Orbital lymphangioma.

In summary, we have presented a patient with an uncommon disorder, orbital lymphangioma, with an equally uncommon outcome, restoration of normal visual acuity and ocular motility following total surgical excision. High-resolution axial thin-slice CT scanning with paraxial re-formations was used for preoperative assessment and surgical planning in the case of this 5 1/2-year-old boy with probable lymphangioma of the orbit. The tumor became symptomatic following minor local trauma. Complete excision was possible without impairment of visual or extraocular motor function.

Prescribing glasses for myopia.

Myopia is a one-symptom refraction problem: blurring of distance vision. Correction of the myopia should eliminate this symptom, but a prescription for glasses may, in fact, produce other symptoms equally--or more--disturbing. A few of the more common pitfalls in prescribing for the myopic patient are: (1) failure to recognize accommodative stress masquerading as low myopia; (2) confusion as to when, or whether, to prescribe for the unilateral myopic patient; (3) overcorrecting the myopic refractive error and failure to appreciate the symptoms that result from overcorrection; (4) difficulties in gaining acceptance of bifocals by the myopic presbyopic patient; (5) appropriate selection of bifocal segments; and (6) acquired myopic anisometropia caused by sclerosis of the crystalline lens. Although myopia would appear to be the least troublesome of all refractive problems, the maximizing of visual potential with comfort and safety needs refracting skills, experience, and clinical judgement.

Myopia--a treatable "disease"?

The authors cite various information and misinformation regarding proposed treatments (atropinization, contact lenses, orthokeratology, wearing glasses, not wearing glasses) for the progression of myopia. They conclude that there is insufficient evidence to support the more vigorous approaches to treatment of myopia, and provide some useful explantations for patients and their parents who question the ophthalmologist's decision to treat conservatively.

Progressive power lenses.

With the rapidly increasing public awareness of progressive power lenses for the correction of presbyopia, there has been a corresponding proliferation of manufacturers and types of progressive lens designs. This paper discusses the various lens design philosophies, often grouped in "hard" and "soft" categories. In the absence of visible multifocal segment lines, problems arise in attempting to identify the various lens types, in neutralizing progressive lens powers and in dispensing. These problems are discussed from the viewpoint of the refractionist. The advantages and disadvantages of progressives are considered, together with their indications and contraindications.

Lady Luck.

When a patient with no distance complaints is unable to read with comfort, a shift in the cylinder axes should be among the first things considered. A simple technique for measuring the axes is described, interpretation of the measurements is discussed, and various therapeutic approaches are suggested.
