[Surgical treatment of bleeding gastroduodenal ulcers].

The active-temporizing tactics accepted in the clinic, which is based on determination of the indications for operative treatment according to the endoscopic classification, abandonment of hemostatic therapy for elderly and senile individuals, active antiulcerous therapy, and consideration for immunological reactivity made it possible to reduce total mortality among patients with ulcerous gastroduodenal bleeding from 12.6% to 4.5%.

[Hemostasis and humoral immunity in patients with peptic ulcer hemorrhage].

In ulcerative gastroduodenal bleeding, the high blood coagulability is preserved, and prescription of the hemostatic preparations of general action can lead in elderly and senile patients to the thromboembolic complications. The operative treatment should be performed, depending on immunoreactivity of an organism: at the peak of bleedings--at the first 24 h, in stopped bleeding--at the period of immunologic compensation.

[Treatment of acute cholecystitis in patients over 70 years of age].

Under analysis are results of treatment of 176 patients aged from 70 to 92. The clinical picture of acute cholecystitis in such patients is fairly distinct with a typically rapid and active inflammatory process. The individual choice of terms of the operative treatment of senile patients with acute cholecystitis allowed the postoperative lethality to be almost 2 times reduced.

[Emergency fibrogastroduodenoscopy in the diagnosis and treatment of peptic gastroduodenal hemorrhages in the middle-aged and elderly].

Urgent fibrogastroduodenoscopy was performed in 145 of 178 elderly and senile patients with bleedings from upper portions of the digestive tract, ulcerous bleedings being revealed in 92 of them. Surgery is thought to be the best method in cases with persistent bleedings and in the revealed during examination thrombosed vessels in the ulcer without active bleeding. In 33 patients not subjected to endoscopy the latter could have given better results of treatment.
