Double minutes in human large bowel cancer.

Two patients with large bowel carcinoma are described in whose tumors the presence of double minutes was observed. In one of these, the first such patient to be reported, double minutes were present in 100% of metaphases examined in both the primary tumor and its liver metastasis. The number of double minutes varied from 6 to 93 per metaphase. In the second patient, double minutes were seen only in two metaphases. The literature on double minutes is summarized, and it is concluded that double minutes are found in a wide spectrum of tumors, including human large bowel cancer and are not confined to tumors of neurogenic origin as suggested by earlier workers. In additon, evidence is suggestive of an association between the lack of differentiation and the presence of double minutes, but further data are required to substantiate this.

Chromosome analysis of primary large bowel tumors: a new method for improving the yield of analyzable metaphases.

Colon tumors were processed immediately after surgical resection. Using L-Arterenol and sodium citrate in combination with standard chromosome culture techniques, 12 of 17 consecutive tumors (75%) had countable figures ranging from 5 cells to 59 cells. Seven of the 12 cases had over 25 countable cells. Chromosome counts did not vary more than +/- 4 for any one case. Seven tumors were karyotyped, 6 with banding techniques. One of these specimens had mosaic patterns. The chromosome counts varied: 2 were hypodiploid; 1 was pseudodiploid and 4 were hyperdyploid. There were some similarities in the karotypes although no identical karyotypes were seen in this series.

Urinary diversion for ureteral obstruction in the presence of recurrent colon carcinoma.

Selection of patients who might benefit from urinary diversion when recurrent colorectal cancer obstructs both ureters is difficult. Combinations of chemotherapy and radiation therapy will prolong a useful, comfortable and dignified life in some patients. Radiation therapy seems to be more effective than chemotherapy and, therefore, patients most likely to benefit are those with localized pelvic recurrences without carcinomatosis and those who are well nourished, active and highly motivated. Herein we discuss 6 patients who were diverted and the methods of selection.

Stressbreakers: a practical approach.

The use of stressbreakers for removable partial dentures will doubtless continue to be controversial. Most of the data available for guidance are clinical trials and observations. It is not reasonable to expect a 100 per cent success rate for any biomechanical device. All available designs should be considered in order to choose the right treatment for the situation. When indicated and correctly executed, the 12-gauge chrome wire and the split palate stressbreakers have proved to be practical adjuncts to our treatment armamentarium.

Dietary fiber.

It is obvious that the effects of fiber on colonic functions are complex. The typical "Western diet" which is high in fat and protein and low in fiber may alter the bacterial flora and milieu interieur of the gastrointestinal tract, particularly the colon, to produce a high incidence of colonic cancer. However, the exact nature of the interrelationships between these dietary constituents remains elusive and will undoubtedly be the subject of intense investigation in years to come.

Stricture of the esophagogastric junction with adjacent extraluminal cavity following radiation.

We report a radiation stricture of the esophagogastric junction with an adjacent extraluminal cavity formed secondary to radiation in a patient receiving 4,500 rads for a histiocytic lymphoma of the stomach. We suggest that the presence of a radiosensitive lymphoma may have undermined the normal reparative response to radiation of the esophagogastric junction and predisposed it to stricture and cavity formation at this dose of radiation.

Radiological detection of colonic dysplasia (precarcinoma) in chronic ulcerative colitis.

Epithelial dysplasia occurring in long-standing ulcerative colitis is a precancerous lesion. Macroscopically it has a nodular or villous appearance or may be indistinguishable from the surrounding mucosa. An investigation into the radiological diagnosis of dysplasia, using in vivo and in vitro double-contrast examinations with magnification radiographic studies, correlated with the histological analysis, has been made in four patients. Characteristic radiological abnormalities have been identified in the areas of the mucosa associated with histologically proven dysplasia. These appearances include nodularity and irregular areas with sharply angulated edges which may represent enlarged areae colonicae. The demonstration of these changes is an indication for endoscopic examination and biopsy of the suspicious area.

The prognosis of carcinoma of the colon and rectum complicating ulcerative colitis.

Thirty-three carcinomas of the colon and rectum occurring in patients with ulcerative colitis have been compared with matched controls from a population without colitis. There is no difference in length of survival between the patients in the two groups. More extensive tumor spread in the control group is due to the method of selection.

Ultrastructure of the "transitional" mucosa adjacent to large bowel carcinoma.

The transitional mucosa immediately adjacent to large bowel carcinoma was examined in 13 patients by scanning electron microscopy (SEM), and this was correlated with transmission electron microscopy (TEM) and light microscopy (LM). Marked abnormalities were present in this mucosa in 9/13 patients with distortion and loss of the normal architecture. Examination of adenomas showed abnormalities also to be present adjacent to the neoplastic mucosa. In all patients, the transitional mucosa was compared with two pieces of normal mucosa; in one patient, typical changes of transitional mucosa were found in this region. In the remaining patients, normal mucosa by SEM was found to consist to large deep furrows (primary crypts) as well as the crypts of Lieberkuhn (secondary crypts). The secondary crypts structure showed marked individual variation between patients but one type was present in nine patients. It is uncertain whether this is a pattern with a high incidence in the general population, or whether patients with this type of mucosa are particularly predisposed to large bowel carcinoma. SEM may, therefore, play a part in identifying patients particularly likely to develop large bowel carcinoma.

Azathioprine-associated acute pancreatitis.

Azathioprine therapy in a patient with granulomatous ileocolitis was associated with an attack of acute pancreatitis, which was confirmed by laparotomy. Recurrence of pancreatitis occurred after rechallenge with a single dose of azathioprine.

Pancreatic cancer screening. Analysis of the problem and the role of radionuclide imaging.

The clinical, surgical, and pathologic findings in a five year prospective study of 192 patients referred with a high probability of pancreatic cancer are reported. We have defined the requirements of any pancreatic imaging procedure as its ability to distinguish a normal pancreas from pancreatic cancer or chronic pancreatitis and the capability of detecting tumors less than 5 cm in diameter. There was a 47 percent incidence of pancreatic disease (27 percent pancreatic cancer and 20 percent chronic pancreatitis). Prospective radionuclide imaging as routinely performed was found to be of little clinical value in this patient population; it was neither specific nor sensitive to pancreatic cancer or chronic pancreatitis. Preliminary data with longitudinal multiplane emission tomography show an improved diagnostic accuracy and the ability to detect resectable tumors, but its efficacy needs to be prospectively compared with other screening tests on a carefully defined patient population.
