Sensitive method for the determination of baclofen in plasma by means of solid-phase extraction and liquid chromatography-tandem mass spectrometry.

A simple and sensitive method for the determination of baclofen in plasma is described. Baclofen and the internal standard, KM 08205, were isolated from plasma by solid-phase extraction using C18 material. After separation by reversed-phase liquid chromatography, the analytes were detected with tandem mass spectrometry. The extraction procedure was optimised regarding the solid-phase extraction material, the pH in the conditioning solution and the washing volume. The method was proven to be selective and sensitive with an absolute recovery of about 95%, a relative standard deviation below 5% and a limit of quantification of 10 nmol/1.

A harness and computer system to facilitate automated body temperature data collection in heat-stressed broilers.

An easy-to-use, low-cost system was developed that permitted nearly continuous, automated core body temperature (Tc) readings on 7-wk-old male broiler chickens via direct computer linkage to thermistor probes held in place by a specially designed harness. Elevated Tc was noted in heat stress studies following the replacement of expelled temperature probes in some hyperthermic birds. To demonstrate the usefulness of the data collection system described herein, three treatments with three to four birds per treatment were used to examine this observation. Birds were designated as handled only (HAN), handled to remove and replace the temperature probe (RPL), or left as nonhandled controls (CON). Treatments had no effect on subsequent Tc in experiments when the thermoregulatory capacity of the birds was not challenged. However, when the birds were sufficiently challenged, Tc of HAN and RPL birds increased within 4 min of the initiation of handling and remained above baseline for up to 45 min. The Tc of CON birds in that trial also increased, but to a smaller degree, within 5 min and remained above baseline for up to 20 min. This study indicates that Tc of hyperthermic birds can be superelevated by simulated manual placement of cloacal temperature probes and that fixed probes connected to an automated data monitoring and collection system is a relatively simple way to avoid this problem.

Role of transoesophageal echocardiography in evaluation of cardiogenic embolism.

OBJECTIVE: To determine the value of transoesophageal echocardiography in the assessment of selected patients at risk of cardiogenic embolism or after it. DESIGN: Prospective comparison of the results of transoesophageal and transthoracic echocardiography. Transoesophageal echocardiography was performed with a 5 MHz single plane phased array transducer. SETTING: University teaching hospital. PATIENTS: 100 patients referred for transoesophageal echocardiography after a cerebral ischaemic event or peripheral arterial embolism (n = 63), before percutaneous balloon dilatation of the mitral valve (n = 23), or before electrical cardioversion of atrial fibrillation (n = 14). RESULTS: Transthoracic echocardiography showed potential sources of embolism in four patients including left ventricular thrombus in two patients (with one false positive), left atrial appendage thrombus (n = 1), and patent foramen ovale (n = 1). Transoesophageal echocardiography showed 59 potential embolic sources in 45 patients including left atrial spontaneous echo contrast (n = 33), left atrial appendage thrombus (n = 13), left ventricular thrombus (n = 5), patent foramen ovale (n = 3), left ventricular spontaneous echo contrast (n = 2), mitral valve prosthesis thrombus (n = 1), mitral valve prolapse (n = 1), and pronounced aortic atheroma (n = 1). Transoesophagal echocardiography showed potential embolic sources in 36/53 (68%) patients with atrial fibrillation compared with 9/47 (19%) patients in sinus rhythm. Percutaneous balloon dilatation of the mitral valve was performed without embolic complications in 18 patients without left atrial thrombi and in three patients with small fixed thrombi in the left atrial appendage. It was cancelled in two patients with large thrombi in the left atrial appendage. Cardioversion was performed without embolic complications in 14 patients without left atrial thrombi. CONCLUSIONS: Transoesophageal echocardiography detects potential sources of embolism better than transthoracic echocardiography in selected patients at risk of cardiogenic embolism or after it.

Idiopathic thrombocytopenic purpura complicating chronic lymphocytic leukemia.

Three patients had severe, symptomatic idiopathic thrombocytopenic purpura (ITP) complicating chronic lymphocytic leukemia (CLL) that was refractory to prednisone treatment but that responded to splenectomy alone or in addition to immunosuppressive-cytotoxic treatment. Evidence of high titer of an antiplatelet factor, most likely an antibody, was corroborated in one case by the 14C-platelet serotonin release assay. This experience suggests that the treatment of ITP complicating CLL is similar to that of ITP not associated with lymphoproliferative disorders.
