[Long-term follow-up and therapeutic results of the protocol of the National Society of Internal Medicine in centrofacial malignant granuloma. Report of 40 cases].

We have studied 40 cases of mediofacial necrosis with no specific diagnosis on biopsy. After an exhaustive work-up we reached a specific diagnosis in 13 patients (Wegener's granulomatosis in 7, classical malignant lymphoma of the nose in 3, squamous cell carcinoma = "goundou" in 1, syphilis and tuberculosis in 1, aspergillosis in 1). Those patients received the appropriate treatments with good results. The remaining 27 patients, however, had ulcerative lymphomas of the midface (according to the immunofluorescence and molecular biology techniques). Their fate was worse since only 15% remain alive on the long-term, despite intensive treatments with chemotherapy, radiotherapy, interferon, artificial nutrition and antibiotics as needed. We present a new protocol with intensified chemotherapy and growth factor treatment in order to ameliorate the very poor prognosis of these patients.

Genital Chlamydia trachomatis infections in patients with cervical atypia.

Of 177 gynecologic outpatients with cervical dysplasia studied, 29 (16%) were found to excrete Chlamydia trachomatis from their urogenital tract. Sera collected from 93 of these patients were significantly more often positive for and showed higher levels of antichlamydial micro-complement fixation (CF) and immunofluorescence (IF) antibodies than sera obtained from the controls. However, when the sera were tested for anti-herpesvirus type II (HSV-II) and cytomegalovirus (CMV) antibodies, no difference between the cases and the controls were encountered.

[Experimental studies of various model materials. 1. Comparative study on linear dimension behavior].

The linear dimensional behaviour of the model materials Begolith, Dentaloc, Tewestone, Impredur and Keramik-Cement was investigated at various mixing temperature, the thickness of layer ranging from 3 to 5 mm. It seems that the model materials Begolith, Dentaloc and Tewestone will yield good results in practice. Impredur may be recommended on certain conditions. On the contrary, Keramik-Cement is not suited as a model material.

[Pseudomembranous colitis caused by antibiotics].

A case of antibiotic-induced pseudomembranous colitis is presented. Following resection of a carcinoma of the colon, an 81-year old man was treated with clindamycin for 9 days and with epicillin for another 9 days. One week after discontinuation of antibiotics the patient developed progressively severe diarrhea. Death from central pulmonary embolism ensued 10 days after the onset of diarrhea. Autopsy revealed severe pseudomembranous colitis of the entire large intestine. Pseudomembranous colitis is often observed as a complication after the administration of different antibiotics. The Anglo-American literature contains several recent reports of clindamycin-induced pseudomembranous colitis. The etiopathology of this drug-induced disease is still unclear. A possible interpretation is an antibiotic-induced change in the intestinal flora. Recent observations suggest that toxin-producing clostridia are responsible for the pseudomembranous colitis.

Genital chlamydial infections in patients attending a gynaecological outpatient clinic.

Investigation for genital chlamydial infections was carried out on 160 patients of fertile age attending a gynaecological outpatient clinic. Thirteen (9%) of the 144 patients with valid specimens were found to be Chlamydia-positive. Seven (54%) of them had antichlamydial antibodies by the complement-fixation test, and 11 (85%) antichlamydial antibodies by a single-antigen immunofluorescence test compared with 21 (16%) and 70 (53%), respectively, of the 131 Chlamydia-negative patients. Patients with overt genital chlamydial infection often complained of ocular and urethral symptoms. Usually there was clinical evidence of cervical infection as well as of inflammation on cervicovaginal smears. Colposcopical and histological examinations did not show any specific alterations.

Experimental spironucleosis (hexamitiasis) in the nude mouse as a model for immunologic and pharmacologic studies.

Experimental infection of the nude (nu/nu) mouse with the intestinal flagellate, Spironucleus (Hexamita) muris, was used as animal model for immunologic studies and for the testing of protozoa cides. The fecal discharge of cysts was shown to correlate with the immune status of the host. It was dependent on the strain, presence of the thymus (nu/nu and nu/+), and age (immunologic maturity). Four protozoacides (dimetridazole, metronidazole, tinidazole, and chlormethoxyacridylamino-diethylaminopropanol-dihydrochloride) were tested in infected mice. All lowered or suppressed the fecal discharge of cysts during treatment; however, the cysts reappeared in the feces when treatment, lasting 1-3 weeks, was terminated. It was concluded that experimental infection of the nude mouse with S muris was an excellent model for immunologic and pharmacologic studies.

Role of polyadenylic acid in a deoxyribonucleic acid-membrane fraction extracted from pneumococci.

After the addition of radioactive polyadenylic acid to cell suspensions of pneumocci, part of the radioactivity becomes associated with a deoxyribonucleic acid (DNA)-membrane fraction extracted from the cells. A variety of techniques show that a portion of this associated radioactivity may represent oligoadenylates complexed to DNA, probaby as part of a ribonucleic acid (RNA) component. Polyadenylic acid, which had previously been shown to enhance DNA synthesis in cell suspensions (Firshein and Benson, 1968), also enhances the extent of DNA synthesis by the DNA-membrane fraction in vitro under specific conditions of concentration and conformation. The mechanism of action of this enhancement may be related to the ability of oligoadenylates to increase the number of initiation sites for DNA replication by stimulating the production of an RNA primer, thus providing additional 3'-OH groups with which DNA polymerase can react.
