Long-standing anterior dislocation of the crystalline lens.

We believe this is the first reported case of an anteriorly dislocated lens that produced no pain or discomfort for at least 4 months, ie, March to July 1972. Even after July, the symptoms were not severe enough for this man to seek medical attention until September 1972. The lens may have been dislocated as early as age 5 since, according to his history, surgery was suggested at age 7 for the right eye. A pupillary block may have developed in March 1972, causing an intraocular pressure rise which eventually led to enucleation in September 1972. The microscopic evidence of fibrovascular adhesions of the lens to the iris and cornea documents the long-standing nature of this condition.

Toxic maculopathy. Part I. A result of quinine usage.

A group of 16 heroin-dependent patients undergoing voluntary detoxification were studied to detect "toxic" quinine amblyopia." No true "amblyopia" was found but 8 of 16 (50%) patients had positive Amsler grid tests. This was called "toxic maculopathy." A control group of US Army reservists showed 1 of 25 (4%) to have positive Amsler chart tests. The difference between the two groups is significant and probably represents a chronic, low dose quinine macular toxicity. The strongly suggested relationship between chronic low doses of quinine and subtle macular changes led us to create a second study to document objective abnormalities. Toxic maculopathy part II: A result of quinine usage as demonstrated by fluorescein angiography and electroretinography.

In this paper, further investigation by fluorescein angiography and electroretinography (ERG) was undertaken to substantiate the possible toxic maculopathy due to quinine usage. Again, positive Amsler chart tests were found in 3 of 14 (21%) of the drug addicts with blurred vision, a common complaint among these patients. The results of 5 fluorescein angiography examinations demonstrated no leakage of dye, especially in the macula. Twenty-four ERG examinations revealed no statistical differences between the test group patients and the control subjects. It was concluded that there is a definite need to carefully examine the eyes of any person who has used quinine for a long time since positive Amsler chart tests were found in 37% of the drug addicts.
