Malignant melanoma originating from intradermal nevi. A clinicopathologic entity.

Two cases of malignant melanoma arising in preexisting intradermal nevi without a junctional element are described. These cases and the two known to have been reported previously demonstrate similar clinical and morphologic findings. These include an innocuous gross appearance, a nodular growth pattern, and deep levels of invasion. It appears that the origin of the tumor in the dermis conceals malignant transformation. The consequent time lapse and the dermal position of the lesion favor deep penetration of the tumor prior to removal.

Retroperitoneal liposarcoma metastatic to scalp.

Metastasis of liposarcoma to the scalp occurred in a 50-year-old-man. Review of the literature disclosed no documented reports. Although cutaneous metastases from internal malignancies are of relative infrequent occurrence, such metastases may be limited to the scalp and a diagnosis can be established by a biopsy.

Histiocytosis X of the skin in an elderly man.

A diffuse truncal cutaneous eruption of six months' duration in a 76-year-old man is described. Light and electron microscopy revealed infiltration of the papillary dermis and overlying epidermis by histiocytic cells containing Langerhans granules, findings characteristic of histiocytosis X. The Langerhans cell as a probable cell of origin and unifying feature for the subtypes of histiocytosis X is discussed. It is suggested that with increased awareness of the variability of histiocytosis X, atypical presentations will be identified more frequently.

Apocrine hidrocystoma of the shoulder.

An apocrine hidrocystoma of the shoulder of a 59-year-old woman is, to our knowledge, only the third reported case that arose outside of the head and genitalia. Its origin was nonetheless consistent with the distribution of apocrine glands in adults.

Malignant melanoma of the gallbladder: report of three cases and review of the literature.

Three cases of primary melanoma of the gallbladder are described to add to the five reported in the literature. As an organ of endodermal origin, the gallbladder appears to share the melanoblastic potentiality possessed by similarly derived organs, and hence, the development of a melanoma within the gallbladder is not unexpected. Presence of melanocytes within the gallbladder mucosa is absolute proof that the tumor is primary. Whether these cells originate from the neural crest and migrate to the gallbladder, or result from local differentiation from preexisting cells remains to be answered.

Metaphyseal giant-cell tumor in a girl of 14.

A case of giant-cell tumor in the radial metaphysis of a 14-year-old girl is reported. 3.5% of these tumors are detected in patients younger than 15 years of age, when closure of the cartilaginous growth plate occurs. A significant number of the 14% of giant-cell tumors detected in patients younger than 20 may originate in a metaphysis during adolescence and become symptomatic only after long bone epiphyseal involvement.
