The prevention of diabetic retinopathy.

Strict regulation of blood sugar reduces the severity of vascular complications including the retinopathy suffered by diabetic patients. The artificial pancreas, insulin pump, and careful insulin administration have been compared with the finding that intensified conventional control is efficacious. Multiple injections of regular and long-acting insulin coordinate physiologically with the serum glucose, minimize hyperglycemia and lessen diabetic retinopathy. This paper will review the facts and support the contention that tight control is worthwhile in patients with long life expectancy. The role of the ophthalmologist should be reinforcement of the principles of good control.

Mitochondrial abnormalities in progressive external ophthalmoplegia.

Three cases of chronic progressive external ophthalmoplegia demonstrated ragged-red fibers. Histochemical stains showed that the ragged-red fiber appearance was produced by abnormal mitochondria in the muscle. Electron microscopic studies demonstrated the ultrastructural origin of the intramitochondrial inclusions seen in ragged-red fibers. The associated abnormalities in lactic acid metabolism possibly represent an underlying generalized defect in metabolism. This evidence suggests that chronic progressive external ophthalmoplegia is a diffuse disease of the mitochondria.

Postpartum uveitis.

A review of the literature supports the concept that serum levels of corticosteroids are elevated during pregnancy. Even a concurrent increase in cortisol-binding globulin (CBG) does not entirely neutralize the increased levels of cortisol. Thus, body tissues are exposed to more free cortisol than in the nonpregnant state. After delivery there is a rapid reduction of serum corticosteroids to the nonpregnant level. A granulomatous disease such as sarcoidosis may be suppressed by the high serum cortisol level. In the puerperium with a reduction of the serum cortisol, exacerbation of such a disease is likely. The case presented demonstrates this phenomenon.

Hypertension update.

Hypertension affects approximately fifty million Americans. About 80% of hypertensive patients are aware that their blood pressure is elevated. While more than 50% are on medication, only about 20% of all hypertensive adults are controlled at normotensive levels. Ophthalmologists should be aware of the seriousness of hypertension because it affects many of their patients and is a risk factor for myocardial infarction, stroke, congestive heart failure, end-stage renal disease and peripheral vascular disease. As medically trained eye specialists, ophthalmologists should be knowledgeable about and take an interest in their patients' medical problems, thus playing an integral role on the health care team. As a primary health care provider, ophthalmologists should perform in-office blood pressure monitoring.
