Long-term follow-up of fracture nonunions treated with PEMFs.

One hundred thirty-nine established fracture nonunions were treated using a pulsed electromagnetic field (PEMF) device that also recorded patient usage. Patients who used the device less than an average of three hours a day had a success rate of 35.7% (5/14), while those who used the device in excess of three hours daily had an 80% success rate (108/135). The difference in the success rate was statistically significant at p less than .05. Treatment success was unaffected by long versus short bone, open versus closed fractures, nonunion of nine to 12 months duration compared to one to ten years, age of patient (whether less than or greater than age 60), gender, recalcitrant versus first time treatment, infected versus noninfected nonunions, fracture gaps up to 1cm, or weightbearing versus nonweightbearing. Ninety-seven fractures in 90 patients (90% follow-up) who averaged more than three hours of PEMF treatment daily and were originally classified as healed were reevaluated clinically and radiographically at four years following treatment (range: 3.6-5.4 years; mean: 4.1 years). Eighty-nine (92%) maintained a solid union. The success rate of PEMF treatment for nonunion repair demonstrated no statistically significant change over long-term follow-up.

An acute intervention trial: the research nurse coordinator's role.

A multicenter randomized clinical trial can offer exciting and challenging opportunities to the clinical coordinators and other health professionals who participate in the conduct of a clinical trial at the clinical units. This article will address an acute intervention trial, the Multicenter Investigation of the Limitation of Infarct Size, and demonstrate some of the important responsibilities of the clinical unit research nurse coordinators in the conduct and operation of this trial.

Carcinoembryonic antigen and skin test reactivity in tumor radiotherapy.

Serial carcinoembryonic antigen (CEA) levels were obtained from 122 cancer patients. In a random selection, the levels in 67 of these patients were compared with clinical response to radiotherapy. Skin tests were also performed for histoplasmin, tuberculin and mumps. CEA levels, skin-delayed hypersensitivity reaction (DHR) and clinical tumor response were evaluated and correlated. Clinical response of tumors to radiotherapy was more often seen in patients with positive skin tests, but no correlation was observed between skin test reactivity and CEA response curves.

Carcinoembryonic antigen (CEA) monitoring in the management of radiotherapeutic and chemotherapeutic patients.

We have measured serial carcinoembryonic antigen (CEA) titers in 122 radiotherapy and 52 chemotherapy patients, randomly selected from the patient population of a Radiotherapeutic and a Cancer Chemotherapy Clinic, and correlated with the clinical and follow-up evaluation. Seventy-eight the radiotherapeutic patients had detectable CEA levels which represents the majority of these proven cancer patients. In this group, 57 patients (73 per cent) showed correlation between the curves of CEA response and clinical evaluation diseases activity in the radiotherapy group. Thirty-one of 52, or 60 per cent of patients of the chemotherapy group showed correlation between CEA and clinical response. These data indicate that CEA serum levels parallel, in general, clinical tumor reponse.

Carcinoembryonic antigen (CEA) monitoring in the management of radiotherapeutic patients.

We have measures serial carcinoembryonic antigen (CEA) titers in 92 patients, randomly selected from the patient population of a Radiotherapeutic Clinic, to correlate with clinical and follow-up evaluation. 57 out of 92 patients had positive CEA levels (2.5 ng/ml) or 62 percent of proven cancer patients. In this series, 67 percent (63/92) showed positive correlation between the curves of CEA levels and the clinical evaluation of disease activity. A simple computerized program was designed with the data collection directed to identifying parameters and trends of different groups. Breast CA showed 89 percent correlation; lung carcinoma showed 80 percent large bowel 75 percent, and other organs showed less. This assay is a prototype study of human response to therapy in relation to tumor antigen and host response, as measured by a nonspecific tumor associated antigen.

Expert systems as computer assisted instruction systems for nursing education and training.

The ever-increasing specialization of nursing care may require nursing professionals to provide nursing care outside of their specialty. Nurses will have to familiarize themselves with a new specialization area at short notice. Fortunately, expert-systems technology can prove particularly helpful in achieving this familiarity. As such, this technology can prove a valuable tool for education and training of nursing professionals and students. This article describes the unique edge that expert systems technology provides in this context. Using VP-Expert, an expert system shell, the authors present two examples to illustrate the development of a computer aided instruction system and the unique benefits such a system offers.
