[Esophagojejunal anastomoses with the EEA stapler after total gastrectomy. Technic and immediate results apropos of a recent series of 32 cases].

32 consecutive total gastrectomies for cancer (20 extended total gastrectomies) were carried out through an abdominal approach. Reconstruction using a Rouxen-Y loop (28 cases) or an interposed jejunal loop (four cases) was done with an EEA stapled oesophago jejunostomy: 25 end to side and 5 end to end anastomosis were performed and two technical failures led to complete manual anastomosis. All patients have had post-operative X-ray examination: there was no anastomotic leakage in this series, even in the 3 patients (10%) who have died during the post-operative course. A critical study of this series and eight others one from elsewhere is presented: it is concluded that the stapling device allows an easier and safer oesophagojejunostomy than manual procedure, when great care is taken of technical details.

Effect of metal ions on the antimicrobial activity of 5-sulphosalicylic acid.

Bis-(5-sulphosalicylato)-diaquo chelates of VO(II), Cu(II), Ni(II), Co(II), Fe(II) and Mn(II) exerted antifungal activities. The chelates have been characterised for elemental, thermal and infra-red spectral studies and were found to be more fungicidal than the chelating agent. The antifungal activity was found to be in the order: Cu(II) congruent to Ni(II) greater than Co(II) greater than Fe(II) greater than Mn(II) greater than VO(II).

Chlormezanone-induced fulminant hepatitis in a pregnant woman: successful delivery and liver transplantation.

The case of a 29 year old woman affected by fulminant hepatitis during the third trimester of pregnancy, after a 3 week administration of chlormezanone is reported. Following induced Caesarean delivery, the patient underwent an orthotopic liver transplantation. The mother and her baby were in good condition 26 months after liver transplantation. In this case, chlormezanone was probably responsible for the fulminant hepatitis.

[Caustic rectal stenosis. Trans-anal resection using an EEA stapler].

An original treatment of caustic rectal stenosis due to abuse of analgesic suppositories is presented. The stenosis had been excluded by distal colostomy during 2 years before reconstruction of the circuit was attempted. Anatomically, the stenosis appeared as a complete diaphragm which could be recanalized by an EEA stapler for circular anastomosis, installed in two sites: endoluminal from the colostomy and perineal. After a 2-year follow-up, the anatomical and functional results are satisfactory.

Mechanically stapled esophagojejunostomy. Results of a prospective series of 176 cases.

BACKGROUND/AIMS: To evaluate the reliability of stapled esophagojejunostomy. MATERIAL AND METHODS: We studied a non-selected prospective series of 176 consecutive total gastrectomies (169 cancers, 7 benign pathologies). RESULTS: Hand-sewn esophagojejunostomy was performed 5 times after failure of the stapled esophagojejunostomy. There were fourteen hospital deaths (8%), and 63 patients (36%) presented complications. There were 5 anastomotic leaks (2.8%) but non were responsible for deaths. In these 5 cases, there had been an incident during construction of the esophagojejunostomy. Such an incident was the only significant risk factor for an anastomotic leak: 17% after an incident and 0% in the absence of an incident (p < 0.001). We observed no cases of anastomotic stricture. CONCLUSION: Stapled esophagojejunostomy is a reliable technique when technical precautions are taken. It is easier to reproduce than hand-sewn esophagojejunostomy and has demonstrated low specific morbidity and no direct mortality.
