Changes of ongoing activity in Cebus monkey perirhinal cortex correlate with behavioral performance.

A Cebus apella monkey weighing 4 kg was trained in a saccadic eye movement task and while the animal performed the task we recorded the extracellular activity of perirhinal cortical neurons. Although the task was very simple and maintained at a constant level of difficulty, we observed considerable changes in the performance of the monkey within each experimental session. The behavioral states responsible for such variation may be related to arousal, motivation or attention of the animal while engaged in the task. In approximately 20% (16/82) of the units recorded, long-term direct or inverse correlations could be demonstrated between the monkey's behavioral state and the cells' ongoing activity (independent of the visual stimulation or of the specific behavior along a trial). The perirhinal cortex and other medial temporal structures have long been associated with normal memory function. The data presented here were interpreted in terms of recent reports focusing on the subcortical afferents to temporal lobe structures and their possible role in controlling arousal, motivation, or attention.

Battering in pregnancy: an assessment of two screening methods.

OBJECTIVE: To compare two different screening techniques for identifying women with a history of domestic violence or battering in the current pregnancy. METHODS: The five-question Abuse Assessment Screen was incorporated into routine social service interviews and applied prospectively to all registrants for routine prenatal care at Women & Infants' Hospital during an initial social service evaluation from September 7 through October 29, 1993. This group (N = 143) was compared to a historical control group of all new registrants from July 12 through September 3, 1993 (N = 191) who had routine interviews by social services. Demographic and medical data were compared, as well as the specific information addressed by the screen, including history of domestic violence, physical or sexual violence within the last year, violence during the current pregnancy, recent sexual abuse, and fear of partner. RESULTS: The median age of the study population was 23 years old, 50% were white, 63% were single, and 42% had no insurance. There was a higher detection of violence in all categories using the Abuse Assessment Screen compared with the standard interview--any history: 41 versus 14% (relative risk [RR] 3.0, 95% confidence interval [CI] 2.0-4.5); recent history: 15 versus 3% (RR 5.6, CI 2.2-14.5); during pregnancy: 10 versus 1% (RR 9.3, CI 2.2-40.5); recent sexual abuse: 4 versus 0% (P = .006); and fear of abuser: 6 versus 3% (RR 1.8, CI 0.6-5.0). CONCLUSION: Use of a structured screen improves detection rates of battering both before and during pregnancy, enabling clinicians to have a greater opportunity to intervene.

Supersensitivity to vasoconstrictor action of serotonin precedes the development of atheroma-like lesions in the rabbit.

We have studied the relationship between the early morphological changes and arterial responsiveness to vasoactive agents in a new animal model that is proposed to mimic the events of early human atherosclerosis. Atheroma-like lesions were produced by positioning a hollow Silastic collar (referred to as a cuff) around the common carotid arteries of rabbits. Following a period of either 48 h or 1, 2, or 4 weeks after surgery, vessels from both cuffed and sham-operated animals were removed, and vascular reactivity to cumulative concentrations of agonists were studied in isolated rings in organ baths. The contralateral arteries were perfused and fixed, studied by light microscopy, and the degree of intimal thickening was quantified by computer-assisted morphometric analysis and expressed as changes in the ratios of the cross-sectional areas of the intima and media in each artery. At 48 h, rings prepared from cuffed arteries were sixfold more sensitive to the contractile effects of serotonin (5-HT) than the corresponding controls. Histologically, such vessels showed some perivascular inflammation but no other morphological abnormality. At 7 days, cuffed vessels were again sixfold more sensitive to 5-HT than controls, and showed a thickened intima with marked smooth muscle proliferation and some infiltration by monocytes. Intimal/medial cross-sectional area ratios remained elevated at 2 and 4 weeks, but the supersensitivity to 5-HT diminished by 2 weeks to threefold and was absent at 4 weeks. The augmented reactivity to 5-HT at 48 h was specific, in that it did not occur for the alpha-adrenoceptor agonist, phenylephrine.(ABSTRACT TRUNCATED AT 250 WORDS)

Juvenile Parkinsonism: clinical and metabolic characteristics.

Twenty one patients with idiopathic Parkinsonism beginning before the age of 40 years were investigated. The mean duration of the disease was 19 years. There was a good and sustained response to levodopa. Only four patients reached stages IV and V (Hoehn and Yahr). Intolerance to levodopa was observed in the more advanced stages of the disease. In the series the familial incidence of Parkinsonism (2 cases) and essential tremor (3 cases) was very low. Thyroid disorder, diabetes mellitus or macrocytic anaemia was not found in any of the cases.

Parasites of man in remote areas of Central and South Sulawesi, Indonesia.

A biomedical survey was conducted in 10 villages in remote, high mountain valleys of Central and South Sulawesi, Indonesia to learn whether Oriental schistosomiasis was endemic and to determine the prevalences of other intestinal parasites, malaria and filariasis in those areas. Although persons with Oriental schistosomiasis were found in three villages of South Sulawesi, follow-up inquiries revealed that these had recently migrated from a known schistosomiasis area in Central Sulawesi. Other intestinal parasites diagnosed were Ascaris lumbricoides (14%), Trichuris trichiura (7%), hookworm (68%), Entamoeba histolytica (3%), Entamoeba coli (17%), and Giardia lamblia (5%). Enterobius vermicularis, Strongyloides stercoralis, Trichostrongylus sp., Physaloptera sp., Diphyllobothrium sp. heterophyid, echinostome and dicrocoelid-like termatodes, Endolimax nana and Chilomastix mesnili were detected infrequently. Malaria parasitemias due to Plasmodium falciparum, P. vivax and P. malariae were detected in 4% of the sampled populations, Malayan filariasis was diagnosed in 21% of the subjects examined.

[The elderly patient at the General Hospital of Santo António. Analysis of one year of admissions].

A retrospective analysis was made on 3850 elderly patients who were admitted to the Santo António General Hospital during one year. There were 2029 women and 1821 men. Their mean age was 74 years. They represented 16.4% of the totality of the patients admitted in the same period of time at the same hospital. The main diagnosis presented on each discharge register was made according to the International Statistical Classification of Diseases, Injuries and Causes of Death, 9th Revision. For each of the 17 groups of diagnosis found and for the total group were drawn: number of cases, sex, mean age, the average length of stay, mortality and Departments they were admitted to. The circulatory diseases were the most frequent (24.4%). The average length of stay was 15 days. The mortality was 14.1%. The elderly patients represented about 30% of the admissions to the Departments of Internal Medicine, Cardiology, Vascular Surgery, Ophthalmology and Urology. The results were interpreted and some ideas about future medical care for this group of patients are presented.
