[On work capacity certification among persons with nervous system dysfunction].

A new work ability certification procedure useful in the case of persons with the nervous system dysfunctions is presented. It is the opinion of the author that the identification of the type and the intensity of neurotic symptoms as well as of experiencing, behavioural and motivation disturbances and reasons for patient's reluctance to work is the most essential task. If often happens that not only patient's occupation and the kind of work performed but also a given workpost can ply a decisive role.

[Functional disturbances of the nervous system in the practice of occupational health].

On the basis of a questionnaire survey performed in the group of 80 persons, the author defined how the Occupational Health Service physicians conceive functional disturbances of the nervous system in their daily practice. The survey shows that they are not trained enough to be able to assess this kind of sickness. Their work is hindered by a lack of generally adopted definitions, classification of diseases and methodological guidelines how to define working ability of persons with functional disturbances of the nervous system.

[Evaluation of the nervous system in workers needed for preventive examinations. III. Indications for neurological consultation].

The reasons why a physician who carries out preventive examinations, including certain aspects of the nervous system assessment, should refer a particular worker to a neurologist for consultation are identified. Indications for the consultation by a specialist are divided into those resulting from taking a case history, aberrations observed during a physical examination and kind of patient's job. Indications most common in the day-to-day practice are discussed thoroughly.

[Classification of neuroses in the practice of occupational health physicians].

The author discussed current opinions on classification of neuroses. She stressed the diversified and complicated terminology which has been developed in this regard for the requirements of psychiatrists and statistics. The description of individual types of neuroses, useful for occupational health service physicians and based on a concise definition of major symptoms, is proposed.

[Evaluation of the nervous system in workers as needed for preventive care. Methodical indicators. II. Objective examinations].

A model of medical examinations useful in general evaluation of the nervous system was proposed. It is designed for doctors of occupational health services who perform prophylactic examinations but they are not neurologists. Technically simple elements of routine neurological examinations are selected. They are easy to perform and provide most observations which can be interpreted by a doctor who is not specialised in neurology.

[Evaluation of the nervous system in workers as needed for preventive examinations. Methodology. I. Subjective examinations].

A model for taking a case history useful in the preliminary evaluation of the nervous system is proposed. The evaluation is to be made by physicians/working in the occupational health services who are not neurologists. Guidelines how to take a case history are given and some questions are thoroughly discussed. The most important questions apply to the loss of consciousness and past diseases of the nervous system.

[Preparing physicians for indicatory physical examination of the nervous system].

A questionnaire survey was carried out among 104 doctors applying for the licence to perform prophylactic examinations in patients including the assessment of their nervous system. Responses of the interviewed on the proposed assessment method were evaluated in a scale from 2 to 5. The evaluation was compared in individual groups of doctors stratified according to specialisation, period and place of employment. In 55.8% of the interviewed the skill was found inadequate and it differed significantly in individual groups. Doctors without specialisation proved to be best prepared to assess the nervous system in patients and those with non-clinical specialisations received the lowest grade.

[Neurological assessment of health status in dentists].

A thorough neurological examination supplemented with electroneurographic and radiological examinations was performed in 31 dentists. A special attention was focused on the condition of the peripheral nervous system, particularly cervical roots, and peripheral nerves of upper limbs. All persons under study complained of lumbar pains. In 80.6% of subjects these pains indicated radicular neuralgia. Lesion of ulnar nerve was found in 22.6% and lesion of median nerve in 35.5% of dentists examined. The studies performed also allowed to detect some micro-symptoms.

[Certification of ability to work in patients after percutaneous diskectomy of the intervertebral cartilage].

Following the literature and own experience, issues related to the certification of working ability among patients after percutaneous nucleotomy of the intervertebral cartilage, are discussed. Certain premises are presented, according to which an operational effect and prognosis as to recovery, should be carefully assessed. Types of jobs contraindicated and indicated are identified.

Certification of health status and work ability of individuals with radicular pain syndrome.

Methods for diagnosis and determination of work ability of persons with radiculalgia were discussed following the literature and own experience. The author identified elements of neurological examinations, additional tests and consultations with other specialists which should best serve the purpose, and indicated the most common inaccuracies found in medical certificates.

[Problems with certification of work capability for people with symptoms of functional and organic diseases of cerebral vessels].

The problems of certifying work capability for people with the symptoms of functional and organic diseases of cerebral vessels were investigated basing on the documentation of 470 medical consultations performed at the Out-Patient Department of Occupational Diseases, the Institute of Occupational Medicine, Lodz, Poland. The certification was most difficult in people with angiogenic headache, symptoms of transient cerebral ischaemia and apoplexy with non-intensive deficiency signs. The certification criteria most appropriate for that group of diseases were formulated.

[Carpal tunnel syndrome of occupational etiology--new problems in diagnosis and certification].

The work is based on the author's own experience during her employment as an expert at the Institute of Occupational Medicine, Lódź, and on the literature on diagnosing and certification of occupational etiology of the carpal tunnel syndrome. The paper also discusses the applicability of the electrophysiological tests for occupational certification, as well as the significance of having adequate information about the workplace of the examined people and the problem of expected difficulties in evaluating exposure to the due variable structure of production and services.

[Opinions on health status and ability to work of persons with a history of subarachnoid hemorrhage].

This paper is based on the author's own experience obtained during her activities at the Outpatient Department of Occupational Diseases, Institute of Occupational Medicine, Lódź, Poland, as well as on the literature data relating to the methods of evaluating health condition in people with subarachnoid hemorrhage history. Factors affecting prognosis and their effect on the possibility of taking up occupational job have discussed. Jobs contraindicated for people with subarachnoid hemorrhage history have been specified.

[Etiopathogenetic factors of degenerative diseases of the spine and the effects of exertion and working conditions].

The work was based on literature concerning the etiopathogenesis of spondylosis and on communications estimating the effects of work conditions upon generating or intensification of degenerative disease syndromes. Traumas, micro-traumas, excessive static and dynamic workload are very important etiopathogenetic factors. The most frequently analysed occupational hazards comprise exercise body position at work, type of the movements performed, exposure to vibration. According to most authors, work conditions and ways of performing a given job may be listed among the factors that affect degenerative changes of the spinal cord.

[Cerebellar syndrome after long-term exposure to organic solvents. Case report].

A clinical picture of cerebellar syndrome with cerebellar atrophy revealed by CT scan in a worker for 28 years occupationally exposed to petrol and trichloroethylene vapours has been described. The relationship between the illness and the occupational exposure to the organic solvents has been discussed.

[Functional disorders of the nervous system in those occupationally exposed to mixtures of chloro-organic solvents and vinyl chloride].

A group of 334 males, workers of the Nitrogen Plant in Włocławek were examined: 172 subjects were employed in the department of solvents and vinyl chloride synthesis, 90 subjects in polyvinyl chloride processing department, and the rest of subjects formed a control group. Detailed neurological examinations made it possible to see that in the group occupationally exposed to solvents functional disorders of the nervous system of different intensities were observed (25.6%). The frequency of these changes in workers employed in particular departments characterized by different work conditions was compared, using statistical methods, with the results obtained in the control group. Functional disorders of the nervous system were found to be remarkably more frequent in the employees of the solvents production department where the greatest exposure to carbon tetrachloride and tetrachloroethylene were observed.
