[Incurable colorectal cancer. Possibilities and limits of differentiated therapy (author's transl)].

The different palliative procedures in incurable colorectal cancer are critically reviewed. In regard to the progress in operative and non-operative management the aims and possibilities have to be newly defined. The alternative, colostomy or enteroanastomosis, is unable to satisfy higher demands. This also means a tendency to aggressive resection therapy in colonic and proximal rectal carcinoma as well as combined local surgical and radiological treatment in distal rectal cancer. In emergency cases the minimal intervention necessary has to be planned in preparation for a more radical effort later on. The role of chemotherapy in colorectal cancer is still not precisely defined. In complications where surgical or radiological palliation is hopeless, the limited success of a combined therapy with 5-fluorouracil and MeCCNU seems justified.

[An analysis of factors which can be influenced in geriatric surgery (author's transl)].

A retrospective study has been made of 1457 surgical patients over 70 years of age in three one-year periods (1960/61, 1969, 1973). While the average age remained 77, there has been changes in the frequency of diseases leading to hospitalisation. Degenerative lesions and casualties have increased. There are more elderly patients undergoing major surgery. On the contrary the mortality has reduced to 7.8%. The results of geriatric surgery are influenced by previous and concomitant diseases, the surgical lesion and the type of therapy. Suggestions for reducing the number of emergency operations are discussed. When necessary smaller palliative operation which improve the quality of life should be considered instead of major curative surgery. Surgery in the aged is a speciality and requires simultaneous consideration of various factors.

[Duodenogastric reflux and stress ulcer (author's transl)].

Duodenogastric reflux in 14 patients in an intensive care unit was compared with that in 12 healthy controls. In addition, intragastric bromsulphalein concentration was measured up to 60 minutes after intravenous administration. Reflux was much more frequent in the intensive-care patients and correlated with the incidence of gastric erosions and stress ulcers. Reflux is apparently one of three factors (in addition to ischaemia and HCl) which synergistically lead to stress-induced lesions of the gastric mucosa.
