[Results of a multicenter study of Rebif-22 mcg administration in Russia].

The results of multicenter study of Rebif 22 micrograms in Russia have been reported. 167 multiple sclerosis patients have received Rebif 22 micrograms three times a week for 1 year. This study provides evidence for reduction of the relapse rate in patients with relapsing/remitting and secondary progressive multiple sclerosis. The tendency to the decrease of the severity of relapses, less need for steroid use and the decrease of EDSS score in patients with relapsing/remitting multiple sclerosis have been shown. In general Rebif 22 micrograms was tolerated well.

[Betolepsy in patients with multiple lesions of major cerebral blood vessels].

The reported cases of betolepsy, namely convulsive fits and syncopal attacks induced by cough were observed in patients suffering from different pulmonary diseases, vegetovascular dystonia, and venous encephalopathy. The extracerebral mechanism occurring by the Valsalva type related to the lowering of cardiac output and blood supply to the brain was assumed to be implicated in the origin of betolepsy. In the described group of the patients with multiple occlusive lesions of the cerebral arteries, the pathogenesis of betolepsy is obviously connected with a decrease of the arteriovenous gradient of cerebral circulation to the level close to the critical one. In this case, enhancement of the cerebral blood flow attained by different approaches including reconstructive operations on cerebral vessels rather than administration of anticonvulsants is an effective means of counteracting betolepsy.

[The role of computerized tomography in the diagnosis of brain lesions in patients with epileptic attacks].

Computer-aided tomography was used to examine 449 patients suffering from epileptic attacks. In patients with setting in epileptic attacks, computer-aided tomography has been shown to play a preventive role, contributing to early diagnosis of cerebral diseases, primarily tumors. Specification of the character of cerebral disease and morphological substrate of epileptic attacks makes it possible to prescribe adequate therapy. It is recommended that computer-aided tomography be performed in all the patients suffering from epileptic attacks.

[Features of the EEG correlates of activity in epileptic patients].

A correlational analysis of the EEG in epileptic patients and normal subjects made it possible to elucidate differences in the brain activity in the performance of similar tasks: the performance of the correction test, collection of the Link cube, exercise with a hand expander. It is supposed that in disease, the performance of a motor act is achieved through some functional elements of the brain, which are different from normal.

[Tactics for managing patients who developed epileptic seizures and paroxysmal states for the first time].

The authors examined 230 patients who developed paroxysmal conditions. The majority of patients (n-193) presented with epileptic seizures. In the remaining 37 patients various syncopal conditions were diagnosed. The authors emphasize the necessity of a comprehensive examination of such patients, the early detection and treatment of somatic pathology which usually aggravates the course of epileptic process at the early stages. The authors advocate an active multimodality treatment of patients who developed seizures for the first time, the therapeutic and diagnostic use of pneumoencephalography with the administration of nitrous oxide into the cerebrospinal fluid. Approximate schemes of the treatment of patients are offered and a long-term follow-up on the outpatient basis is recommended.

[Relation between the diffuse and local in the clinico-electrophysiologic evaluation of patients with generalized "peak--slow wave" discharges].

Despite the long history of research into epilepsy, the mechanisms of the development of a paroxysm of generalized "peak-slow wave" discharges with a frequency of 3 fluctuations per sec are still unclear. An expanded clinical-electrophysiological examination of 36 patients showed that generalized disorders in the central nervous system were also accompanied by local changes. Important data were obtained during biotelemetric recording of the superficial EEG and stereoelectroencephalogram in patients in the process of carrying out various activities and in the period of paroxysms. It is concluded that there are different sites of paroxysm generalization due to the domination of either cortical or subcortical pathological foci.

[Motor phenomena during electric stimulation of the brains of epileptics].

Ninety-eight patients with long-term electrodes implanted into different structures of the brain were subjected to clinical and electrophysiological examinations. Motor phenomena activated by electrical stimulation of the brain were studied by radiotelemetry and stimulation myography. The threshold and amplitude of H-reflex was shown to be decreased during the interictal period. In cases of the development of a non-convulsive attack, there was a bilateral elevation in the value of the monosynaptic potential. This indicator reached the peak value before the appearance of bilateral tonicoclonic convulsions. The authors note that the clinical characteristics of the activated epileptic phenomena depend not only on the site of electrical stimulation but also on the direction of the after-action discharge propagation.

[Radiotelemetric EEG and EKG studies of epilepsy patients].

In 17 epileptic patients the EEGs and ECGs were recorded in clinical conditions using a 7-channel bioradiotelemetric system designed at the Sverdlovsk Institute of National Economy. The recording was performed under various physical, mental, and emotional loads, as well as during the seizures. This method of examination made it possible to specify in a number of cases the localization of the pathological focus, to differentiate the forms of unconscious motions, to disclose a number of the factors provoking or inhibiting the pathological activity of the brain.

[The dynamics of temporal lobe epilepsy manifestations and the social-occupational prognosis in children following surgical removal of the epileptic focus].

The authors conducted a follow-up study of 28 children with temporal epilepsy who had undergone an anterior temporal lobectomy performed at the age of 4-13 years. Besides, a discontinuation of convulsive seizures in almost all of the cases there was an improvement in the social adjustment: 25 of 28 children could go to school for the first time or renew their studies, whereas before the operation 7 children could not study, because of frequent seizures and behavioral disturbances. Following 2-10 years after the surgical operation II of the patients were still studying at school or in vocational schools and 12 of them were working. Not being able to study was I child of school age, and 4 patients older than 18 years were incapacitated. Surgical treatment of epilepsy in childhood was more effective than in adults.

[Pathomorphological characteristics of cerebral arterioles and capillaries in surgical epileptic patients with generalized seizures].

Clinical and morphological analyses of brain biopsy specimens from epileptic patients undergone surgery have indicated that the brain vascular system in epilepsy has the most vulnerable site, namely: pre-capillary arterioles and capillaries. At the same time, the paper describes some dissimilarities of qualitative and quantitative characteristics of the above vessels from their changes in hypertensive angiopathy. There was a relationship of arteriolar changes to the duration and severity of epilepsy. The use of silver-impregnated serial brain sections revealed 5 types of cerebral capillary morphological changes that could be arranged in successive stages of transformation (neocapillarogenesis). There was an association of the activation of neocappilarogenesis with the duration and severity of epilepsy. It forms an impression that the life of a capillary is shorter due to its accelerated morphofunctional evolution particularly in patients with advanced epilepsy. This is accompanied by the rates of neocapillarogenesis that is regarded as a compensatory and adaptive response of the brain capillary system.
