A population-based Danish data resource with possible high validity in pharmacoepidemiological research.

The incomplete understanding of drug effects at the time of marketing has led to ongoing epidemiological monitoring. The Danish National Health Service covers part of the expenses for most kinds of prescription drugs. The accounting system within the pharmacies is increasingly computerized. In 1992 we found that 96% of the pharmacies were computerized. The computerized accounting systems give no information about adverse drug effects, but this can be obtained from the Danish Hospital Register by cross-checking registries, and in some cases from the register of birth and the register of congenital malformations. There is a great need for new data sources in pharmacoepidemiology. In spite of some evident methodological problems, the Danish data sources probably have high validity compared with international sources, since they are population based.

Psychiatric care of refugees exposed to organized violence. A comparative study of refugees and immigrants in Frederiksborg County, Denmark.

Psychiatry in Scandinavia has a tradition for research and clinical work with people from other countries, especially related to refugees and survivors after the Second World War. In recent years an increasing number of refugees have arrived, especially from the countries of the Middle East. The aim of this study was to evaluate the refugee situation in a Danish county, on the basis of data on 49 refugees treated in the county's psychiatric departments from January 1, 1986 to December 31, 1987 and 44 immigrants seen from January 1, 1983 to December 31, 1987. The clinical data illustrated marked differences between immigrants and refugees with respect to social situation and background, symptom pattern and exposure to organized violence. Both groups terminated treatment after very few contacts, and the psychiatric staff seems to need better training. The general need for psychiatric and psychological treatment of immigrants and refugees is still uncertain and further research is required.

The active medicine user.

The purpose of the study is to analyse medicine behaviour seen from the user's point of view. The study intends to generate ideas to specify topics of problems and to try out a combination of qualitative research methods. The practitioners and four asthmatic patients attached to a health centre in Billund, Denmark, were interviewed. The patients kept a diary based on topics, the notes of which were elaborated by weekly telephone interviews. In a final interview all participants evaluated the course of the study. The combination of qualitative methods has been very suitable to provide the perspective of the user. The results of the study question the ideal picture of the patient as a passive user of medicine. The main trends show that: the users develop different strategies to evaluate medication therapy; it has negative consequences to the patient when medication is changed regardless of patient experience; the therapy improves when the doctor draws on the experience of the patient. The study contributes to the present, sparse knowledge about the consciously acting user of medicine and indicates the importance of incorporating the user's experience and life situation in the health care system's handling of health problems.
