[The use of air-plasma flows in military field surgery and disaster medicine].

Plasma currents of high energy are considered to be very promising in the surgical treatment of modern pathology. The scientists of this country have constructed a portable field surgery apparatus in which a heated atmospheric air assumes a form of a high-temperature thin jet used like a surgeon's scalpel. Experiments and clinical tests proved the efficacy of this microplasmotron that can destroy and coagulate body tissue when treating it with plasma. This new method has, besides hemostatic action, some noticeable antimicrobic effect that leads to acceleration of tissue granulation and wound healing. In specialized and skilled medicare this progressive and priority method should be applied for initial surgical wound treatment, secondary hemorrhage arrests and surgical interventions when a high risk of infection spreading is involved.

[The diagnostic and treatment characteristics of liver injuries in combined trauma].

The results of diagnostics and treatment of the closed liver's injuries in 180 injured with combined trauma are analyzed. Was established, that the complexity of the diagnostics of the liver trauma and the heavy accompanying injuries, accomplishment of the diagnostic researches on the shock background, consciousness frustration, hemodynamics, breath, hypovolemia requires intensification of ultrasonic researches, computer tomography and laparoscopy, which reliability has made 91-95%. The injured with combined trauma and liver's injuries should be rendered the urgent specialized help. The volume of the operation on liver should be the most minimum, but reliable to stop hemorrhage. The surgical correction of other injuries of the abdominal and peritoneal cavities should be carried out. The operation is finished by cranial decompression trepanation and stabilization bone breaks of pelvis and extremities (under indications) as a preventive measure of shock, loss of blood and embolic complications.

[The efficacy of using ozone preparations in the combined treatment of paranasal sinusitis].

In controllable study efficiency of application of ozone preparations in 39 patients with acute and chronic purulent maxillary polysinusitis was studied. Purulent maxillary sinusitis, which in 44% was combined with frontitis and in 56%--with fronto-ethmoiditis was diagnosed in all patients. Besides traditional therapy all patients were washed out daily inflamed sinuses with 0.9% solution of sodium chloride, saturated of ozone. After washing out the sinuses were aerated during 15-20 minutes through paracentesis needles and catheters with ozone-oxygen mixture, containing 6 mg/l of ozone. Speed of mixture flow--1 1/min. To the half of patient with phenomena of intoxication were introduced I/V 400 ml of physiologic salt solution (ozone concentration--0.8 mg/l). Treatment rate is 3-6 infusions. In patients pyorrhea and allocation of bacteria stopped 2-3 days earlier, than in control group, level of molecules of average weight was faster reduced to norm in the plasma of blood. Reduction of concentration of molecules of average weight corresponded to essential improvement of patient clinical condition (P < 0.01). Experience of ozone application testified about its high efficiency in paranasal sinusitis treatment.

[Possibilities of using plasma flow in lung surgery].

Experiments on 10 mongrel dogs demonstrated the efficacy and reliability of argon plasma flux in achieving hemo- and aerostasis in surgical debridement of gunshot wounds of the chest "Fakel" plasma surgical complexes were used as a source of plasma. A total of 43 operations were performed on patients whose ages ranged from 16 to 68 years: pulmonectomy 3, lobectomy 24, bilobectomy 4, atypical resection of the lung 4, pleurectomy with lung decortication 5, diagnostic thoracotomy 2, rib resection 1. Argon plasma flux was used in thoracotomy, treatment of wound surfaces of the lungs and pleura, separation of the interlobar fissures, and division of the main and lobar bronchi. The plasma flux produced an analgesic effect; as a result much less narcotics and analgesics were needed and physical activity was permitted earlier. The amount of discharge from the pleural cavity along the drain was 1.5-2 times less (200 +/- 40 ml) as compared to that in the traditional methods for conducting similar operations. No complications associated with application of plasma flux were noted. Incompetence of the bronchial stump, rethoracotomy for bleeding or suppuration were not encountered.

[Experimental studies after resection of the bronchi with a plasma scalpel].

Experimental studies on 18 mongrel dogs showed that resection of large bronchi can be accomplished effectively with a plasma scalpel even in the presence of purulent bronchitis. Reliable hemo- and aerostasis are provided, a marked bactericidal effect is produced, the zone of irreparable thermal changes is relatively small, and proliferative processes prevail in healing. The obtained information allow plasma scalpel to be recommended for resection of large bronchi in clinical practice.

[Coagulation of experimental gunshot wounds of the liver and spleen using plasma argon flow].

Experiments were conducted on 26 dogs to study the possibility of debridement of gunshot wounds of the liver and spleen with plasma argon flow. The method was found to be highly effective in providing final hemo- and cholestasis. The elaborated ++plasma coagulation method allows organ-preserving operations to be undertaken and may be recommended for use in field surgery. Analysis of morphological changes in the tissues showed that the wounds of the liver and spleen heal in the usual period of time without coarse cicatricial deformity of the tissues.
