High serum insulin, insulin resistance and their associations with cardiovascular risk factors. The northern Sweden MONICA population study.

OBJECTIVES: To estimate the prevalence of insulin resistance and high serum insulin levels and to investigate their relationship to other cardiovascular risk factors. DESIGN: Cross-sectional cardiovascular risk factor survey. SETTING: Northern Sweden. SUBJECTS: A subsample of the population-based Northern Sweden MONICA Study. This subsample underwent an oral glucose tolerance test after an overnight fast, and consisted of 354 men and 404 women in the 25-64-year age range. MAIN OUTCOME MEASURES: Delineation of low insulin sensitivity and high serum insulin by the diagnostic test technique, prevalence of these variables and their associations with cardiovascular risk factors. RESULTS: The participants were classified into four subgroups by an insulin sensitivity index and fasting serum insulin. The combination of low insulin sensitivity and high serum insulin was present in 17% of the male and in 18% of the female 25-64-year-old population. In both sexes this combination was closely associated (P < 0.001) with body mass index, waist-hip ratio, blood pressure and serum triglycerides, and correlated inversely with serum HDL cholesterol (P < 0.001). When high serum insulin was present as an isolated entity it was as closely associated with other cardiovascular risk factors such as isolated low insulin sensitivity, except that impaired glucose tolerance occurred exclusively in the group with isolated low insulin sensitivity. CONCLUSIONS: The combination of insulin resistance and high insulin levels is associated with a marked clustering of cardiovascular risk factors and is present in one-sixth of the middle-aged population in the north of Sweden.

Coffee drinking and blood cholesterol--effects of brewing method, food intake and life style.

The strongest correlations between coffee consumption and serum cholesterol levels have been found in countries where people drink coffee brewed by mixing coffee grounds directly in boiling water (boiled coffee). In the present study of a population-based sample of 1625 middle-aged subjects (the Northern Sweden MONICA Study), approximately 50% of the participants were drinking boiled coffee, and 50% were drinking filtered coffee. Consumers of boiled coffee had significantly higher serum cholesterol levels than consumers of filtered coffee. Subjects who drank boiled coffee reported a higher intake of fat. A linear multiple regression analysis with serum cholesterol as the dependent variable confirmed that boiled coffee was an important independent determinant of cholesterol levels. We conclude that subjects who drink boiled coffee have higher serum cholesterol levels than those who drink filtered coffee, and that the most likely explanation for this finding lies in the type of brewing method.

Endometrial carcinoma: current concepts and future perspectives.

Endometrial carcinoma is increasing worldwide and, in most of western Europe and the U.S., it is now the most common malignancy in the female pelvis. The most common parameters in deciding therapy are staging, according to the International Federation of Gynecologists and Obstetricians (FIGO) and histopathological grading, and evaluation of myometrial invasion. The treatment results in terms of a 5-year survival rate, are about 90% for well and moderately differentiated stage I to II endometrial carcinomas, whereas that for poorly differentiated ones is about 70%. The latter figures point towards the need for further prognostic parameters to identify subgroups with a fatal form of the disease. One such parameter might be the concentration of steroid receptors and promising results have been published recently but still better prognostic parameters have been proposed recently. It is known that the DNA content in individual tumor cells is a prognostic parameter. Due to the development of flow cytometry such determinations are rapid and more precise, and the prognostic significance has been proved to be superior to the more commonly used parameters.

Prognostic value of steroid receptor concentration and flow cytometrical DNA measurements in stage I-II endometrial carcinoma.

In order to make a more individualized therapy possible in endometrial cancer we investigated the prognostic ability of steroid receptor concentration and the DNA content in individual cells as measured by flow cytometry. We found that although the steroid receptor concentration was a prognostic parameter, the DNA content proved to be a better parameter than all other known prognostic parameters in endometrial carcinomas stage I-II. Thus it was possible to divide the tumors into three groups; 1) diploid tumors with a relapse rate of 6%, 2) aneuploid tumors with a high estradiol receptor concentration and without myometrial invasion of more than the inner third, and with a relapse rate of 18%, 3) aneuploid tumors, poorly differentiated with a low estradiol receptor concentration or myometrial invasion and with a relapse rate of 44%.

The nevoid basal cell carcinoma syndrome--Gorlin's syndrome. Multiple jaw cysts and skin cancer.

The nevoid basal cell carcinoma (NBS) syndrome with its characteristic multiple cysts of the jaws and skin cancer is of great interest to the dentist, the dermatologist and the oncologist. The dentist may have the opportunity of making an early diagnosis as the multiple cysts appear mostly in the ramus of the lower jaw. There is a great risk of recurrence as surgical removal is often very complicated. The disease is hereditary and is transmitted through females. The skin cancer can in most cases be treated surgically. It is essential that the family with the syndrome is investigated regularly and at periods of about six months.

Identification of chorion villi in abortion specimens.

The incidence of ectopic pregnancy is rising. All patients with a diagnosis of spontaneous abortion should be observed until they have been proved not to have an ectopic pregnancy. To help in identifying patients with an ectopic pregnancy, sensitive human chorionic gonadotropin tests, ultrasound, and laparoscopy have been used, yet there has been a tendency to forget the simple method of looking for chorion villi. In material specimens of 272 patients with the initial diagnosis of spontaneous abortion, the gynecologist could identify villi in 50% and the pathologist could identify another 30%. Thus, only 20% had to be observed by other methods and one-third proved to have an ectopic pregnancy.

Plasma steroid hormones, cytosol receptors, and thymidine incorporation rate in endometrial carcinoma.

Histopathologically evaluated endometrial carcinomas were analyzed for the presence of estradiol and progesterone receptors and for tumor thymidine incorporation rate. Plasma estradiol and progesterone concentrations were also measured. With the use of an improved assay, all endometrial carcinomas were found to contain estradiol receptors; 81 of 92 (88%) contained measurable concentrations of progesterone receptors. The concentrations of both receptors were positively correlated to the degree of differentiation. The moderately and poorly differentiated tumours could be further divided into subgroups with low and high concentrations of receptors, which might be a reflection of an independent biologic characteristic of a tumor. These subgroups did not differ in any of the other variables studied, including the age of the patients. The plasma concentrations of estradiol showed a positive correlation to the degree of differentiation and to the concentration of both receptors. It is suggested that improved analyses of estradiol and progesterone receptors in endometrial carcinomas may add information to the conventional histopathologic and clinical classification which will be of significant value in the proper treatment of the disease.

Serum lipoprotein lipid and apolipoprotein concentrations in grossly obese patients before and after jejuno-ileal shunt operation.

Fourteen grossly obese patients were studied before and 2, 6, 12 and 24 months after jejuno-ileal bypass. The major weight loss occurred during the first 6 months while there was no significant change of the mean body weight during the last 12 months of the study. During the initial period of pronounced weight loss there were significant reductions of all lipoprotein classes, e.g. at 2 months the very low density lipoprotein triglycerides had decreased by 41% (P less than 0.01), the low density lipoprotein cholesterol by 42% (P less than 0.001), the high density lipoprotein cholesterol by 27% (P less than 0.001) and the serum apolipoprotein B, A-I and A-II concentrations by 30% (P less than 0.001), 17% (P less than 0.001) and 27% (P less than 0.01) respectively. The fractional removal rate (K2) at the intravenous fat tolerance test had increased by 38% (P less than 0.01), indicating an increased removal capacity of triglyceride-rich lipoproteins. At 24 months the very low density lipoprotein triglycerides, the low density lipoprotein cholesterol and the apolipoprotein B concentration remained reduced by 28% (P less than 0.05), 48% (P less than 0.001) and 35% (P less than 0.001) respectively. The intravenous fat tolerance test K2 remained significantly increased by 44% (P less than 0.05). However, the high density lipoprotein cholesterol and the apolipoprotein A-I and A-II concentrations had successively increased again to preoperative levels. The low density lipoprotein cholesterol/high density lipoprotein cholesterol ratio was decreased by 44% (P less than 0.001) and the apolipoprotein A-I/apolipoprotein B ratio had increased by 74% (P less than 0.001), both changes presumably beneficial in regard of atherogenicity. The apolipoprotein A-I/high density lipoprotein cholesterol and apolipoprotein B/low density lipoprotein cholesterol ratios had increased by 23% (P less than 0.001) and 28% (P less than 0.001) respectively. The data indicate that the reduction of high density lipoprotein may be a transient phenomenon during the initial period of weight loss but that definite changes occur in the high density lipoprotein and low density lipoprotein composition which are apparent also 2 years after surgery.

Some aspects of prophylactic oophorectomy and ovarian carcinoma.

The early diagnosis of ovarian carcinoma is as difficult and rare as ever before. Despite the appearance of sophisticated drugs and combinations of these with surgery and radiotherapy the prognosis still remains the same as during the last 30 years, i.e. overall survival between 20 to 30%. This study concerns the impact of prophylactic oophorectomy in Sweden. 4.6% of ovarian carcinoma patients had received pelvic surgery at some time prior to the diagnosis of ovarian cancer and after the age of 40. With the help of statistical data it is estimated that about one hundred out of the approximately thousand women per year who now develop ovarian cancer in Sweden could be saved if prophylactic oophorectomy were practised in our country. Various aspects of prophylactic oophorectomy and its effects are discussed. It is our opinion that every gynaecologist/surgeon should seriously take into consideration and discuss the possibility of prophylactic oophorectomy in the perimenopausal patient before operation. Not to perform prophylactic oophorectomy during pelvic surgery after the menopause seems to us to be an unreasonable practice.
