[Effects of family-centered group therapy with demented patients and their caregivers].

Alzheimer's disease, the most frequent cause of dementia, progressively takes the ability of acting autonomously from the sufferers, makes them more and more dependent from their partners and eventually dissolves their personal identity. The present 2-year study which conducted a novel form of family-centered group therapy with demented patients and their primary caregivers investigated the effects of an open, uncontrolled group therapy approach in 32 families (demented patients and caregivers) over a five-months period with weekly sessions, by using the BEHAVE-AD and the NOSGER for the moderately demented patients and the Befindlichkeits-Skala (BfS) for the primary caregivers. In addition, a structured instrument to determine the patterns of interaction between patient and caregiver was administered prospectively at each of the weekly sessions. The results demonstrate a significant improvement of communication abilities among the patients and a significant reduction of coping problems and of care burden with respect to the caregivers. The present family-centered group therapy shows that over a period of 5 months functional and communication abilities of the moderately demented patients may be maintained and caregiver burden may be significantly reduced.

Mercury and selenium distribution in human kidney cortex.

Concentration of mercury and selenium were analyzed in tissue fractions of human kidney cortex samples from seven autopsy cases. Total mercury content ranged between 0.3-9.0 nmol Hg/g wet wt. Between 27-61% of the total mercury was found in the 105,000g supernatant of the tissue homogenate from six cases. In kidney cortex from the seventh case, a decreased dentist with the highest concentration of mercury, only 3% of the total mercury was found in the 105,000g supernatant and about 88% in a SDS-insoluble fraction. In this fraction the molar ratio between mercury and selenium was close to 1:1. This study supports results from previous animal studies and indicates that mercury in human kidney cortex could be deposited in forms with different solubility. It could be of importance to speciate different forms of mercury in tissues according to solubility and association to selenium when interpretations of mercury concentrations are made.

Is cadmium released from metallothionein in rejected human kidneys?

Concentrations of metallothionein and metals, i.e. cadmium, copper and zinc, were determined in six rejected transplanted human kidneys and one kidney prepared for transplantation. Tissue samples separated by gel chromatography showed that almost all of cadmium in tissue was in the form of firmly bound cadmium-metallothionein.

Cadmium, zinc, and copper in rabbit kidney metallothionein--relation to kidney toxicity.

Twenty-two rabbits were given repeated subcutaneous injection of cadmium chloride. The cumulative cadmium dose given ranged from 13 to 214 mumole/kg body weight. Five rabbits served as controls. The treatment resulted in cadmium concentrations in kidney cortex that ranged from 0.3 to 3.2 mmole Cd/kg and a subsequent production of metallothionein. The molar ratio of cadmium, zinc, and copper in metallothionein fractions from kidneys with different concentrations of cadmium was determined. At low concentrations of cadmium in rabbit kidneys, zinc was the dominating metal bound to metallothionein (70-90%). At high concentrations of cadmium in kidneys, cadmium was the dominating metal in metallothionein. Evidence of kidney toxicity, in the form of beta2-microglobulinuria, was seen when cadmium constituted 85% of the metal ions recovered from metallothionein fractions. The remaining 15% was zinc. This indicates that at most six of the seven metal-binding sites in mammalian metallothionein are occupied by cadmium and that the remaining site is occupied by zinc. Our data provide further support for the hypothesis that chronic cadmium nephrotoxicity develops when there is a lack of metal-binding sites available for cadmium in metallothionein.

Augmentation mammaplasty: the surgical and psychological effects of the operation and prediction of the result.

The aim of this research was to evaluate the effect of augmentation mammaplasty on women and to determine if it is possible to predict which women will benefit most from the operation. Interviews were conducted with both women who were about to undergo augmentation mammaplasty and those who had undergone the surgery. These interviews were compared with interviews from a control group composed of a normally distributed population. The women who apply for augmentation mammaplasty are a homogeneous group who experience their breast problems in a very similar way. Our study revealed that they are distinctly different from the control group in personality and childhood environment. Most of the women were generally satisfied with the operation even though many had complaints about the technical result. Certain behaviors had changed, but despite these positive results these changes were not radical and in reality the improvement was not as the women initially described it. The women's behavior reactions rarely changed enough to become similar to the reactions of the control group. Using a personality test, it is possible to predict which women will benefit most from the operation. When an assessment of psychiatric problems was made using the neurosis scale in a personality test, the women who scored low on a scale of neurotic self-assertion were satisfied with the operation to a greater extent.

Metallothionein in rabbit kidneys preserved for transplantation.

Thirteen rabbits were given repeated cadmium injections to achieve cadmium concentrations in kidney cortex ranging from 0.05 to 1 mmole Cd/kg wet weight. Another four animals served as controls. One kidney from each animal was frozen directly to -70 degrees C whereas the other kidney was kept for 24 hr at +4 degrees C in a preservative (Sachs' solution) to simulate conditions for preservation of human donor kidneys before transplantation. Protein binding of cadmium, zinc and copper in kidney homogenates and the concentration of metallothionein (MT) were measured in the kidney that was frozen directly and in the kidney that had been preserved. No gross differences in either the protein binding of cadmium, zinc and copper or in the MT content were seen between the directly frozen and preserved kidneys from the same animal. This indicates that MT is not rapidly broken down in rabbit kidneys which have been preserved similarly to human donor kidneys for 24 hr in a standard preservative solution prior to a transplantation.

[Semi-continuous peritoneal dialysis - an advantageous modification of the peritoneal dialysis technic].

Using a single-needle-monitor known from the haemodialysis in the peritoneal dialysis vastly automatized short cycles may be achieved which make possible an increase of effectiveness and thus a shortening of the time of dialysis. Thus patient and personnel are relieved. In the practical use the technique proved to be simple, no known complications conditioned by technique appeared.

A psychological study of patients seeking augmentation mammaplasty.

Data from interviews with 64 women who were to undergo or already had undergone an augmentation mammaplasty were compared with data from a control group of 28 women. Great differences existed between the groups, mainly with regard to personality test data and childhood milieu. The women operated on were in general less certain of themselves and felt less feminine. These factors seem to be a sufficient basis for the appearance of the problem. Cultural norms are obviously also important but seem to influence all women in the same way.
