Percutaneous transluminal angioplasty of proximal subclavian artery stenosis after left internal mammary to left anterior descending artery bypass surgery.

A patient is described who underwent percutaneous transluminal angioplasty, through a brachial approach, of a high grade stenosis at the proximal portion of the left subclavian artery 1.5 years after coronary artery bypass grafting including left internal mammary to left anterior descending artery anastomosis. Symptoms of class IV angina, vertebrobasilar insufficiency and occupational arm claudication that developed after bypass surgery were promptly relieved after balloon dilation. Percutaneous transluminal angioplasty of the subclavian artery can be performed safely and provides an alternative to carotid-subclavian or axillary-axillary bypass surgery for treatment of internal mammary artery graft malfunction.

Acute myocardial infarction resulting from the migration of a Greenfield filter.

A paraplegic patient who underwent the placement of Greenfield filters to prevent pulmonary emboli had one of the filters migrate proximally to the junction of the inferior vena cava and right atrium, then into the right atrium a few months later. This resulted in an acute myocardial infarction by apparently causing an intimal dissection of the posterior descending artery. Treatment, follow-up and causes of filter migration are discussed.

Prolonged beneficial effects of dobutamine in refractory heart failure--case histories.

Two patients with refractory congestive heart failure were treated with dobutamine for forty-eight to seventy-two hours with close monitoring of weight and symptomatology. After two to four cycles of dobutamine treatment, 1 patient showed sustained hemodynamic benefit after six months, whereas the second patient had partial improvement. At the end of a one-year follow-up, both had sustained improvement in effort tolerance, documented by a stress test. The quality of life has markedly improved, and they have not required further hospital care.

Dual left coronary systems.

Dual left coronary systems, one originating normally and the other anomalously from the right sinus of Valsalva, is described in a 53-year-old patient with atypical chest pain with exercise-induced ST-depression in the anteroseptal area (V2-4). Clinical consequences of this hitherto undescribed anomaly are discussed.
