[Primary non-small-cell lung cancer: analysis of 419 T1 (<or=3 cm) tumors in the KBP-2000-CPHG study].

T1 tumors have the best prognosis among primary non-small-cell lung cancers, basically because surgery is generally possible. Among 5.667 patients with primary lung cancer included in the KBP-2000-CPHG study, we examined the characteristics of 419 T1 tumors, i.e. 9.2% of the non-small-cell cancers. Compared with the group of patients with non-T1 tumors, patients with T1 tumors were younger (p=0.0007). They had an equivalent percentage of squamous-cell tumors but more adenocarcinomas (40.3% versus 35.5%, p=0.05). TNM staging showed that 27.6% of the T1 tumors were metastatic at diagnosis (stage IV) with 12.4% T1N0M1 nad 15.2% T1N1-3M1. For the M0 tumors, 52.2% were T1N0 (stage IA) and 20.1% were T1N1-3. Squamous-cell tumors were significantly more frequent among the T1M1 tumors (p=0.07). More than one quarter (27.6%) of the T1 tumors were in stage IV, pointing out the importance of the initial work-up. This findings suggests we should revisit strategies in order to take into account new diagnostic possibilities. Likewise for the therapeutic strategy. Combinations using chemotherapy, surgery and radiotherapy should be better defined for this group of tumors.

A left pleural effusion with a calcified tumoral mass and left hemithoracic uptake on bone scan.

Diffuse malignant pleural mesothelioma is a rare primary tumor of the pleura with three principal histological types, epithelial; mesenchymal and mixed epithelial and mesenchymal. We report here a case of a mesenchymal mesothelioma with foci of osteosarcomatous degeneration revealed by dense calcifications associated with the pleural effusion on the computed tomography (CT) of the thorax. The bone scan revealed extraosseous uptake corresponding to the left pleura.

Iliac bone defects revealing systemic sarcoidosis.

Bone lesions are fairly uncommon in sarcoidosis (5 to 10% of cases). We report the case of a 40-year-old man in whom sarcoidosis of the lungs and bones was revealed by excruciating buttock and sacral pain. Computed tomography showed multiple punched-out defects in the left iliac bone. No similar cases have been reported in the literature.

[Immunohistochemistry in the differential diagnosis of mesothelioma and adenocarcinoma. Evaluation of 5 new antibodies and 6 traditional antibodies].

The aim of this study was to evaluate three recently marketed putative mesothelioma-binding antibodies, calretinin, HBME-1 and thrombomodulin, and two putative adenocarcinoma-binding antibodies, AUA1 and MOC31, on paraffin sections from 28 mesotheliomas and 30 adenocarcinomas. Moreover, the expression of ACE, BerEP4, CA125, CA19.9, LeuM1 and vimentin was assessed. Calretinin, HBME-1 and thrombomodulin, which showed a 100%, 89% and 43% sensitivity, and a 50%, 70% and 87% specificity for mesothelioma respectively, were less efficient than vimentin (100% specificity and 67% sensitivity) for the positive identification of mesothelioma. AUA1, BerEP4 and MOC31 were 100% sensitive to adenocarcinoma, with BerEP4 and MOC31 having the highest specificity (86% each). The immunophenotype "vimentin-positive, ACE-negative, CA19.9-negative" yielded 100% sensitivity and 97% specificity for diagnosis of mesothelioma. We advocate the use of the four-marker panel of ACE, CA19.9, MOC31 (or BerEP4) and vimentin for differentiating mesothelioma from adenocarcinoma.

[Uninodular bronchiolo-alveolar cancer. Apropos of 5 cases with a prolonged course].

Five cases of uninodular bronchioloalveolar cell (BAC) carcinoma are reported. These 5 cases were characterized by the protracted course of the disease: 14.5 and 11.5 years respectively in the 2 patients who died; 7.5, 12 and 12.5 years respectively in the 3 patients who are still alive. Prior to being diagnosed at excision, the tumours had been followed up for an unusual 2 to 10 years period, with phases of radiological stability and phases of very slow growth. This very slow course is exceptional in lung cancer. It differs not only from the course of diffuse BAC carcinoma, but also from the habitual course of uninodular BAC carcinoma and from that of peripheral adenocarcinoma. Similar cases could be found in the literature, and a comparative study has been conducted between these slow-course carcinomas and normal-course carcinomas. Whatever their outcome, slow-course carcinomas are asymptomatic tumours which present at radiography of the chest as a peripheral round or oval opacity devoid of pathognomonic features. The post-operative prognosis of slow-course tumours seems to be more favourable than that of the other lung cancers. At histology, carcinomas with a protracted course usually show a pattern similar to that of lesions called by some authors "benign adenomatosis"; however, some of these tumours exhibit fairly distinct signs of malignancy, and electron microscopic images also are far from being uniform.

[Preliminary results concerning the value of chemotherapy in the treatment of inoperable bronchial carcinoma (excluding small-cell anaplastic carcinoma)].

Inoperable non- microcellular primary bronchial carcinomas have been reputed up to now to be chemo-resistant. The introduction of Cis-platinum into a polychemotherapy protocol leads to revision of this concept. The authors report the preliminary results of a polychemotherapy protocol (including Cis-platinum, Vindesine, CCNU, Cyclophosphamide) associated, in cases of non- metastasized carcinomas, with radiotherapy to the tumour itself, the mediastinum and the supraclavicular fossae. These results confirmed the value of such chemotherapy in forms with metastases. In localised inoperable forms, conclusions could be reached only on the basis of a randomised comparative trial of chemotherapy + radiotherapy versus radiotherapy alone.

["Thorakeurynter," a simplification of the postoperative period following pneumonectomy].

Based on the technique described by Gabler et al. several years ago, stabilization of the mediastinum after penumonectomy is obtained by introducing a blown up latex balloon into the operated side of the thorax. The balloon is introduced immediately following surgery after being distended by means of Kuss's apparatus, and is removed, after being emptied in the same way, five to seven days later. Good stabilization of the mediastinum during the immediate postoperative period allows rapid effective coughing to take place. The balloon also prevents postoperative emphysema through the mediastinal and parietal walls, especially in enlarged excisions. The method is of undoubted value in the postoperative period following pneumonectomy, particularly in chronic bronchitis cases. No particular complications were observed.

[Estimation of cardio-respiratory energy reserves by ergometric exploration].

Beyond the usual respiratory functional tests, ergometry represents a complementary test, yielding much information on the functioning of the lung-heart system. Simultaneous recording, studying and comparing different respiratory and haemodynamic functional parameters during effort, provides an objective appreciation of the cardio-respiratory energetic reserve. Such a study is necessary to estimate a functional handicap and to understand the pathophysiological alterations of the cardio-respiratory system. The estimation of energetic reserve based on the simultaneous study of multiple respiratory and circulatory parameters also provides valuable information. Ergometry is of equal value in the estimation of operating risks in pulmonary surgery. Information drawn from more than 600 ergometric tests composed of simultaneous recording of 8 parameters (heart rate, ECG, pulse volume, spirogram, ventilation per minute, SAO2, capnogram, right ventricular pressure) have confirmed the great value of this method in pulmonary medicine.
