Inter-observer variability in ultrasonic evaluation of abdominal aortic aneurysms.

This study was undertaken to assess the consistency in 16 independent observations of the diameters of abdominal aortic aneurysms by diagnostic ultrasound. Confidence intervals were developed for the anterior-posterior dimension and the transverse dimension. Clinical treatment decisions should be made with appreciation that measurements of diameter increases greater than 0.21 cm in the anterior-posterior dimension and 0.31 cm in the transverse dimension are significant and are not a result of inter-observer variation.

Pressor effects of indomethacin in two anephric patients with chronic hypotension.

Two anephric patients with chronic hypotension were treated with indomethacin (150 mg/day). Indomethacin increased supine blood pressures from 63 +/- 4/37 +/- 3 mm Hg (mean +/- SD) to 76 +/- 5/48 +/- 6 mm Hg (patient 1) and 93 +/- 6/47 +/- 6 mm Hg to 112 +/- 8/61 +/- 5 mm Hg (patient 2), P less than .01. Similar changes occurred in standing pressures. The increases in pressures were associated with decreases in blood concentrations of prostaglandins 6-keto-F1 alpha, E2, and F2 alpha, measured by radioimmunoassay. These results suggest that indomethacin may raise blood pressure in certain anephric patients by inhibiting synthesis of extra-renal, vasoactive prostaglandins. Indomethacin may be useful in the treatment of chronic hypotension in anephric patients.

Absence of blood-pressure lowering effect of captopril in anephric patients.

Captopril, an orally active inhibitor of angiotensin-I-converting enzyme, was administered in a single dose to five anephric patients who had extremely low plasma renin activities. The drug did not lower blood pressure or significantly alter plasma renin activity in any patient but increased mean arterial pressure slightly in three. These results suggest that the blood-pressure lowering effect of captopril depends on an active renal renin-angiotensin system. Though additional mechanisms, such as accumulation of vasodilator kinins resulting from kininase II inhibition, may also contribute to captopril's action, these mechanisms apparently require the presence of the kidney to have a clinically detectable effect.

Renal artery dysplasia in a patient with membranoproliferative glomerulonephritis.

A 27 year old man with nephrotic syndrome due to membranoproliferative glomerulonephritis had multifocal stenoses of the renal and intestinal arteries. The arterial lesions demonstrated by angiograhy closely resembled those of medial fibromuscular dysplasia. The dysplasia progressed over a five year period to involve both renal arteries from their extrarenal segments through their interlobar branches. Low serum levels of complement components C3 and C4, focal reduplication of the glomerular basement membrane on light microscopy, and the patterns of glomerular localization of IgG and C3 by immunofluorescence were characteristic of type I membranoproliferative glomerulonephritis. The development of the arterial dysplasia in a patient with chronic glomerulonephritis suggests a common immunologic pathogenesis of both disorders.
