[Surgical treatment of purulent necrotic complications in patients with diabetic foot syndrome].

Results of treatment of 207 patients with purulent-necrotic complications of the diabetic foot syndrome (DFS) were analyzed. Tactics of treatment was determined considering the DFS form presented, stage of the extremity ischemia, character and severity of purulent-inflammatory affection and the patient's homeostasis state. Urgent operation was performed in 79 (38.1%) patients, in 38 (48.1%) of them--amputation of extremity. In late follow-up period 98 (47.3%) patients were operated on, in 76 (75.5%) of them necrectomy or small amputation was performed, allowing to preserve the extremity. Conservative treatment was conducted in 30 (14.5%) patients with trophic ulcers. Application of elaborated program of treatment had permitted to increase the preservation frequency of supporting function of foot from 28.9 to 56.3% and to reduce general lethality from 18.3 to 13.5%.

[Preparation of the large intestine for the surgical treatment of intestinal obstruction].

Obstruction of the large intestine of tumorous origin was treated by the traditional methods in 191 patients: an emergency operation in peritonitis or nonoperative management of the obstruction by cleansing enema, purgatives, and infusion-transfusion therapy. In another group of 341 patients emergency colonoscopy and ultrasonic diagnosis were resorted to and the "obstruction index" (Deltz et al. 1989) was used for objectification of the indications for an emergency operation. The method of fractional oral treatment with 30% solution of polyethelyne oxide-400 and 1% chloramphenicol was applied in this group. Traditional treatment caused resolution of the obstruction in 22.8% of patients in the control and in 28.3% of patients in the experimental group. Postoperative mortality was 24.1% and 16.4%, respectively. The obstruction index allowed correct evaluation of the indications for operation in 97% of cases, the number of forced operation reduced from 52.2% to 14.1%.

[Prevention of peritonitis after surgery of the stomach and large intestine].

We have used the combined preparation of foamy aerosol Gyposol-A and nitasol for the prophylaxis of postoperative peritonitis, originated due to gastrojejunal and colo-colonic anastomoses insufficiency. The introduction of preparation via the colonic tube ensures the physical hermetic control of anastomosis and allows to optimize the reparative processes by means of local antimicrobial, antiinflammatory, and stimulant action. Method was used in 76 patients. The frequency of postoperative peritonitis occurrence decreased from 20 to 2.3%.

[Use of multicomponent ointments on a hydrophilic base in the treatment of suppurative wounds].

The results of experimental and clinical studies on the development of multicomponent ointments on a hydrophilic base for local treatment of wounds at phase I of the inflammatory process have been analyzed. A pronounced therapeutic effect of the use of the remedies suggested (levosin, levomecol, dioxycole, sulphamecole, iodmetroxide) resulting from simultaneous dehydrating, antimicrobial, necrolytic and anesthetic effect, 2-fold shortening of the period of wound treatment and improvement of the results of treatment of the patients with different types of purulent surgical infection were noted.

[Effect of some drugs on the intensity of lipid peroxidation in surrounding tissues in the local treatment of suppuration].

The effect of some drugs in their local use for treatment of purulent wounds on the intensity of peroxide oxidation of lipids in the perifocal tissues of an experimental suppurative focus was studied. The increased antioxidant and antiinflammatory effect in use of lyposomal form of gentamycin with the ectericide-modified membranes versus ectericide solution as well as more pronounced antioxidant and antiinflammatory effect of ferecterisole vs. cimesole was revealed. The results obtained indicate that the use of ferecterisole and lyposomal forms of some antibiotics for local treatment of purulent wounds has good prospects.

[A method of preparing the large intestine for surgery by using the peroral administration of a polyethylene oxide solution].

A new method for preparing the large intestine for operation is suggested, it is based on oral administration of a solution of polyethylene oxides with chloramphenicol. The preparation possesses a marked hydrophilic activity, which causes inhibition of absorption processes in the lumen of the intestinal tube. This leads to accumulation of the intestinal contents and their quicker passage. Polyethylene oxides also ensure accumulation of antibiotics in the tissues of the intestinal wall. The suggested method was used to prepare the intestine for operation in 93 patients. Comparative evaluation proved its advantages according to the main clinico-laboratory criteria: the method is technically simple, reduces the period of preparation of the intestine, has no negative effect on the homeostasis values, ensures adequate preparation of the large intestine with decrease of the level of its microbial contamination. With the use of the method the number of postoperative purulent complications in interventions on the large intestine reduced.

[Changes in the pancreatic ducts and the tactics in the treatment of acute pancreatitis].

On the basis of multiple-factor analysis of changes of the pancreatic duct system in acute pancreatitis and comparison with the criteria of its clinico-laboratory characteristics, the authors determined the contribution of the system to the pathogenesis of the disease and the formation of the nature of the affection of the organ (edematous or destructive pancreatitis). A mathematical model of the pancreatic system in acute pancreatitis was developed, which allowed the form of the disease to be recognized from 3 dominating factors. Improved complex diagnosis of acute pancreatitis with appraisal of the condition of the pancreatic duct system, and a differentiated approach to the choice of the operation and the optimal time of its performance made it possible to improve the general results of the treatment of patients with acute pancreatitis, evidence of which is presented in comparison of the indices of 1981-1983 with those of 1984-1986. Thus, despite the growth of surgical activity in 1984-1986 (from 19.6% to 32.2%), postoperative mortality was 14.7%, whereas in 1981-1983 it was 29.2% in a practically stable value of total mortality in the recent 6 years (8.1-7.7%).

[The surgical treatment of acute cholecystitis in patients in a group at increased risk].

The article deals with the results of treatment of acute cholecystitis in 190 patients of the increased risk group. An urgent operation was performed on 25 patients, the others received intensive nonoperative treatment and were subjected to complex examination aimed primarily at assessing the hepatic function (ultrasonic examination with photometric determination of screen luminescent brightness, intraduodenal contact electrothermometry, plasma biochemical tests). In failure of this treatment 88 patients underwent transhepatic microcholecystomy, one-stage external-internal drainage was carried out in 43 patients. Operation was undertaken in the cold period in 104 patients; chloroform with procaine hydrochloride was administered through a catheter in 15 patients to dissolve gallstones.
