[Aspiration syndrome in cesarean section. Our experience from 1980 to 1990].

In the last twenty years maternal mortality attributed to anaesthesia has decreased. Inhalation of gastric contents is the commonest cause in patients undergoing cesarean section; in fact pregnant women are considered "high risk" because of gravidic modifications. In this retrospective study of 10017 caesarean sections performed under general anaesthesia in our institution between January 1980 and December 1990, we evaluated the frequency of this syndrome (7 cases = 1:1431). We had no case of maternal and neonatal mortality. All these seven patients were admitted at our recovery room for less than 5 days; aspiration pneumonitis occurred in only three patients. Our results suggested that induction of anaesthesia with high doses of thiopental reduces complications related to light anaesthesia, including vomiting. At a dose of 5-6 mg/kg thiopental didn't produce any significant neonatal depression as documented by Apgar scores.

Hypervolemic pulmonary edema and severe coagulopathy after intrauterine dextran instillation.

A healthy young woman with recurrent spontaneous abortion and septate uterus underwent hysteroscopic metroplasty with intracavitary instillation of 1350 mL of 10% dextran 40 in normal saline. After the operation, the patient developed acute hypervolemic pulmonary edema and severe coagulopathy due to temporary impairment of platelet function. Fluid overload, which could have been caused by the hyperosmolar properties of dextran, worsened progressively as fluids were drawn from the interstitial space and urine output was reduced. The use of a dilute and/or low-molecular-weight dextran solution to distend the uterine cavity during operative hysteroscopy is not without risk of complications.

Recent acquisitions in pain therapy: meclofenamic acid.

A better utilization of nonsteroidal anti-inflammatory drugs (NSAIDs) is possible today based on recent pharmacodynamic and pharmacokinetic studies. The analgesic action of these drugs may take place in the central nervous system (CNS). The analgesic action with a lower dose occurs earlier than the anti-inflammatory action. Some NSAIDs cause an increased level of plasmatic bendorphines in humans. NSAIDs not only have antiprostaglandin action, but also may block the release of substance P. The NSAIDs may be useful for headache, dysmenorrhea, rheumatic disease and in cancer pain therapy. For the safe use of NSAIDs the previous anamnestic and clinical features of the patient must be considered, and a high therapeutic level must be satisfied. Considering this goal, the authors examine pharmacologic and clinical behavior of meclofenamic acid.

Clomipramine compared with pentazocine as a unique treatment in postoperative pain.

Forty patients who underwent laparotomy for hysterectomy were administered either clomipramine or pentazocine for the treatment of postoperative pain. Both drugs were similarly effective during the eight hours of observation. The results of this study indicate that clomipramine, often used in chronic pain as an adjuvant drug, exerts as well as analgesic effect in acute pain.

Nefopam in postoperative pain.

A comparative study between nefopam (Acupan) and pentazocine was carried out in 90 patients for treatment of postoperative pain following gynaecological operations. The results show that nefopam has an analgesic activity comparable with that of pentazocine, but its duration of action seems to be longer-lasting, even if with a longer period of latency. At equieffective analgesic action, nefopam shows a lower interference with the respiratory function. As far as side-effects are concerned a significant increase in drowsiness was observed with both types of treatment; sweating was observed only in nefopam group.

[General anesthesia with continuous-infusion ketamine].

The importance of Ketamine in continuous perfusion is described on the basis of a studied carried out in 30 patients. The clinical data are compared with results predicted by the mathematical model. Special study was made of the behaviour of the cardiocirculatory apparatus, particularly in reference to myocardial contractility and electroencephalographic activity. It is concluded that the drug can be advantageously used in continuous perfusion.

[Incidence and complications of the aspiration of gastric contents syndrome during cesarean section in general anesthesia].

Maternal mortality attributed to anaesthesia, decreased in the last twenty years, has in inhalation of gastric contents one of its principal causes. Pregnant women are considered "high risk". In this retrospective investigation we studied 12,380 patients undergoing caesarean section in general anaesthesia between January 1977 and December 1991. During this period of obstetric activity the frequency of aspiration syndrome is 1:1,547 general anaesthesias (0.064%), being the 61.5% of all anaesthetic complications. In the 8 patients we had no maternal and neonatal mortality. Aspiration pneumonitis occurred in three patients. All patients were admitted at our recovery room for less than 5 days. The emergency procedure (8 cases) and a difficult tracheal intubation (4 cases) are the more important risk factors. In our clinical practice we used prevention in few cases (3.1%). Our results suggest the utility in obstetric anaesthesia of high doses of thiopental (5-6 mg/kg); these doses didn't change the principal neonatal parameters, while give in pregnant woman a light anaesthesia.
