Eight patients with cutaneous T-cell lymphomas (6 patients with mycosis fungoides, 1 patient with Sézary's syndrome, 1 patient with low-grade malignant lymphoma, unclassified) were treated with TP-5, a synthetic pentapeptide having the same biologic activity as the thymic hormone thymopoietin. TP-5 was administered three times weekly at a dosage of 50 mg subcutaneously in 6 patients, 50 mg intravenously in 1 patient, and 100 mg subcutaneously in 1 patient. Clinical evaluation at the end of the trial disclosed improvement in 4 patients (2 patients with mycosis fungoides, 1 patient with Sézary's syndrome, 1 patient with low-grade malignant lymphoma, unclassified), deterioration in 3 patients with mycosis fungoides and no change in 1 patient with mycosis fungoides. As TP-5 evidently exerts some effect on cutaneous T-cell lymphomas, further investigations of its therapeutic potential in this group of diseases seem worthwhile.
In five patients developing axillary contact dermatitis subsequent to the use of an antiperspirant containing propantheline bromide patch testing was performed. Three patients reacted to propantheline bromide. The other two patients exhibited positive patch test reactions to the proprietary product; in these cases, the active ingredient was not tested individually. In all five patients, in addition to the antiperspirant, at least one other contact allergen was identified. In case of localized adverse reactions due to topical treatment of hyperhidrosis axillaris patch testing is recommended. If contact allergy to propantheline bromide is revealed, one should be aware of cross sensitization to methantheline bromide.
In a 45-year-old woman with chronic urticaria anaphylactoid reactions occurred after (1) ingestion of coffee and (2) taking an analgesic drug. Prick testing in the patient and passive cutaneous anaphylaxis with coffee extract in the monkey Macacus nemestrinus were positive. With regard to further prick tests and oral provocation tests there is conclusive evidence that anaphylactic hypersensitivity to coffee was co-existing with idiosyncrasy to acetylsalicylic acid, indomethacne, metamizole (dipyrone), and caffeine.
Having removed a pigeon's nest, a 20-year-old woman developed pruritic skin lesions due to infestation by the mite Dermanyssus gallinae (De Geer 1778). Parasites from inside the nest are demonstrated by light microscope and by scanning electron microscope. As many mite species may infest humans temporarily, such parasites should be considered a possible cause in pruritic dermatoses with unclear origin.

Scabietic infestation resulted in a generalized bullous eruption in two 9 and 13 years old female siblings with epidermolysis bullosa hereditaria dystrophica. Various dermatologic conditions are known to be complicated or imitated by scabies.

37 observations on allergic contact dermatitis due to tromantadine are reported. With the increasing distribution and application of this adamantane derivative, the incidence of this type of clinical picture will probably increase further. The attendant physician should therefore accordingly explain to his patients and inform them of the initial symptoms - presumably striking intensification and local extension of the herpes eruption in spite of intensive treatment.
Bacteriological cultures of the ejaculates of 28 patients with acute unspecific epididymitis yielded in 6 cases growth of organisms which were considered to be the cause of the disease. Tests with conventional clinical antibiotics showed organism resistance in half of these cases. Because of possible contamination of the ejaculate with organisms in the urethra and in the preputial area, the results of bacteriological cultures of the ejaculate must be evaluated critically. But a bacteriological ejaculate culture should be carried out in all cases of acute epididymitis at the first consultation so that the antibiotic treatment can be begun immediately afterwards with a tetracycline preparation. If the cultures prove to be tetracycline resistant an appropriate antibiotic can then be substituted.
Ten subjects were treated orally with acitretin (Ro 10-1670) over a period of 3 months. Before, during and after treatment, semen and blood analyses were performed to investigate drug-induced impairment of spermatogenesis and the hypothalamic-pituitary-gonadal axis. Our study concludes that acitretin in therapeutically effective doses does not influence spermatogenesis, sperm morphology, sperm motility and the hypothalamic-pituitary-gonadal axis.
Ultraviolet (UV) radiation causes hemolysis of human erythrocytes in the presence of photosensitizers. This can be used as an in vitro model for evaluating photosensitizing properties of substances. Antioxidants such as ascorbic acid (vitamin C) and d-alpha-tocopherol (vitamin E) have been found to be photoprotective in such test systems. We assessed the effect of combined systemic intake of both ascorbic acid and d-alpha-tocopherol by human volunteers on phototoxic in vitro lysis of their erythrocytes. In a double-blind placebo-controlled study, 10 subjects took daily 2 g ascorbic acid combined with 1000 IU d-alpha-tocopherol, and 10 took a placebo. Blood was taken before and after 7 days of treatment, erythrocytes were prepared and then incubated with 10(-3) mol/l fenofibrate, a photosensitizer acting in the UVA and UVB region. The suspensions were exposed to radiation rich in UVA (up to 40 J/cm2 UVA) or to radiation rich in UVB (up to 1.6 J/cm2). Photohemolysis of the samples was calculated as a percentage of complete hemolysis. At the end of the treatment phase, in the placebo group photohemolysis was not significantly reduced compared with the initial values at all irradiation doses except for 1.6 J/cm2 UVB (96% vs 79%; P < 0.01). In the group taking vitamins, photohemolysis was significantly reduced at nearly all UV doses, most impressively after moderate UVA irradiation (20 J/cm2 UVA: 86.5% vs 14.5%; P < 0.01). It is concluded that the results of the photohemolysis test are influenced by the antioxidative status of the cell donor and that ascorbic acid and d-alpha-tocopherol also may protect against phototoxic damage in vivo.
'Mild' analgesic preparations (MAPs) are well known elicitors of anaphylactoid reactions (ARs). The identification of the causative agent is often difficult since reliable skin tests or in vitro methods are not available. In 128 patients (32 males, 96 females) with a history of ARs to MAPs containing only one single analgesic compound, placebo-controlled oral provocation tests (OPTs) were performed. Among 100 of them undergoing the OPT with the suspected drug, there was a concordance between the patient's history and the OPT in 44 patients. Concomitant ARs to other mild analgesics (without a history of previous incompatibility) were observed in 113 of 413 OPTs, but in almost all cases one or more alternative analgesics were tolerated in the OPT. Our results confirm that the OPT remains at present the most reliable diagnostic procedure alongside a comprehensive history in patients with ARs to MAPs. Because many of the patients showed concomitant ARs to unsuspicious analgesics OPTs should include a standard battery of different analgesics. The simple recommendation of one single preparation, such as acetaminophen, as a 'safe' substitute without an OPT is not recommended.

A 48-year-old male developed an induration of the skin of the neck a few days after a blunt trauma to this region had occurred. A diagnosis of scleredema adultorum (Buschke) could be established 3 years later. In view of the history and the lack of other diseases thought to be related to the elicitation of scleredema, a traumatic origin of the skin condition is probable.
