[Neuromyeloencephalopathy caused by high-dose muzolimine medication in patients with renal failure].

We report on 7 patients suffering from chronic renal failure (2 females, 5 males; aged 35-75 (phi 53.5) years) who showed severe neuromyeloencephalopathy (NME) after high doses of a new Henle's loop diuretic, Muzolimine. The temporal and phenomenological development of these systems was strikingly parallel. The neurological deficit was revealed on neurophysiological, neuroradiological and in 2 cases on neuropathological tests (gross demyelinisation of the posterior column, mainly of the fasciculus gracilis, less in the lateral corticospinal tract and in some spinal roots). The critical drug dose for first neurological impairment was 52 g on average; at this point the patients had been treated for 78 days. The maximal daily dose was 1.440 mg. Dominant clinical features were pallhypaesthesia, ataxia, signs of peripheral neuropathy in combination with hyperreflexia and progressive para- to tetraspastic paresis. Constellation of symptoms, course of disease and findings of additional investigations, especially those of neuropathology, very much resemble Vitamin B12 deficiency and SMON-(Subacute Myelo Optic Neuropathy) syndrome. The rare entity of Muzolimine-NME is discussed in respect to other endogenous and exotoxic neuromyelopathies. We present the hypothesis of a toxic, partially dialysable metabolite of Muzolimine.

[Cerebral blood flow scintigraphy using 99mTc-HM-PAO in the diagnosis of brain death].

In 9 patients with the clinical diagnosis of brain death, a HM-PAO-SPECT was performed to document the arrest of cerebral circulation. Besides brain-stem areflexia, negative apnea-test and an observation time of at least 12 hours neurophysiological examinations (EEG and evoked potentials) were performed. In 4 cases, an additional arterial digital subtraction angiography proved the arrest of cerebral circulation shown in SPECT. In 5 cases, there was cortical low activity still present in EEG with negative SPECT. HM-PAO-SPECT seems to be a reliable noninvasive radiological method in the determination of brain death. It is especially helpful in cases with persisting focal cortical EEG-activity.

[Grand mal series and status using high-dose theophylline medication].

We report on two female patients, aged 22 and 50 receiving high-dose theophylline, 750-1,500 mg/day, for bronchial asthma, who were admitted to hospital with severe grand-mal seizures, in one primary focal with secondary generalisation. Extensive neurophysiological and neuroradiological diagnostic evaluation revealed no evidence of cerebral lesions, and there was no history of epilepsy. The probable pathogenesis is the high-dose theophylline, which is known to be neurotoxic, in one case enhanced by additional beta-sympathomimetics.

[Syndrome of painful muscle fasciculations. Case report and differential diagnostic review].

The case of a patient with spontaneous painful muscular activity is described. We diagnosed the muscular pain fasciculation syndrome. Biopsy of the sural nerve and electrophysiological examination provided evidence of the neurogenic nature of the disturbance showing a remarkable axonal degeneration. In addition, when immunohistochemical studies were performed, the detection of immunoglobulins and complement components suggested an immunological process in the etiology of the disease. The patient made a good response to carbamazepine therapy. This effect is found in various syndromes of abnormal continuous muscle activity. Clinical, neurophysiological and morphological features of this case are discussed in relation of pathogenesis, possible drug effects and related diseases.
