Menstrual cycle and sex hormone profile in perimenopausal women after liver transplantation.

UNLABELLED: Excellent long-term outcomes of transplant patients let many female liver-recipients experience perimenopausal problems. This study assessed menstrual patterns and sex hormone profiles in women of perimenopausal age who experienced end-stage liver failure treated by transplantation (OLT). MATERIALS AND METHODS: Menstrual patterns, sex hormone profiles, and biochemical parameters of liver function were analyzed before and after OLT in 13 liver-transplanted patients of perimenopausal age. Nineteen healthy perimenopausal women served as controls. RESULTS: The most common abnormality of the menstrual cycle observed in the study group was secondary amenorrhea, which affected six liver-transplanted women. Three months after OLT amenorrhea was still observed in six patients, regular menstrual cycles in six and irregular bleeding in one graft recipient. One year after transplantation regular menstruations were noted in four, irregular bleeding in four, and secondary amenorrhea in five liver-transplanted women. Similar levels of follicle stimulating hormone, luteinizing hormone, prolactin, progesterone and testosterone as well as lower levels of estradiol and DHEA-sulfate were observed in patients with liver failure, both before and after grafting, compared with healthy women. After OLT E2 levels increased from 32.05 +/- 18.04 to 49.12 +/- 22.21. CONCLUSIONS: One year after OLT disturbances in menstrual patterns affect most (69%) perimenopausal female liver recipients. Both before and after OLT significantly lower levels of estradiol and DHEA-S were observed in transplanted patients compared with healthy controls. Hormonal therapy of amenorrhea or irregular menstruations may be required in that group of patients.

Interaction between ethanol and diazepam in mice: chronobiological aspects.

The mechanism of action of benzodiazepines and ethanol demonstrates that these agents can synergistically affect the central nervous system (CNS). The effects of both ethanol and diazepam are likely to depend on the time of the day when they were administered. Diazepam influence on ethanol-induced sleeping and hypothermic activity in mice as well as the influence of combined administration of these agents on spontaneous locomotor activity and coordination in mice (rota-rod) were investigated. Experiments were carried out in the light phase (10:00-12:00 h) and the dark phase (22:00-24:00 h). It was shown that ethanol-induced sleeping time was longer in the dark phase than the light phase, and that ethanol increased spontaneous locomotor activity both in the light and the dark. Ethanol-induced hypothermia was lower in the dark than in the light. Diazepam decreased locomotor activity more strongly in the dark phase than by day. It impaired the hypothermic action of ethanol in the light phase, but did not have such an effect in the dark phase. Diazepam prolonged ethanol-induced sleep in the light phase, enhanced its action on locomotor coordination and decreased the stimulating effect of ethanol on spontaneous locomotor activity in mice. The chronobiological effect of the interaction between diazepam and ethanol seems to be of practical importance (sleep and motor coordination).

Comparison of pharmacophore cinnoline and quinoline systems on the basis of computer calculation and pharmacological screening of their condensed systems.

A series of new pyrimido[5,4-c]quinoline derivatives were prepared to compare the pharmacophore systems of cinnoline and quinoline. These compounds were obtained by the cyclocondensation of appropriately substituted 4-amino-3-quinolinecarboxylic acids 3 with acetic anhydride to the respective 2-methyl-1,3-oxazino[5,4-c]quinolin-4(3 H)-ones 4. These derivatives reacted with amines and gave N-3 substituted 2-methylpyrimido[5,4-c]quinolin-4(3 H)-ones 6. 4-Amino-3-quinolinecarboxamide 2 reacted with diethyl carbonate to give 1,2,3,4-tetrahydropyrimido[5,4-c]quinolin-2,4-diones 5. The SAR parameters of the derivatives obtained were analysed with the HyperChem 5.1/ChemPlus 2.0 computer program. The compounds synthesized were screened for their effect on the CNS.

[Survival of patients with diabetic nephropathy on continuous ambulatory peritoneal dialysis. Own experiences].

We analyzed 5 years survival on CAPD diabetics with respect to the type of diabetes (D). There were 24 pts with type 1 D, 13 pts with type 2 D and 29 non diabetic CAPD pts (for comparison of long-term survival). In the D groups we analyzed adequacy of dialysis, nutritional status (albumin level), frequency of peritonitis (PN) as well as character and progression of cardiovascular complications. Patients with type 2 D were older (58.46 +/- 8.54 yrs), had lower initial level of albumin's in serum (3.3 +/- 1.0 g/dl), more cardiovascular complications at the start of CAPD. Survival of 1st 3rd, and 5th year of CAPD was achieved respectively in 89%, 70.8% and 58.3% of type 1 D; in 100%, 69% i 38.5% of type 2 D; and in 96.5%, 72.4% i 65.5% of the control group of non diabetics. Despite similar adequacy of CAPD (Kt/V 2.14 +/- 0.62 vs 2.08 +/- 0.36; weekly CrCI 64.15 +/- 16.1 vs 62.08 +/- 12.72 ml/min/1.73 sq.m) and low ratio of PN (1:30.6 vs 1:32.7 ptsmnths) cardiovascular complications progressed in both groups of D, especially in type 2, being responsible for death of 3/24 pts with type 1 D and 8/13 pts with type 2 up to 5th year of observation. Because of progressive character of cardiovascular complications and high ratio of death especially in diabetes type 2 despite adequate dialysis, we recommend early referral of predialysis diabetics to nephrologist as well as earlier start of CAPD with simultaneous application for kidney or kidney and pancreas transplantation.

[Acute kidney failure in the course of rhabdomyolysis with hemodialysis in personal material from 1995-1999].

Eleven cases (5 F + 6 M; mean age 48.0 years) of acute noninflammatory renal failure (ANRF) in the course of rhabdomyolysis (RBM) were treated with hemodialysis in years 1995-1999. The causes of RBM were the following: ischemia of lower limbs after vascular operations (4 cases), exhausting exercise with rapid body cooling (3 cases), multiorgan failure after traffic accident, acute myositis (1 case), status epilepticus (1 case), rapid clinical course of viral infection (1 case). It was necessary to perform from 1 to 13 hemodialyses in every patient. In nine cases, complete normalization of renal function during 5 to 30 days of therapy was achieved. Two patients died due to multiorgan complications after vascular operations despite effective dialysis therapy. The following correlation were found: positive between initial values of creatine phosphokinase (CPK) activity and creatinine and uric acid concentrations in the blood and negative correlation between CPK and serum calcium concentrations. The higher initial values of CPK activity were observed the more hemodialysis procedures were necessary and the longer time was needed to normalize renal function. On the base of initial, limited up to now, own results it seems that hemodialysis in ANRF in the course of RBM should be started immediately in cases with high activity of CPK in the blood (above 10,000 U/L).

[Continuous ambulatory peritoneal dialysis: is long-term therapy possible?].

The aim of this study was an analysis of long-term results of CAPD. 12 patients treated with CAPD over 5 years were analysed retrospectively. The most common cause of end-stage renal disease, present in 8 patients, was diabetes nephropathy. In the 1st, 3rd and 5th year of CAPD the biochemical parameters, dialysis adequacy, peritoneal transport were evaluated. Despite the fact that adequacy of CAPD was achieved, the progression of cardiovascular complications occurred, especially in diabetic patients with type 1 diabetes. It is suggested that early kidney transplantation should be considering, before cardiovascular complications progressed.

[The role of peritoneal dialysis in intensive medical care: benefits and limitations].

This review analyses the advantages and limitations of actually existing technics of continuous peritoneal dialysis (CPD) which is proposed more often as the renal replacement therapy of acute renal failure in patients hospitalized in intensive care units (ICU's). Application of CPD for routine practice in polish ICU's needs the permanent disposable equipment for manual and automatic CPD as well as training the medical staff in the nearest nephrological reference center for PD. The last one is especially urgent, because growing population of pts treated with CAPD and APD in Poland. Some of these pts might be hospitalized in ICU's because of extrarenal complications. They need to be simultaneously treated with the technics of CPD used ambulatory before hospitalization.

Biomechanical testing of resorbable screws used for mandibular sagittal split osteotomies.

PURPOSE: The object of the study was to determine the suitability of specific resorbable screws for fixation of mandibular sagittal split osteotomies by in vitro biomechanical strength testing. MATERIALS AND METHODS: Resorbable screws (2.5 mm diameter) composed of a polylactic acidpolyglycolic acid copolymer were placed in an inverted L-pattern in overlapping urethane blocks representative of sagittal split mandibular surgery. In an in vitro model at room temperature, the test specimens were statically loaded until tensile failure occurred. On a different set of test specimens, dynamic testing was done in an in vitro water bath at body temperature through cyclic loads representative of mastication until failure. RESULTS: In static testing, three 2.5-mm resorbable screws sustained an average peak load of 131 Kiloponds (Kp) (standard deviation, 5.2 Kp) with 5.5% strain at yield. In dynamic testing, the resorbable screws tolerated a 45.3-Kp load for an average of 340,675 cycles (22,783 standard deviation). Several of these test specimens did not ultimately fail and were further evaluated by static testing with an average load of 77.4 Kp until fixation failure occurred. CONCLUSIONS: These laboratory results indicate a relatively high resistance to biomechanical loads representative of mastication and suggest that 2.5-mm resorbable screws of this particular polylactic acid-polyglycolic acid copolymer may be effective in fixation of the postoperative unrestrained sagittal split mandibular osteotomy.

[Own modification of the visualization of adhesions in the peritoneal cavity in patients treated with continuous ambulatory peritoneal dialysis].

The intraperitoneal adhesions were imagined with 3 non-invasive methods such as scintigraphy, ultrasonography, computerised tomography in 25 patients (pts) on CAPD (13 females and 12 males, aged 8-72 years). Initial imaging of peritoneal cavity was performed up to 4 months after the start of CAPD and repeated every 6-12 months of further therapy. In 9 patients the adhesions where detected at the start of CAPD. Only in 4 the progression of adhesions was diagnosed as a change of the structure from single to multiple or multilocular. In all these pts, peritonitis occurred at least once in the course of CAPD. Loss of ultrafiltration resulted in transfer from CAPD to HD. In others 5 pts the consecutive images of peritoneum were stable. In 16 remaining pts the adhesions were detected in the course of CAPD programme, in 6 of these pts after peritonitis and in other 10 without known causes. In 4 pts the progression of adhesions to multiple or multilocular was detected during next 2-3 years of CAPD programme. Two of these pts had to be transferred to HD. In 12 pts the images of adhesions were stable. Together, 6 pts (24%) had to be transferred to HD, because of the progression of intraperitoneal adhesions in the course of further CAPD. In conclusion, own modification of imaging of peritoneal adhesions let us not only to estimate the influence of this complication on the intraperitoneal dialysate distribution, but also enables non-invasive monitoring of CAPD programme.

Peritonitis in continuous ambulatory peritoneal dialysis: twenty years of center's experience.

Twenty years of our experiences in diagnosis and treatment of peritonitis in patients treated with continuous ambulatory peritoneal dialysis are presented. Special attention was put on reduction of frequency and efficacy of treatment improvement of this complication. Influence of peritonitis on peritoneal transport and dialysis adequacy was also analyzed. There were neither persistent changes of peritoneal membrane permeability nor worsening of dialysis adequacy following peritonitis.

[Continuous ambulatory peritoneal dialysis as a renal replacement therapy in blind diabetics with type 1 diabetes].

UNLABELLED: Controversy around the place of CAPD in renal replacement therapy of blind Diabetics (BD) with type 1 Diabetes induced us to start CADP programme also in this group of patients. In yrs 95-98 we treated with CAPD 5 BD send to our Centre in IV/V phase of diabetic nephropathy (acc. to Mogensen). Duration of D up to 20 yrs, complete blindness in the last 2-10 yrs. Criteria of qualification for CAPD: standard medical +motivation of patient and family helper for self dialysis, unaided everyday life as well as blindness not before maturity. CAPD equipment: twin bag system with special modifications for BD. Own training programme of 2 weeks for patient/family helper modified for BD (without visual aids). We achieved full clinical and biochemical compensation of uremia; KT/Vurea > 1.9, weekly creatinine clearance > 60 1/1.73 sqm, UF > 400 ml. Peritonitis occurred in 2 of 5 patients with ratio 1:28 ptsmnths. IN CONCLUSION: for BD with type 1 Diabetes, leading unaided everyday life, the blindness doesn't seem to be limitation for CAPD, if patients are motivated for self dialysis and the training programme has been specially modified for them.

[Levels of LDL,HDL and total phospholipids in blood serum of male patients with psoriasis].

The levels of LDL and HDL phospholipids as well as concentrations of total phospholipids were determined in the blood serum of 38 patients with generalized form of psoriasis (PG) and in 35 healthy volunteers of the control group (C). Patients with psoriasis were divided into two groups according to age at the onset of the disease. Group I (PG I, No = 11) were patients in whom the onset of psoriasis was observed before the age of 14.5, whereas group II (PGII; No = 27) were those in whom the first symptoms of the disease manifested themselves over the age of 15. A statistically insignificant tendency was noted towards a slight decrease in the concentration of total lipids, and towards an increase in the level of LDL phospholipids, whereas a statistically significant decrease was observed in the concentration of HDL phospholipids.

Subcutaneous implantation of human post-transplant lymphoproliferative disease lesions in SCID mice.

The tumorigenic potential of six post-transplant B-cell lymphoproliferative disease (PTLD) lesions was evaluated in SCID mice. Three animals developed local subcutaneous B cell tumours, two of which contained Epstein-Barr virus (EBV) DNA. Two animals developed CD3 positive thymic neoplasms, and one mouse developed an uncharacterized spontaneously regressing subcutaneous tumour. Immunoglobulin gene rearrangements studies with a JH probe, and EBV clonality studies with a Bam NJ fused terminal probe, showed only one mouse tumour to be genealogically related to the corresponding clinical lesion. It is concluded that lymphoid clones which constitute human PTLD are not autonomous, but sustained by host-derived growth stimuli distinct from those operating in the SCID mouse mileu.

Histopathology of renal posttransplant lymphoproliferation: comparison with rejection using the Banff schema.

The Banff schema for renal transplant pathology was applied to (1) nine specimens with renal posttransplant lymphoproliferative disease (PTLD) documented by Epstein-Barr virus in situ hybridization and analysis for B-cell lineage and (2) nine allograft nephrectomies classified as severe acute rejection (SAR) based on severe tubulitis, T-cell interstitial infiltration, and absence of Epstein-Barr virus. Tubulitis, venulitis, and infiltration of hilar soft tissues was demonstrable in all specimens. Hemorrhagic infarct-type necrosis was universal in SAR, but occurred in only three of the nine PTLD lesions (P < 0.05, Fisher's exact test). Arteritis in extrarenal vessels was more frequent (nine of nine SAR v four of nine PTLD) and severe (eight of nine Banff grade v3 SAR v four of nine Banff grade v1 PTLD) in SAR. Intrarenal arteries entrapped within PTLD showed grade v3 vasculitis in two cases. Expansile interstitial mononuclear infiltrates, nuclear atypia, and serpiginous necrosis were seen only in PTLD (nine of nine cases). It is concluded that the quality of the cellular infiltrate, its expansile nature, and the presence of serpiginous necrosis help distinguish between severe SAR and PTLD. Tubulitis and intimal arteritis, lesions regarded as specific for rejection in the Banff schema, do not have absolute discriminatory value in this clinical setting.
