160% more ADP had to be added than before the application of Iloprost. When 0.5 microgram/ml Iloprost are added to the perfusion fluid and to the reperfusion fluid 0.22 microgram/ml and 0.27 microgram/ml, respectively, are to be proved in the venous efflux. Whether or not the difference is absorbed or metabolized must remain open. During the 72-hour storage of the kidney in a Euro-Collins-solution with 0.5 microgram/ml Iloprost after 24 hours a decrease of 0.34 microgram/ml, after 48 hours to 0.33 microgram/ml and after 72 hours to 0.20 microgram/ml takes place. Thus Iloprost is present in an effective concentration also after 72 hours. The decrease to one plateau rather speaks for a consumption by active metabolic effect or absorption than for a chemical decomposition.

[Effect of the stable prostacyclin analog (iloprost) on survival of ischemically-stressed swine kidney transplants].

Kidneys of 5 pigs were pretreated in situ with Iloprost (Fa. Schering) (0.5 micrograms X kg-1 X min-1) and exposed to a warm ischaemia time of 45 minutes. The perfusion behaviour of these kidneys during the initial fall current perfusion and hypothermic machine perfusion for 24 hours was to be compared with that of kidneys with a warm ischaemia time of less than 5 minutes. After retransplantation in all kidneys a good blood supply and an immediate urine production was to be seen for the duration of the experiment of 3 hours.

[Extended storage preservation of swine kidneys to 72 hours using iloprost].

The intraaortal infusion of Iloprost (1 micrograms/kg X min) for the duration of 15 minutes leads to an increase of the renal hematorrhea by 85.2%, to a decrease of the renal resistance by 51.6% and to a slight decrease of the average arterial blood pressure by 13.3%. After a 72-hour hypothermic storage preservation under addition of Iloprost the renal blood supply after retransplantation corresponded to the initial values of the blood flow before nephrectomy. The production of urine began after 1-2 minutes. Enzyme investigations of the transplant urine resulted in an increased excretion of the cytosolic enzymes, the structure-bound enzymes, however, remained in the area of the norm. The investigation of the electrolytes of the urine showed a hypersodiuria as well as a hypopotassiuria and calciuria. As to urea and creatinine the values were below the parameters used for comparison. In the contralateral kidneys used as control kidneys no blood supply developed after retransplantation. The application of Iloprost for the conditioning of donor kidneys is very well suited and shows some advantages compared with the preparation used up to now. The prolongation of the hypothermic storage preservation to 72 hours seems to be possible by the application of Iloprost.

[Morphological research on DMSO-perfused kidneys].

It is reported on the morphological investigations of kidneys which had been perfused with the cryoprotective dimethylsulfoxide (DMSO). Some of these kidneys were stored for 72 hours at -4.5 degrees C and then retransplanted. The light-microscopic and electron-optical findings show considerable changes of the cells of the proximal tubulus epithelium and the collecting tubules. These in most cases focal findings of different extension do not differ on principle from the results in preservation techniques without DMSO. The dilatation of the basal labyrinth of the tubulus cell as well as the fissuration and vacuolization in the cytoplasm give references to a passive transport of substance by the cell. The results of the DMSO-perfused and at -4.5 degrees C stored kidneys show that for a beginning of the function an optimal perfusion must be supposed. On the one hand a sufficient concentration of freezing defense remedies in the renal tissue is necessary, on the other hand an optimum removal of the DMSO in the reperfusion is necessary, in order to keep the osmotic load of the kidney as low as possible.

[Bacterial spectrum and pathogen resistance in chronic primary and secondary pyelonephritis].

The analysis of 168 patients with chronic infections of the urinary tract served to clarify whether different findings are obtained concerning the spectrum of germs and resistance in cases of primary, secondary monoinfected and secondary superinfected pyelonephritis. It was found that in cases of secondary obstructive pyelonephritis the degree of chemotherapeutic sensitivity is reduced and especially where problem germs (proteus, pseudomonas, Klebsiella) are concerned therapy will hardly be successful except by means of aminoglycosides. In this context the recognition and senitation of multifactorial urological primary diseases which cause infection are of special importance.

[1st experiences with a urethral endothesis in bladder neck adenoma].

Our examinations hitherto performed allow the following conclusions: 1. The placed before endothesis for the pars prostatica urethrae allows a spontaneous miction which is nearly free of residual urine. Here the endothesis cannot render possible the physiologic funnel-shaping of the vesical neck. This is theoretically not possible and was confirmed by means of fluoroscopic control of the act of miction in lying endothesis. By the endothesis the pars prostatics urethrae is passively kept open. The continence takes place by the so-called sphincter externus and the diaphragma urogenitale (5). The voluntary relaxation of the diaphragma urogenitale and the contraction of the detrusor render possible the depletion of the urinary bladder. By reason of the lacking funnelling the residual urine proved in several patients can be explained as well as the relatively low maximum value of the urinary flow of about 15 ml/s despite a miction which was subjectively regarded as good. 2. The disadvantages of the permanent catheter mentioned at the beginning as well as the usual care of the catheter and bladder irrigations are unnecessary. 3. Incrustations were not observed up to the 78th day.
