Acquired drug resistance in tuberculosis in Harayana, India.

Sputa of 200 treatment failure cases of pulmonary tuberculosis over a period of 1991-1995 were subjected to culture and sensitivity testing against commonly used anti-tuberculosis drugs. Out of 200 cases, 75% revealed resistance to one or more anti-TB drugs, resistance to isoniazid was observed in 72% cases, it was 49% for rifampicin, and 37% for streptomycin, while the resistance rate for other drugs was low. Majority of patients had resistance to two or three drugs concomitantly while resistance to 4, 5 and 6 drugs was of very low order and resistance to reserved drugs like kanamycin, ethionamide and cycloserine was encountered infrequently (1%). Out of 200 treatment failure patients multidrug resistance (MDR) was seen in 59% cases as 16% revealed resistance to isoniazid alone and strains in 22% cases were sensitive to all drugs. The study concludes that acquired MDR against first line antituberculosis drugs had increased as significant resistance against 3 drug combinations was observed although resistance against 2 drugs concomitantly was insignificant. Most ominous acquired drug resistance was seen against rifampicin in our region. The trends of drug resistance in the country and Haryana State are compared and their implications on outcome of chemotherapy are discussed.

Adenosine deaminase levels in cerebrospinal fluid in tuberculosis and bacterial meningitis.

Adenosine deaminase activity was measured in cerebrospinal fluid of patients with confirmed tuberculous and bacterial meningitis. The values were compared with those of control subjects without meningitis. A statistically significant increase in the level of this enzyme was noted in the two types of meningitis, but no definite demarcation in the levels was observed between the two types. Therefore increases in adenosine deaminase activity may not be of such diagnostic significance as reported elsewhere.

Resistant falciparum malaria in an endemic area of Allahabad (U.P.).

Over the last three decades the problems of resistance in P. falciparum malaria to chloroquine have emerged in many parts of the world including India. The present study was carried out at PHC, Jasra, an endemic area for malaria in Allahabad distt. to ascertain the level of resistance to chloroquine in P. falciparum in selected villages according to WHO extended 28-day field test. One hundred one (58.4%) cases were positive for malaria. Amongst the positive cases, 92 (91.1%) showed P. falciparum and 9 (8.9%) P. vivax infection. Of the 92 cases, 31 (33.7%) showed resistance to chloroquine and 61 (66.3%) were sensitive to the standard dose of chloroquine. Out of the 31 resistant cases, 19 (61.3%) showed resistance at RI level and 12 (38.7%) at RII level.

An epidemiological study of hepatitis B virus amongst blood donors.

The study was carried out on 186 professional blood donors. A detailed-socio economic profile of these donors was taken and the serum was tested for HBs. Ag. Majority (63.4 per cent) of the donors were in the age bracket of 30-40 years with an average age of 34 years. About 83.3 per cent were illiterate and belonged to the lowest rung of the socio economic scale. The average length of blood donation was observed to be 4 years with blood letting frequency of 4.4 within a stipulated period of three months. Most of them were anaemic with an average haemoglobin level of 8.2 gm per cent. The HBs Ag prevalence was observed to be 15.59 per cent. HBs Ag positivity was found to be significantly associated with the length and the frequency of blood donation (P less than .05). Remedial measures for prevention of viral hepatitis-B through professional blood donors was emphasized.

An electrophoretic method for the identification of poly-gamma-glutamyl chain lengths in folates and related compounds.

An electrophoretic method for the identification and separation of folyl polyglutamates of different chain lengths and of the corresponding p-aminobenzoyl polyglutamate compounds has been developed. These compounds have been separated using electrophoresis in a 40% polyacrylamide gel using a higher voltage and other modifications of the standard polyacrylamide gel electrophoresis procedures used to separate larger polypeptides. Good separation has been obtained on folates containing up to 12 glutamyl residues. Further, this method has been used to investigate the nature of the products formed by the gamma-glutamyl carboxypeptidases from hog kidney and bovine liver.
